I was visiting an elderly friend at The Grand Rehabilitation and Nursing at Barnwell. It was horrendous. I asked them if they had had a toilet overflow because the stench of sewage was so strong, and they weren't changing anyone's diapers. It's sterile from the outside, but the rooms were awful, and they had linoleum floors. The beds were covered with plastic. The room where they did the activities, which is also where they ate, had dirty socks on the floor. They had a TV, and some puzzles and boxes. One young man seemed to be very caring, but most people were on their phones the whole time.

This facility is absolutely disgusting, there were cockroaches all over when I was visiting my sister. When I asked staff to speak to the DON, they said good luck she does nothing. We don’t even see her if there is an emergency. There was 1 CNA assigned for the unit and 1 nurse, it smelled like urine, and was dirty when I went to see her before work. STAY AWAY!!!

My father went her for rehab. to strengthen legs. Only had physical therapy if lucky 2 times a week. Every time you call to discuss matters either no call back or get told short staff will look into it...etc. Last 3 weeks of being there he declined rapidly when called to talk to nursing or staff just get told he is fine. Finally had enough Sunday before he passed called left 6 messages to talk to head nurse, I wanted my dad taken to hospital, because of course supposedly she was on phone every time I called, received no call back. Sunday evening get phone call at 8 pm that the were transporting my dad to hospital for trouble breathing. That Monday had to place on Hospice due to UTI and bed sore on behind that had sent sepsis throughout his body. Passed away that Thursday.

Mom is not getting weekly showers. She is getting rashes from the sweat. Fungus infection that hurt burn and itch, under her breast. Im sad to hear her over the phone tell me these things. She needs more attention. Someone to dress n clean her.

terrible place for rehab. they do not have proper equipment. My brother is staying there until we can get him out. food is the worst!!!! for supper he is served cold noodles on a plate with no sauce or anything!!! someone should look into the dietary needs of these patients.everybody passes the buck around there and all the answers we get is I know, I know. Well they do not know at all .

Mother in-law passed away from a UTI that went septic in 2018. She had tested positive on a Tuesday. They never started her on antibiotics and she died the following Monday due to their neglect. Bathrooms reek of urine. Aides hang out at nurse’s stations on cell phones, food served on disposable plates. Very typical to have 2 aides per floor. That would be 2 aides for 50 plus residents. This is a horrendous, money making establishment that does not care about their residents.

This facility took patient to wrong doctor and listed his name as a different patient, whereby dr. put heart monitor on patient that didn't need one.!! Practically upon entering the facility, they had patient sign a form that changed his health insurance so he no longer had doctor, hospital and prescription but just prescription, when he had no idea what he was doing. This wasn't discovered until patient was taken out of facility for their neglect. Staff very rarely answered the call bell when rung and if they did it always took too long. The filth and urine smell in this place was enough to make you sick.

I'm ashamed of how bad the care has become.Years ago this place had excellent care and a better rating.Residents ring bells for long periods of time wanting help. when I was there at supper time bathroom bells would be ringing for a long time.I did speak to the D.O.N. she said things were getting better.WOW I would have hated to see things before they got better! J ust sad to see how poor the care is and the lack of staff.At one time there were some great CNA"S who gave excellent care to all the residents. My mom is a resident there now,which is why I'm very concerned not only for her but the other residents as well. when I call the unit staff don't even say their names WHY? I thought the state said no passing meds during meals but its being done. My mom dosen't need her meds with food but they were given more then once during meals.I really hope that things do improve esp with staffing as with only 2 aides on a 40 bed unit residents needs are not being met.I will continue to watch as long as my mom remains a resident there!

I visited this facility

Very impressed with the new ownership, proactive administrator is very nice. Overall this is a place I'd recommend anyone to visit, the atmosphere is very good.

Don't even think about putting a family member here. I have never seen such an unorganized facility. The don't provide the residents the proper medications let alone the proper diet. Their food is slops. I wouldn't feed it to my dog. When you ask the staff for assistance at any time they ignored you. The only good thing I can say is that we were able to get a bed quickly for our family member but now I know why. Their turnover is very high. So please avoid placing your family member here if possible.

I do not even know where to begin with the things that Barnwell has done that are totally wrong. I have had to call the state several times. I am currently working with the NYS Dept of Aging to help me get my Dad out of that facility. Last month they gave the paramedics the wrong info on my Dad and it could have effected his care at the hospital. Luckily I was able to talk to the ER doctor and correct the info while my dad was being treated. Last year they randomly started giving my dad (a diabetic) double portions of food which made him gain weight and had to have his meds adjusted. I kept telling them that this was inappropriate to give my dad this much food. Plus the fact that I would find my dad with a ham sandwich stuffed in his pockets hours after the meal. He could not eat it because he has no teeth. I had to work with them to get his food prepped so he could chew it! I also had to talk with them about monitoring him from eating off other peoples trays and eating food that had been sitting around or in his pockets and could make him sick! One time I discovered a patch on my dad and had to dig to find out what this was for. I had previously asked to be told of any medication changes. This one was for his dementia and it would have helped for me to notice changes in his behavior since I was the only one who could review changes throughout the years. Once you are in there it is very difficult to get out. I have been working for the last year to get my dad out. They will not be helpful in getting paperwork done for you to move your loved one out. I have to travel to see my dad and want him closer to me. I have many good nursing homes near me but can not make the transfer because Barnwell will not help you or answer any phone calls!! The staff changes so often it is hard to rate them. I have seen some good people there but it is difference faces all the time. The nurse manger on my dad's floor changes every 6 months. You have to ask yourself why so much turnover??

Have a Loved one that was sent to Barnwell for Rehabilitation & only being there 4 days, he's already had some clothes come up missing! He's spoken with many individuals & their response is: "We'll fill out a report and we'll look into it." Filling out a Report doesn't get one's underwear back! Their Physical Therapy (which is the main focus my Brother is there for) program, per my Brother, is lacking. There is no set time/schedule. It's up to the individual to go to PT themselves! Some may not be that motivated & just slack off from time to time. Be nice if the Physical Therapist came to your room, picked you up, & whisked you off to PT. That way you know for sure you have PT scheduled! My Brother is supposed to go Monday - Friday, 2 times a day! And like many other reviews I've read: This place has many staffers that 'just don't care!' Which probably due to understaffing & being under paid! Example: Brother knows of an older Woman there that has Alzheimers & he's mentioned it to the staff of her constant screaming & the staff says this: "We know, she does that a lot." AND they ignore her!!! That is someone's Mom/Wife/Daughter/Aunt for crying out loud!!

Barnwell seems like a very nice place considering it’s a nursing home, with pretty good food of very good-quality meat, vegetables, and fresh green salad. I was very impressed. I only visited the rehab floor, and the accommodations weren’t great, just very minimal with little closet space. The staff seemed competent and friendly. They had good activities, but my friend didn’t go to any; I remember they have salsa dancing. I was there during a medical emergency, and the staff was extremely capable and proactive.

I am/was a resident of this facility

One thing I did not like about Barnwell Nursing & Rehabilitation Center is that they were two-bed rooms, and the girl in the next bed was sick and kept having seizures. Her parents were there, and they slept in her room. You couldn't even turn around in that room. It was not meant for four people, two of whom were not patients. I did not enjoy being with two other people in the room when it was meant only for two. I liked their physical therapy. They had at least fifteen therapists, a big room, all kinds of machines, and they were very good. I think their accommodations were very small. It would have been hard for two people staying in there.

I visited this facility

It was not a very good experience for my Mother who was there for 3 weeks for rehab for a broken hip. She was put on the 4th floor which was a long term floor. She would wait forever for someone to help her to the bathroom. One time a bed pan was given at 11pm it was left under her til the next morning. One morning she awoke to another patient hitting her in the head with a stuffed animal. Very depressing place that is under staffed. The staff all act like they don`t care. Would never reccomend this place to anyone.Visit place before sending loved one.

My grandmother stayed here for a bout 6 weeks. I visited every day after work and found the staff to be helpful and pleasant. The facility was always clean and there were many choices for activities available to the residents. Any time I brought my grandmother to an activity, I was always invited to stay. Most of the residents, including my grandmother were happy most of the time. There were religious activities available every day which was very important to my grandmother. Those activities included hymn singing and bible study. She was even able to invite her bible study group from her church to visit once a week. Since she was the one who had previously hosted this weekly bible study group, it allowed her some continuity to a familiar schedule. As she approached the end of her life, family and friends were able to stay with her 24 hours a day. Visitors had to come and go according to their own schedules, but the numbers were usually 3-10 people at a time. She was very happy to be able to die in the company of friends and family. I think she had far more visitors than would have been possible if she had spent her last weeks at home.
