We moved my sister to O'Neill Healthcare Fairview Park. It's family-owned. I have tasted their food, and it's good. They have a chef, which is good. The only complaint is they have steak and chicken in lots of ways, and for me, that's not necessarily good. They don't have trouble keeping employees. Everybody seems very happy. It's one-story, so everything is on one floor. They have a front porch where they can go out and enjoy the fresh air. It's really nice. The facility is very well maintained. I rated it the highest number. The staff is very attentive. Every time I go in there with additional questions, they're very attentive and are always available. Regarding activities, I don't think they have very many, because these people are in the final stages. They have parties, get-togethers, and movies and stuff, but they don't have many physical activities. They have rehab services in the basement, and they take them down by elevator. They accept Medicare waivers, so that's a plus.

family member in here for a fall and first 3days layed in bed , no rehab or excersise. staff came in on 2nd day , i asked when rehab starts, "well its thursday and its almost the weekend and ....." . the seven days family member was there, not 1 shower , even though i requested they do so. my brother and i walked each walked the family member down the hall ourselves, 2 different days, and the staff equaled that over following 4 days! four walks down the hall and one 10 minute session on arm machine, in 7 days, without 1 shower. thats the rehab program here. sad to say, but this really happened. i spoke with the admin, and relayed my feeling that the place is woefully understaffed, resulting in substandard rehab.

My wife was there for 25 days for rehab after a fall. Rehab department was very good. Food was okay. Housekeeping was terrible. Ran out of paper towels and it took days for an aide to at least bring in a roll. No one ever showed up to put it in the dispenser. Had to tear off hunks as needed. Couldn't get a box of tissues. On the last night I caught the nurse attempting to give my wife the wrong pills. When I questioned the nurse, she got very snarky with me. When I tried to inform her what pills, my wife should be getting, she told me to go back to the room and she'd figure it out. She eventually came in with the correct pills. My wife was scheduled for seven showers while she was there. Wednesdays and Saturdays. She got three. When questioned, it was always someone else's responsibility. Very disappointing! So glad she is out of there.

My father was there 4 years ago and it was top notch. When he recently had to go there for a quick therapy after hospital stay I must say I wish I had listened to other people and their reviews. Their PT and OT personal are still top notch and truly care. The food there is less then desirable, cold every meal and when asked for a yogurt for breakfast or a boost it never came. Sat there for 4 days since it was the holiday and then on the last day no food came and sat in his room with no OT or PT. I am his POA got lied to numerous times, from things they should of ordered for his release and forgot and it was no big deal to them. With the amount they bill the insurance this is terrible. NOT ONE PERSON / NURSE talked to him or his son regarding his medicine when he went home other then to say they sent it to store for pick up....

Awful staff there. My mom waited 2 hours for a grilled cheese sandwich and when she finally got it it was ice cold. Dawn the nurse told my mom she was acting like a drug addict when asking for her pain pill. Her room was never cleaned until we finally complained about it. They werent even washing my mom for days at a time. One aid came in smelling like weed and didn't even say one word to us when she came into my mom's room. There are people working there that really should find another field. Happy my mom is never going back there again. I wouldn't recommend to my worse enemy to stay here. But the PT and OT girls are amazing.

We have decided to go with O'Neill Healthcare Fairview Park for my mother-in-law. I was very impressed with the feel and the look of it. It seems very open, and it is bright-colored, airy, and very welcoming. The staff was quite helpful and very friendly, and she gave me a good tour. We talked about what they have available but I did not observe anything. They have a beauty salon.

I have been trying for several months to get a reimbursement for all the clothes that never came home with Emily. Her name was on all the clothes and with her being under 100 pounds, we sent her with the best clothes that she had. I spoke with Liz in your business department the second time in March who said that they were going to mail us a $150 check to cover the loss of clothes by the end of March or April, but we have not received the check. Emily was in your facility for 14 days in December. Initially, I had brought 5 outfits for her 14 day visit, but then had to bring up more outfits because she never received those back. When we picked her up, they gave us the big bag that we brought all the clothes in, but the only thing in the bag was a sweater that was draped over the top that we found out when calling the front desk that it belonged to a deceased person. I left a message early last week, but no one has returned my call. I called on May 18 and found out that Liz is no longer with the company, so this is two people now that promised to reimburse Emil for the lost clothing, but are no longer there. I was transferred to the new administrator named Megan who said that they found a secret stash of clothes and would call me back. It is now May 21, and I still have not received a call back. The most disturbing thing is that Megan said I may have to issue receipts on the clothes. My mother-in-law weighs 89 pounds. The clothes we sent fitted her well. Obviously, we are not going to have receipts since we were not expecting the nursing home to not return any of the outfits. The only thing that was in the bag that we brought home from the nursing home was a sweater draped over the top. We found out when we got home that the sweater belonged to a dead person and none of Emily's clothes were in the bag. I made a call that night and several calls for the first two weeks and was told that the housekeeping supervisor was out sick. I kept calling and not once did she return a call. I will be reporting this to the better business bureau and the Ohio Attorney General. I would not trust a loved one in there care!!!!

My Mom was sent here for rehab. According to Oneills records, my mom full coded on May 22nd 2020. They resuscitated her. They never called and asked her wishes or mine if a code took place. No one called and said, your mother died but we got her back! I guess that woulda been too hard?! Especially being that I WASN'T ALLOWED IN THE FACILITY!!! Shortly after, her pancreas started bleeding. (Probably from being resuscitated). They sent her back to Fairview hospital. The hospital did surgery, repairing her pancreas. They sent her back to Oneill. Discharge orders from the hospital said she was to get an enima and suppositories. Nowhere in her Oneill records does it say these discharge instructions were followed thru on? On June 1st, according to Oneill records, my mom coded again. Still! No one reached out to inform me or ask what wishes might be? Shortly after shes now vomiting vile? They send her back to Fairview hospital. Her bowel ruptured. And she still cant eat! They sent her back to Fairview hospital. They did surgery and repaired her colon. While in ICU, they thought she may be sepsis. They wanted to open her back up. My mom at that time declined anymore surgeries. She requested comfort care. I was not allowed in either facility due to covid. She couldn't eat...couldn't drink....she was literally full of ****!? Why was no one following thru with the hospital discharge orders? Why did NO ONE notify me that my mother CODED?? TWICE!!? When I went to the facility to read thru her records. There was ABSOLUTELY no records of her bathroom urinating & pooping? Just that she was unable by herself. She had a catheter. But no one followed up on the discharge instructions. Long story short.....my mothers COD was Perforated colon DUE TO severe constipation. Had ANYONE, SOMEONE done their job CORRECTLY!! My mother's COD might've just been sepsis. GET YOUR FACILITY AND RECORDS TOGETHER!!! These are ppls lives!!!

I found the facility and all of the employees to be very friendly, very clean. Everyone was caring and understanding. I would highly recommend this facility. Sure at times they were a little slow but that's to be expecting with the number of patients.

When you walk in the front door, fancy. No one around to greet or to ask for help. You go around the corner into the hallway.... alarm constantly going off, very few, if any staff in view. Meal trays were left across the room where my mom could not get to them. She was unable to feed herself, let alone get to the tray. My mother was unable to get up and stand. We were told to pull the cord for help, not to assist her. She stated she had to use the bathroom, I pulled the cord. Thirty seven minutes later and three people telling me "I'll get someone", mom could not hold it any longer, sat in a soiled diaper. I literally went up to a nurse for help, she said "I'll find someone"... I was fit to be tied. This facility is so understaffed. Only one aide appeared to care and was gentle and kind. PT and OT were nice, but the nursing staff and aide staff are not up to par. We moved my mom to another nursing home and the difference is like night and day. I DO NOT reccommend this facility.

Some things are not as they seem. Beautiful facility, Great Physical Therapy, TERRIBLE CARE.!! DO NOT EVER believe the Sales or Admissions department. We were told that our father with advanced stages of dementia would be well taken care of while he was there for rehab. They lied. Our father who we love so much had the worse care. I am not blaming the STNA's as they were short staffed. I am blaming management for not properly staffing this facility . The RN's could have helped more rather than sitting around at the front desk. My sisters and I all had to be there numerous hours per day. We also had to hire someone to sit with him a couple days a week as we wanted to make sure our father was not unattended for long periods of time. We had a sign of things our father needed. I don't believe any of them even looked at it. They would put his food in front of him knowing he couldn't feed himself. Leave him sitting in the same chair for numerous hours per day. He was put to bed with his clothes on sometimes, dentures in his mouth, hearing aids in. Other times left up extremely late and not put to bed. We weren't even sure he got showers when he was suppose to. They sent our father to a doctors appointment without a jacket or blanket on when is was 40 degrees outside. When we complained they said he didn't ask for one. Our father who doesn't even remember his daughters names anymore. If you really love your family member who is not coherent, DO NOT SEND THEM TO O'NEILL's, unless you plan on living with them while they are there. I will be sure to let everyone I know what a horrible experience this was for our family. O'Neill family be sure to change your "About us" "Family Tradition" and "Our Mission" as these are all lies.

My mother in law is in this facility. Do not send your loved one here. Poor care, very smelly place. I’ve never seen a nursing home with so many empty beds, that tells the real story

My mom was there for 4 weeks and we could not get out of there fast enough. I did not leave her alone for more than 3 hours and when I did a few times no one ever checked on her and they would not change her and I would come in and all her sheets would be wet. I felt as I worked there because I would have to wash her up myself or help the aides do it. Half the time, I had to take out the garbage from my mom's room. More than the few times, they gave my mom the wrong medicine and I spoke to the director of nursing and a few nurses regarding the situation and nothing was done. I had to call her doctor to get it resolved. I had to ask three times to order my mom's ointment and yet they never did. I had ask the doctor to put the order in. The staff is lazy and rude and they do not really want to help. They just stand around and talk. I was told they had a one to eight ratio, but when I spoke to the staff they told me its one to 16 ratio. It was one of the worse nursing homes I ever had my mom in. I even put a list of reminders so the aides will remember to wash and change my mom, but it was never done and that's why I had to camp out there. If I was not there nothing would get done. No one seemed to care and nothing was ever done.

Medical care was borderline incompetent. Nursing call button wait times unacceptable: more than 30 mins.. Medical director's staff NP lacks any bedside manner. Food is terrible.. Overall mood of the place was very poor. Building is very new: a shiny apple with a rotten core.

I was at O'Neill Healthcare for two weeks for rehabilitation. The rooms were absolutely spotless. There were a lot of people. It’s not like a hospital but it’s very equipped. The only complaint I had was the food. I didn’t care for it. It’s only been open a year ago. The rooms were comfortable with bathroom accessible for handicapped. They had a lounge and a TV. They check vital signs and check them throughout the night. They always check on me and that’s a big deal to me.
