The facility is clean. The staff are friendly and helpful always. The rehab therapist are off the chain and do a wonderful job. The food is good according to my mom. it is an overall wonderful facility. My mom Gertrude was blessed to get to do rehab there.

My dad went to Azalea Health & Rehab Center. They moved his room closer to the nurse's station just so they can keep an extra eye on him. And they were very active as far as calling us if he falls, and calling us if they needed something from us. So they did an excellent job with that. Most of the time, it was nice and clean. There were a couple of times where everything wasn't great in his room as far as, dirty laundry, that type of thing. I met the rehab team and I like them. There were a couple of staff members that I did not like some of their answers when they talked to me about him not wanting to stay there. They were kind of ugly about it. The food was good. They let us bring our whole family in because he was there over Christmas. And they gave us one of their little party rooms so we all could get together and have Christmas together. So that was nice. In terms of value for the money, Medicare picked up the tab.

My mother has moved into Azalea Health & Rehab Center. They're excellent. They have very caring nurses and very caring aides. They do a great job. They keep it clean, and she's comfortable. They have a courtyard, and they also have a great front porch with rocking chairs and stuff. It's very good. They have a couple of courtyards. It's well maintained. They have a rehab side, and they also have the wellness side. It is split up with long term care and people who are not quite ready for a long term, but still needs the skill set. The PA is absolutely great. She keeps in touch with me and lets me know when medications change or lab results or whatever. I haven't observed anything different. I think that they are exactly on time with their medicine.

The people in Azalea Health & Rehab Center are very caring. When you call them, they get right back at you. My mom is there and they really look out for the residents, their welfare, and their safety. I've seen a part of the facility but I haven't seen the rooms. I just know that she's with a roommate. They're still basically locked down, but they do visitor appointments until they totally open up.

Azalea Skilled Nursing and Rehabilitation Center is a great facility. We'd recommend it. The people are great and the place is clean. They do what they see they're going to do. I'm very impressed with them. The food is really good and my mom likes it very much. They get their nails and hair done. They also have bingo and there's a library.

My grandmother was in this facility from Dec 12th-28th after a week stay in the hospital for broken ribs sustained during a fall. She is 89 years young. There is not enough competent nurses available to care for the patients. If it hadn't been for constant family involvement for my grandmother, I am scared to think of the condition she would be in today. Nobody was bothering to check her blood sugar. She is an insulin dependent diabetic. After her first few days when my mother realized that grandma's sugar had not been checked, my mother brought it to the nurses attention and the nurse had no idea that grandma was diabetic. My grandmother came on on Dec 28th after lunch. On the morning of the 29th, when my mother went to place a new lidocaine patch, she discovered that my grandmother had not had a patch put on since the 26th! These patches are dated and are supposed be put on in the morning and removed at night. That means that she was given the lidocaine patch on the morning of the 26th and that patch stayed on her until the 29th. After reading about lidocaine patches, instructions say that a patch should only be worn for 12 hours out of 24 hours. These things are disturbing for her family. Thank goodness at least one of us was around daily to check on her. Also, when she first got into her room it wasn't completely ready. The batteries in the TV remote were dead, the room smelled of sewage, the toilet wasn't working properly and the shower didn't work. The staff that we came across were friendly and polite. The PT department was great.

There was not enough nursing care at Azalea, so I had to do my own care. I don’t know why the nurses were picking on me.

My mom was here for a little over 2 weeks. Some staff were good and some were not. She was there following knee replacement surgery. My issues with this place were 1) CN were lazy and mom frequently waited a minimum of 45 minutes after ringing call button for help with body functions and it was worse on the weekends. 2)while PTs and OTs were wonderful people, but my mom only twice got therapy more than once a day the whole time she was there. 3) I believe their doctor was terrible as shows that my mom was rushed to the emergency room less than half a day after being discharged for extreme pulmonary distress due to fluid in her lungs. This was caused by inactivity, reaction to narco ( which mom and I decided on our own to start withdrawal and dr .'s refusal to give her her diuretic.. 4) the plumbing backed up and they could not flush toilets for a day and a half. The who place smelled like excrement. Don't send a loved one here.

Mom, who is 90 years old., has been in Azalea Health and Rehab just one 24 hr period, after a 4 day stay at NHRMC for heart problems. Today she was in physical therapy for one hour. Then left in a wheelchair for 2 1\2 hours. Repeated requests to assist her to bed were followed by we'll be back. I am now on my way to my Doctor for high blood pressure. Please pray for her and will have to see what happens. It's a beautiful place. However!?!?

Mom is in Azalea Health and Rehabilitation Center for about three weeks. The facility is very clean. Everyone there is very patient, nice and helpful no matter how long it takes to do something. They answer your questions time after time. The mom says they have very good food. They have private and shared rooms. Mom is in a room with a roommate. The lady she is with is younger than my mom. She is very nice and helpful as she knows that my mom is 95 with hearing problems. She helps her get the nurse down there and she tells my mom is her phone is ringing. They have enough room space. They each have their own TV and their own phones and they don't charge for them.
