My mom is at Pleasant Acres Nursing and Rehab Center. She is in the nursing home, although she has gotten physical therapy from there. The place is kind of a large brick building. When you go to the lobby, the receptionist is friendly, and I can't say enough about the staff who has been caring and kind. All through Covid, they have done Facetime with me every single Friday. I've been able to visit with her that way since I'm six hours away. They have just been sweet. One of the rec people who does the Facetime has helped me find sneakers for my mom. I have sent about six pairs of sneakers, and she has just been wonderful. She makes sure my mom tries them on. She also makes sure she puts some of her snacks away, so they don't get stolen. They have given my mom her meds. The food is good, according to my mom. She has gained weight from when she was really thin. She looks healthy, and she looks much, much better. It was excellent care. I can't say enough positives. They have a little courtyard where you can go outside and sit. I never observed activities, but when I talked to my mom, I would ask her if she's attended the activities. I know that they do exercise. They do current events. They do movies. They do games, coloring, and crafts. When COVID was happening, we went twice and saw her through a fence in the courtyard because they wouldn't allow visitors in. This past Saturday, I actually was on the unit where my mom was; so I actually got to see it. I've been in other nursing homes in the past, and they smell horrible. This one had no smell at all, and people were wandering around. They seemed pretty happy. Her room isn't super plush. It has two beds in it, and she has a bathroom in her own room. The room itself wasn't anything to write home about. She is paying with Medicaid at the moment. It may or may not be gorgeous or plush, but the staff are loving and caring. I can't say enough about the care that they provide, from the social worker, to the aides, to the recreational person, to the nurses, all of them.

Place is dirty, people sit in soiled diapers for hours. If you like chicken this is the place to go plus cold cold eggs. Wrong pills given diabetes meds when I wasn’t diabetic. Human bile was on toilet floor and wall of bathroom no one would clean it. To appreciate this place you have to stay here! Honestly wouldn’t let my dog there!

My relative went to this facility after having hip surgery for short term rehab.Management seemed clueless of her past medical issues, even with medical records. Relative did not receive the physical movement needed for rehab. My loved one is in adult diapers laying in bed all day. My relative was not bed ridden in hospital after surgery. Pleasant acres is just trying to make my relative an invalid so they can keep her. Not even private rooms, semi- private. Promised facetime visits with loved one, but have yet to have staff member call to schedule contact.

If I could give this place a negative number I would. My wife sat in a wet / soiled brief for hours until I went in and raised holy hell. They have 4 beds to a room, UNACCEPTABLE !!!!!

Overall, Pleasant Acres (PA) has caring and competent staff. Mom has been a resident for 2 years and though there have been some blips she receives good care. There have been many nurses, aides and activities staff or social workers that have gone above and beyond to make Mom feel cared-for and valued. In turn, Mom deeply appreciates her caregivers. We too believe that PA staff is under appreciated. However, even the most caring and professional staff can do only so much. If there is anything I would recommend it is that the facility employ more staff so that all residents are attended to in the quickest way possible. It would seem best that PA hire more staff directly, especially nurses and aides, rather than rely on agency personnel that seem less committed to resident care and have lower engagement as would be expected. We hear so much about our front line health care system heroes and we know several of them at PA caring for Mom daily. The doctors and nurses have been quick to help change any medical care that Mom has needed. They also provided great help to our family as we adjusted to Mom living at PA. Mom has made many friends with the staff and residents there and considers them family. This having been said, until COVID settles down patience is the key.

Worst place ever. They loose clothing, they loose belongings. The billing dept is so screwed up it's rediculous. Billed repeatively for sums I already paid. Case workers no longer work there and nobody informs you that your loved one now has a different case worker. Not that it matters who you speak to there anyway, because they all deny that you told them something anyway. The place is the worst ever and I will be getting my mother transfered elsewhere asap.

Took brand new clothes for mother and they lost it. My mother has dementia and I requested that phone calls from certain people not be given to my mom, but the staff gives the phone to her regardless of her calls. My mother was approved for Medicaid with no co payment and they still sent me a bill. When I called to ask why, I was told that administration never forwarded the Medicaid to their billing dept. The floors are always dirty. The silverware brought with meals is bent almost in half. I have my mother on a waiting list to go elsewhere and can’t wait until she can be transferred to a different facility.

Employees really do care. They want to provide good service. Grandparents lived here recently and were taken care of better than at another local place. Each place seems to have good and bad things. This one just got some bad publicity because of the sale. They do have 4 people in some rooms and 2 in others.

My sister was in Pleasant Acres. They're terrible, but I think they're the only recourse for a lot of people. It made me cry when I visited her. The workers there were very nice, but for the number of residents they had, they were understaffed. My sister wandered from day to day, and she wasn't even dressed in the morning. The facility was poor, they lost my sister's laundry, and people were stealing her clothes. There were four people in a room, and eight people shared one bathroom.

Don’t put your loved one in there. P.A. has gone down the tubes. My mom has been there for 6 years and I’m taking her out of there. Call lights are ignored, the food is terrible, I don’t think a dog would eat and served cold sometimes. House keeping needs to improve. Premiere Health took over in Oct 2018 and they’re terrible. They put money first than residents. They cut as many corners as they can. They have a room in there for smoking and sometimes the lobby smells like cigarettes! So gross.

They don't have training on how to lift a tall man and so they're rough when handling them from a wheelchair to a bed. They serve meals late if you're on the 4th or 5th floor. Everywhere smells like urine. They don't clean wheelchairs when they give them to another person. Our relative's wheelchair smelled like urine when it was given to him and they never bothered to clean it so I had to. They ignore residents because they're severely short staffed. Dangerous place to put a loved one in unless they have so many severe issues that they're paid attention to. Food is not edible. They also gave my relative no activities just because he was in rehab.

Love this place. My mother is currently residing there. They work well with family members and take such good care of my mom.

Their staff was always friendly and helpful when ever she needed extra help. They provided a very nice daily menu to fit her picky eating habits. They also had plenty of activities for her to get involved with from crafts to puzzles to movie nights, even special dinner and movie events. The doctors and nurses were quick to help change any medical care that she needed when we felt they needed rechecked. they also provided great family help and we as the family adjusted to the idea of her living there. she has made many friends with the staff and residents there and seems very happy. They also provide field trips for those who can go. which is great she can get out and see places and do special events with friend when we do not have the time to take her on our own with living such busy lives. they also provide two church services on sunday one in the morning and one in the evening. its great that she can still be apart of church activities without leaving her place of residence.
