In 11/2020 during the COVID restrictions my mother had an accident and was hospitalized and released/ transferred to Evergreen House for short term rehab. She was there around 3 months before her passing. This was a difficult time for our family since we were not able to see her and she was a quad suffering from long term MS so communicating was a challenge. I want to publicly thank Evergreen and their whole team for the way they handled Mom's care and end of transition. From admission, to the constant communication with us, the care from the Doctor, Nurses, CNA's, Staff was wonderful. They helped improve our communication with Mom via ipads and video chats so we were able to see her progress and communicate with her daily. As the restrictions lifted we fortunately were able to be allowed in to see her face to face as her health turned for the worst. Mom and our family felt she was surrounded by truly caring people.

This is lenghthy but a true story My 95 yr old mither broke her leg 3/13/23 Her first Saturday at Evergreen at 7am an aide helped her relieve herself. 8 am she rang again as she had to go again. She rang, then her roommate rang for her. They had 2 aides working her floor, both stuck their heads in and left. By that time the room reeked. Some male therapist went to the desk for her. 3 HOURS after she first rang. My brother called me at 11 a.m. furious. They were just cleaning her up. We met with the head nurse and 3 others from admin. We wanted her transferred out of there. They told us the 2 aides didn't know who had her room???? Funny that we found out later that someone had assisted her earlier tgat morning. You stick your head in, smell someone sitting in their own fesces, the person asks for your help and BOTH left her for 3 hours without finding out which one of them was her aide?? One thing I can't stand is making excuses that turn out making no sense whatsoever. My mother cried, she did not want to go elsewhere as she had just gotten use to Evergreen. The staff asked for a chance to make it up to her so we regretfully dropped it. The next day the aide told her to let her know when she needed her because she didnt want to be accused of IGNORING her again. The 95 yr old told her that yesterday she HAD ignored her. Oh yeah I'm sorry. WTHeck? Did she think her too feeble to defend herself? Then they bill her for close to $6000 because she wasn't covered by Medicare after April 4th. No one ever came in with release forms or to tell us medicare would no longer cover her and she was now scheduled for unexpected surgery on April 4th because 3 days earlier someone at the doctors office removed her cast when they weren't suppose to and he couldn't get the bones to line up again. She would never be able to walk on it without the surgery. Upon receipt of the Invoice I called billing and sheinformed me that, at the initial meeting I was told Medicare wouldn't cover past so many weeks. And it wasn't her fault I wasn't given end of stay paperwork. Thats the social workers job. Her job was to collect money. On April 26th I called to find out how much longer she would be covered. At that time BILLING told me she was covered for 90 days. She transferred me to Social worker and she told me she was being covered until May 4th. At NO time did either one tell me she was already paying $200 a day. I was told she would pay $360 after the 4th. I called Blue Cross Federal because I had spoken with them previously and they mentioned paying 10 days longer than medicare. They told me not to pay the 5800 until I heard from them as to what we would owe after they paid evergreen. When EG-H called to collect I told her about my conv with Federal BC. She told me they didn't know she had BC. I told her I was pretty sure it was mentioned at the meeting she's so fond of reminding me of. Blue Cross sent paperwork as to what they paid and amount we owed was 3200. We get a bill from Evergreen around Christmas she owes 3800. She didnt want me to call and paid it in full to get them off her back. The next month she gets a statement, she owes another 200$$. She wrote a letter saying she paid in full and TODAY her and I get another past due statement for 200$$. I have not been able to contact them yet today so I am writing this instead hopefullly ill be in a better frame of mind when i do contact them. OH and to add insult to injury; she had accidents every day because every day they gave her a laxitive... OMG! ! Save yourself the aggravation and go elsewhere.

My aunt has been a resident of Evergreen House Health Center. The facility is very nice. When she started in this place, she was in a big single bedroom, but now she's in a 2x4 bedroom. And for what I'm paying, it's very expensive. The TV situation she has is inadequate. Although her personal care is doing a good job, I'm having problems with the laundry being returned back to her room at a sensible time. It takes them two to three days to get the laundry back to her room. It's just been a little crazy having to call up and find out where her clothes are, because I have to go out and buy new nightgowns and pajamas. I'm not really too happy with that. The staff itself is kind of unfriendly. When you can walk in and if they look up and see you, they won't even greet you. As far as activities, she plays bingo probably as much as she can.

Wondering caring staff that are dedicated to their residents! I would recommend Evergreen House to my friends and family.

I would not recommend this facility. It was a horrible experience to go and visit my family member at Evergreen, and even more horrible to think they had to reside there. If I could have transported her to a different facility I would have, but unfortunately it was not my decision to make. Please do yourself a favor and look into a different nursing home even if the location is convenient. I am sorry for anyone who has no choice and has to use Evergreen. I can only hope they make drastic changes in time for your care.

Evergreen House Health Center was recommended by the ALF that my mother was living in before. They are very good. The staff is very good. I like it most because it is closest to my home.

Evergreen House was extremely well kept. They have an A+ rating. The rooms were great and there are always a lot of staff around. They have a lot of activities going on. I had rehab for my knee as an outpatient. The outpatient rehab was fantastic, the best that I've ever had in any facility. The rooms were bright, clean and neat. They have individual televisions. They had a listing on the wall showing how many CNAs and how many RNs and how many orderlies were on duty at all times. The dining room is very pleasant, bright, and the food was good and always red hot. The overall décor of the place looked like it had been redecorated lately. They had bingos, Wii, movies, there's always a party going on for something, whether it be Valentine's Day or St. Patrick's Day where the residents can be involved in. They go for walks, for trips, and they have a little mini bus that takes them to the drugstore and other places. The grounds are immaculate. The parking is very convenient. Everything is very well organized.

Evergreen House Health Center was a beautiful and lovely place. But I thought that my aunt did not need that kind of sevice yet.

I was troubled when I visited a close family member at this facility because of many things I saw that I felt were inappropriate. Unfortunately, the stench of urine greeted me every time I walked through the front door. It took a long time for the CNAs to respond to my family member's needs and requests. There were never enough staff members on hand. The nurses seemed unprofessional and rude. The living arrangements seems unpleasant and the staff is unfriendly. The facility does not look clean. The workers frequently could be seen and heard making and taking personal phone calls on their cellphones. Several residents confided in me that they did not like their caretakers. I would not recommend this facility to anyone in need of a nursing home.
