I stayed at Heartland for some therapy and nursing. The food was great, the room was clean, and my nurse Kristen was so friendly! Maggie is a good listener and leads a professional, highly trained therapy team. Maranda, Anna Blair, Sam, and Caitlin were all so nice and really helped get me up and moving again!

Don't use this facility! The rehab staff is great, most of the nursing staff are warm and wonderful however the administration cannot get anything correct. My mother was here and her rehab was great she made wonderful progress. The administration is a different story. For example, we had to beg for a family meeting for a treatment plan check which is supposed to happen two weeks after she was admitted. They sent a letter to an address we had not live at in years despite filling out all new contact information and us being there in a daily basis. No phone call either. After asking several times when this meeting would happen we were informed we missed it due to the preceding reason. It took at least three more phone calls to get one scheduled. One day they upset her terribly (and us) by mistakenly telling her she was being discharged. Once again the administration failed. They were discharging someone with her last name, just not her. Two weeks prior to her discharge they were supposed to arrange for a walker. There was a copay associated with it and Heartland gave them her home phone number to call for payment. I was able to straighten this out at the last moment, but was given too late of a notice to have any other options. I can't explain how infuriating it is to deal with their complete incompetence. Heartland sent her home with only written prescriptions no actual medicine despite assisted living requesting a 30 day supply. Then her insurance would not cover some of the prescriptions because they had been filled at Heartland too recently. What a nightmare. Save your loved one and yourself. Don't use this facility.

This place is disgusting! My dads room has one old chair with a large pee stain.. It is the only chair in the room. There is a small TV from the 80's sitting on a small TV stand where he is to put his clothes.He needs a proper chair that reclines, when I asked for one the nurse informs me that there aren't any available,The pee stained chair that he has now has a wooden back and looks like a kitchen chair. Our goal is to get him out of there as quickly as possible! My dad is diabetic and they bring him sweet tea with dinner.He is there for rehabilitation but they keep him in bed! He is there to learn to shower himself but his room doesn't have a shower! The staff appears to be over worked and annoyed when you ask a question or need their help.

My mother was in this nursing home because it was in our neighborhood so I could visit at least twice a day.The long term residents were put in front of the tv in the activity room for the whole day or they sat in their wheel chairs in the hallway.Activities were almost none.The food was substandard and they should have been ashame to serve the meals they did. My mother ended up being rushed by ambulance when a temp nurse took her vitals and realized what a horrible state she was in.The director told the ambulance driver they weren't needed that my mother was dnr which was highly wrong when it came to a treatable illness.In other words the director was playing God and making her own decisions.The elms did what was best for my Mom or she wouldn't be here now.I have nothing good to say about Heartland and would not recommend it at all.

My wife is in rehab right now at Heartland. It’s really nice. She likes it. The staff seems to be really nice. She seems to be happy there. She gets physical therapy and occupational therapy. She has somebody to take her either in a wheelchair or walk with her with a walker and gets three meals a day. The staff seems to be very helpful. I don’t have any complaints with them. The facility’s services seem to be adequate. It’s nice. Her room is good. She has the room by herself, which gives her a lot more room. It is OK. I’m satisfied.

My uncle attend this facility after having major surgery. He was making great strides at the hospital then all progress stopped after he came here. He grew more miserable and depressed with every visit. Then when he was finally given a speech value and could communicate again. He let us in on some of the things that were happening to him. We ended up having him removed from this place because he had to be hospitalized due to getting the pneumonia. He was NPO (nothing by mouth) as he had a feeding tube and his muscles were very weak and couldn't swallow food (everything went straight to his lungs). A staff member gave him medication by mouthw causing the pneumonia to set in. He was unable get around without help also. He told us after his hospitalization that he sat in diarrhea for 5 hours before he got help. He also had bed sores after attending here. When we were there we had him ring the nurses station just to see how long it took for them to respond. We were there over an hour and nothing. Please don't send a love one here if they are not entirely independent . Some of the staff were exceptionally friendly but others seemed just stressed out and too busy to care. Certain areas smelled strongly of urine and feces constantly. I realize nursing homes are severely under staffed but that is no reason to treat those who depend on you in their families absence so inhumane.

My dad had his rehab at Heartland Health Care Center. The physical therapists were wonderful and he enjoyed the people working with him. However, his biggest complaint was the food and it appeared that the people preparing the food were not from the South. The residents there were from the South and they were all complaining about the food. I'm sure it was nutritious, but it was not appealing to them.
