I am a friend or relative of a current/past resident

Absolutely disgusted with the quality of care, lack of integrity and lack of accountability displayed these past few months! Attempts to reach out to anyone regarding these ongoing issues have cyclically fallen upon deaf ears! My dad was transferred to this facility less than 3 months ago in efforts to help his recovery as I’ve made it clear that “ We want him back”! Instead of him receiving adequate care towards recovery, he had been neglected to the point where he has had to be sent BACK to the hospital 3 times in less than 3 months for severe blood loss and life threatening bacterial infections. The day before he was readmitted to the hospital last week, i went to visit him. He was in agonizing pain and they had his heels stuck to a pillows oozing blood and puss from the latest pressure ulcers without any type of gauze, bandage, heel protectors or anything. When I mentioned this to the Nursing Supervisor, she looked at me and said “Okay”. Still waiting for a response, I was told the night nurse will come in and take care of it upon shift arrival. Next day, I went to visit and the room smelled like blood and secretions only to see that his feet were never tended to the day before and now had gotten even worst. Went to readdress the concern only to find out that the Nursing Supervisor there had been there from the day before, everyone called out and there was no type of coverage. My dad has developed a horrible and debilitating sacral wound during this recovery journey that seems to get severely infected causing not only loss of blood but now also bone infection due to him not being turned and the wound not being properly cleaned and dressed daily. The hospital physicians are even taken back each time my dad arrives asking “Who is looking after this wound”! Each stent back to the hospital, I am by my dad’s side everyday for the maximum amount of visiting hours allotted in efforts to help him heal faster. I find it appalling that each time he gets stronger, he is sent back to the rehab with hopes of getting even stronger only to decline immediately barely making it 2 weeks to the point where we have to build his strength and baseline health back up. Have yet to meet his “social worker” after the family’s numerous attempts from day 1. Now he has a new social worker, asked for The Patients Rights and Responsibilities packet, have yet to receive that. Set up a home health assessment with the State nurse to evaluate my dad for 24 hour home health care, finally got ahold of the new social worker for her to tell me that in her experience, these assessments take a few hours and she refuses to be there for the duration. How rude and unprofessional!!! I’m expressing my dire need to SAVE MY DAD’S life by removing him from your facility and you don’t even have the time to do your job. There is no one there that can represent you? Or maybe this isn’t the job for you! My dad is far from an emotional person but damn near begs me to take him with me each time I have to leave. My dad has acute conditions that require acute care and these people just don’t seem to have the resources to accommodate his needs. The fact that I am purchasing medical supplies each week heel protectors, knee braces, gauze and washcloths is beyond logical to me. I ordered sheets and gowns as the last set delivered to the room had spots of blood and whatever else on it. Beyond this and plenty more, my dad’s overall confidence, emotional state and mental strength are now being challenged and I’ve had enough!!!! Why would you treat my dad like this? Do you think an occasional smile, short small talk convo or a quick “how are you” is sufficient enough in tending to his care! He is unable to fend for himself, unable to move his hands oh and now, unable to speak! My parents are valuable to me and regardless of what happens moving forward, I WILL NOT REST UNTIL THIS PLACE AND EVERY CONCERN IS INVESTIGATED AND RECTIFIED! Please, be careful if your loved one needs a place to recover. In the middle of a worldwide pandemic, these people are only about the money.

There were very nice and spacious rooms. My cousins mom came there on a vent during the pandemic. She had covid and had to get a vent at the hospital. She was now discharged from them and was able to walk out herself! it was beautiful.

I am a friend or relative of a current/past resident

If I could give it NO stars - I would. This place is horrific!!!!! My mom suffered from EXTREME bed sores on different parts of her body. She literally had open wounds/holes the size of a fist- there was very little to no care for her. I guess because she was non verbal- they let her lay there and rot to death- literally! They would not allow us to go near her to see or touch her when we were finally able to visit- and made sure she was covered so we were unable to see how badly things was. I can go on and on about the neglect! PLEASE I BEG YOU- If you care for your loved ones DO NOT SEND THEM HERE! No person should be neglected in this manner! It is unfair, inhumane and down right cruel!!!!

I visited this facility

Had to share moms last few weeks as the mom we knew and loved dearly, yes she passed away April 23rd from complications that started at this facility, mom was a patient here to gain her strength back after a hospital stay. We trust the professional to follow up on medication & history. On Jan 18 mom had a stroke due to failure on someone's part of not being on her Coumadin for her entire stay & even after we expressed concerns for change in her mental state, oh yes their in house psychiatrist gave her Xanax which threw her into a comatose state. Then we demanded she be rushed to the hospital, andvyes she had a stroke and life threatening potassium level and needed immediate dialysis. The battle from here was horrendous forvwhatvshe endured. Her last few weeks at this facility when she was able to communicate She was given cold showers, she told me I'm getting use to the cold water every day it's my first time having to be bathed in cold water, the day of her stroke it was finally addressed after we went to administration. Their reason was there's no warm water at that time of day & we'll change her bath time, well mom never got to feel a warm watered shower or bath, because we trusted these professionals to care for her . Even after contacting administration to advise them on her status our family was never contacted to even ask how mom was. Thank god we were able to advocate as best we could for mom, imagine patients that don't have family to help. Administration should hang Their heads in shame!!

I visited this facility

South Shore was so dirty and filthy on the second floor, I couldn’t leave my mother. I didn’t like the layout, and the rooms were dirty. The people made me think they were uncomfortable.

Run as fast as you can!

The facility is dirty, the nursing staff lack professionalism and compassion, and the admission staff leave a lot to be desired. Both clinical and non clinical staff walk around with mask on. The only bright spot are the Physical Therapists They are the only ones that care about the patient experience. If by chance your loved one is there, get them out as quickly as possible. Be prepared however to deal with a corrupt Admissions personnel. They will do everything they can to delay your transfer.

I am a Nurse, whose Mother was in this facility, aides complained all the time about toileting, bed alarms were not responded to in a timely manner 6 minutes? Too late! Improperly dressed without bra, never groomed, hearing aids left out, wheel chairs left unlocked, aggressive manner in transporting patients in wheelchairs, Mother almost fell out and was stunned by aide, who was abusive and angrily jerked chair backwards then forwards, never announcing herself, creating fear in mother, then disputing the incident after I stopped her and told not to assist my other again! Aides not aware of patients needs thoroughly, don't like their jobs or the patients. Dislike communicating or answering questions at all. NOT HELPFUL!! Have attitudes. Very uncomfortable for family and patients, not safe!!!

This facility was old and decrepit. The help was surly at best and the only attention we got was when we yanked him out after less than 24 hours there. My father had been in the hospital after surgery on his brain. He had many issues and he was put in a room at the end of a long hallway and left there all night with no assistance. I stayed myself just to make sure he was safe. There was yelling and screaming in the hallways with no attempt to quiet or assist the people who were screaming. I would definitely stay away.

I am a friend or relative of a resident

My father was admitted to this nursing home from South Nassau Communities Hospital. He was having problems maintaining his balance. On the first day I visited him at this facility, I noticed how well kept the outside grounds were. When I entered the nursing home facility, I was greeted by a receptionist that instructed me to sign in and she directed me to my father's room. The nursing staff was pleasant enough and they always encouraged my father to participate in the community activities which included bingo and crafts. My dad is a very picky eater and he even enjoyed the menu options that were available to him. The choices sounded like something you would find in a 5 star restaurant and menus are tailored to the individual needs of the patient. He went to rehab twice everyday and the therapists were very nice to me and explained his treatment. My father's condition improved quickly and he was discharged in two weeks. The discharge nurse and admissions staff made sure he had everything he needed to continue therapy after he returned home. Great staff, great facility. Highly recommended!
