Samantha the social worker was so helpful with so many problems we were having with Managed Care Agency and the Home Health agency so that we could get our loved one home. She went above and beyond to help us during an extremely difficult situation. The nursing staff and the oldest staff members were very pleasant and kind to us and understanding. The rooms were accommodating however the bathrooms were a little small. But the staff was amazing and made do with what they had. Much much better than another facility our loved one was in in the local area. Just 6 months prior in August of 2023.

We liked Brookhaven Health Care Facility LLC. It's a lot cleaner than where my father is at. Everybody's friendly, nice, and respectful. The structure of the building looked fine. The rooms were small but were laid out OK. There were double rooms or two occupants in a room. The atmosphere seemed nice. There were a lot of visitors visiting residents.

Rooms are ok, my father in law did not like the food. Nurses seemed to take care of him, but something happened with docs not assessing him correctly. He did not like it in there and wanted to be moved to hospice, but diagnosis was all messed up so he ended up staying in there until he passed. which fills me with guilt and regret, so dont use this place to avoid putting yourself through how i feel now, if your loved one is in bad shape near the end.

There are many opportunities that my husband does not avail himself of so some of my reviews are based on observation. I find the staff caring, friendly and professional that includes the nursing staff, Activities, Finance, Front Desk, Maintenance. When you enter BHCF not only the patient but their families are welcomed and treated like family.

Rooms: dirty with bugs flying around. Staff: lazy, did not practice safety when handling my mother. Hurt her on many occasions. Never used regulated belts when lifting her and caused skin being torn on her arms. She was also threatened and very scared of saying anything against anyone. I would not put my dog in this place. If you love the one you are putting in here , move them. I had no choice on my mom being in as other family members put her in here. My mother's stay was sad and frightening. There where some staff that where good at what they do but unfortunately a lot of incompetence from the head nurses to staff. State comes to investigate and they have code words to let the staff know they are there and to follow procedures. State is incompetent also should put people under cover to see it on a daily. They will respond with a generic answer like they really care. Well if you do fix it.

This facility is terrible! Stay away! My father-in-law was almost killed. He was finally sent back to the hospital. His lungs, legs and ankles were full of fluid. His basic medical needs were not taken care of.

I really like this rehab place. They took really good care of my mom. They have a lot of fun things to do and celebrated holidays with the patients.

The maintenance throughout the building was below standards. There was mildew in the rooms, ceiling had leaks, beds did not work correctly and the bedrooms and bathrooms were unsanitary and urine odors were abound. Wall paper was pealing all over and the rooms were cold with little to no heat. The rooms are small, crowded and closet space is below average. The aides for the most part were nice but there were few to be distributed and therefore the patients lacked the care they deserve. Patients seem to linger and were not cared for property or by standards. they are put in wheel chairs and forgotten about. Some patients screamed all day and night and were disruptive to others. Not a place I would sent my family member again. Place seems very unorganized and not a caring facility.

Brookhaven Health Care Facility was not good. It was dirty and smelled. People who couldn’t get around were put in wheelchairs and put outside the room. The rooms were little boxes, almost like cells. What I saw was that it's all about the money; they just collect the money and pay their staff very little. I wouldn’t recommend it.

Brookhaven's food was a little bit better. My mother was able to eat more of the food because it was more palatable. They did attempt to meet her dietary needs too. During that period, she was in four or five different nursing homes. She was going to the hospital, and then they would transport her to a nursing home. Then she would go back to the hospital. She actually contracted a superbug in this nursing home. She kept going back and forth, so in that time it was whatever nursing home that had a bed to take her. So I can't remember what the rooms were like, but I do remember specifically they were meeting her diet much better in there. It seemed clean, but that can be deceiving because somehow she got this superbug in there, and she had it for a year. I think the staff should be more cognitive of safety on the spreading of germs. Otherwise, I don't believe she would have gotten this.

Brookhaven Health Care Facility is a good facility. I'm connected to the church, and I do visitation for the people. They have a good staff. The facility is well kept. It is a convalescent care and rehab. Actually the reason I know it better than just what I said is that my father passed away in that facility almost 20 years ago. He had dementia. I had talked to the staff because they put up with a lot. Dementia patients can be crude, and they can be difficult and stubborn. It takes a rare breed of person to deal with those people on a daily basis.
