My husband has been patient in the rehab program at Liberty. I am so impressed by the care he has been provided. It is hard placing a loved one in a facility but I am able to rest easy at night knowing he is been well taken care of.

Hands down Liberty Rehab is top notch. They have the most compassion for their patients and families. My dad was at Novant prior to his admission to Liberty, his care at Novant was horrible. When he arrived at liberty they immediately cleaned him well. We were so impressed with their TLC they provided within the 1st 30 minutes. Also the meals at Liberty are incredible. He really has enjoyed all his meals. All staff are amazing, but Amy really stands out and treats him like family. The cleanliness of the facility was one of the 1st things we noticed. Most Rehabs have a urine smell, but not here. Also my dads room is very spacious, easy to maneuver, his walker and wheelchair around. And to top all this off, they have a mini fridge in his room, where we are able to stock it with drinks, that he likes, and other snacks. Michelle the DON and Dawn the assistant DON are amazing. They have frequently rounded on my father to make sure his needs are met, you never see this at any other facility. My family feel very comfortable leaving him, we know he will get excellent care. Thank you Liberty Staff for all you do.

I have been a resident for almost a year. I receive daily physical therapy. The PT Dept. is excellent. The therapists are professional, knowledgeable and caring. Rooms are more than adequate for 2 people. There are TV’s in every room and land line phones available in the rooms. The rooms include a large bathroom with shower. Staff members are well trained, knowledgable and helpful. Transportation to appointments is available. There are several nurses and a doctor on the premises . There are many and varied activities such as trivia, exercise, bingo, crafts and outings. There are plenty of outdoor spaces in which to walk and sit. Resident interaction is encouraged. There is a wonderful library/lounge where residents can choose from hundreds of books to read or play games and socialize. There is a common area where residents gather to talk and/or watch TV. Overall I give this facility 5 stars.

This facility is being upgraded, rooms refreshed etc. The PT staff is wonderful, they are caring and work hard. The nursing staff is generally very good, helpful, caring and compassionate, no complaints during the day but at night, there is a huge problem. My mother would call for help and no one would come, she was unable to get out of bed and walk to the bathroom. She consistently soiled herself while there over 11 nights. I complained to the Director of Nursing (in person) who said she would come talk to Mom, she never did, I called her and left a message asking her to call me back, she never did, that's just unacceptable. The food is hit or miss, pretty bad most days. Not sure if staffing is the issue at night or what but care is 24 hours and ought to be handled that way. As far as cleanliness goes, the common areas appear to be cleaned regularly, the private rooms not so much, we had to ask for Mom's room to be cleaned, swept, bathroom disinfected, that can be improved as well. Be involved in your loved ones care, work with the RN to make sure the right medications are being given to your loved one. Overall, this is a decent facility with hardworking, caring people, it would not take too much to make it a much better experience.

I would not even send an animal here. Nothing but problems from the start. Promises to "try" to get medical equipment for the patient when they know they're not going to. They let a post surgical patient go for 8 hours without pain meds. Promised transport for doctor appointments and cancel at the last minute. These are motly administrative issues. The care staff for the most part try to help when their hands are not tied by administration.

My mother was in the care of liberty commons. She was only able to feed herself as she was physically and mentally unable to care for herself and was wheelchair bound. While in their care my mother had injuries to her body, I was never called about. I was her legal medical guardian. I noticed on one visit her eyes were bulging which may be caused by not receiving her prescribed thyroid medication. I asked the head nurse to check into this and call me with findings. My call was never returned and as always when ever you call the person you want is in a meeting and rarely if at all your call is returned. When Liberty Commons laundered her clothes they were constantly lost or destroyed. My mother was left with so few clothes staff told me they left her in bed all day because she had nothing to wear. One morning I was called at 2am by a nurse and told my mother fell out of her wheelchair. The nurse claimed she was fine and no problems. They would put a non slip pad in her wheelchair from now on. I asked when this happened, was told 5pm, I asked the nurse why are you calling at this late hour. I was told I just got around to it. She actually had a stroke. She was put back in bed and as a result of the stroke was unresponsive and unable to swallow therefore was unable to eat or drink. She was never taken to the hospital and left this way for almost a week. Finally taken to the hospital and then to Hospice.

Liberty commons is awful. Do not send anyone you love and care for there. Under staff. My mother said the girls were fussing in front of her instead of taking care of her. My mother was in urine for hours. Had sores on her private area. One of the assistants that was helping my mother was nice but the other assistant told her to just let my mother cry. My mother had a stroke. So you could only imagine. I would not send my dog there. And my mother was in her rt mind. She did not have dementia or any other diseases of the mind. She wanted my brother and sister to call the police. We took my mother out the next day and is doing great and happy. I feel since covid is going on and you can't see your love ones it gives them a opportunity to treat them any kind of way. I am so glad my mother is out of that hell hole. Thank you Jesus.

I have nothing but good things to say about this facility and its staff. Upon arrival the grounds where clean and welcoming with lots of colorful flowers and nice places to sit and rock, the entrance was inviting and always a smiling receptionist there to great you. My granmothers room was clean and decorated like a nice hotel suite and not a medical facility. The staff were very very nice and that was most importantn. My gma was there thru Christmas and they made her feel very welcom and at home and she was happy there.
