My father was in Buena Vida Center, and that's where he fell and hurt his hip. He was there right during the height of COVID, so we never got to really go inside to see anything, and we didn't ever get to do any visits. I had one window visit, but this was not long after he hit his head, so he was super confused. He was refusing everything, so he didn't want to come down for visits. He had a feeding tube. He wasn't eating through his mouth. He was pretty miserable then, but the people were very nice, like the director and the speech pathologist. They were responsive to phone calls. Unfortunately, his accident happened in the wee hours of the morning -- like three in the morning -- so I wasn't familiar with any of that staff. It's hard to reach people in those hours, but the day staff was good. The facility seemed clean. At the time he was there, they looked like they were doing a lot of construction inside the building, so I don't know if they were remodeling the reception area. I didn't go further than the security desk. I can't really say how the inside of the building looked. It was right when COVID was really high, so everyone was super protective. I did not see any outdoor areas other than the patio area where you walk up to the steps of the building. I don't think it was a bad place. It's just that he fell. It looked well maintained from the outside, and it looked really clean from the pictures. I spoke to someone there before I put him there, and they seemed very helpful and responsive to what our needs were at the time.

I come here for temporary services after injury. The doctor doesn't follow discharge prescription(s) orders; meaning change the prescription to what he/she feel the resident should have, or doesn't give the required medication. Some of the staff are very unprofessional, and show extremely low job performance. Housekeeping does poor job performance, and state they don't get paid enough to do great job performance. At night, a resident cried out in pain for help, and the staff ignored her, or tell her clear food out her own mouth. I felt so bad, and upset. Everyone whom is able, must use one shower room, and it doesn't be kept cleaned (sink, shower floor, toilet bowl and surrounding areas. I am able body person, and I speak up when I see wrong doing. The staff doesn't like that, and now I receive the rudeness and disrespect. Imagine your family member(whom is unable to speak for themselves is here... The owner needs to send someone here "undercover" too see for themselves. Not all staff are bad, its just a few

If I could give zero stars I absolutely would. My father has stage 4 cancer and was there for a few weeks after coming out of the hospital. We were told he would be cared for and they would help to rehabilitate him to gain some of his strength back as he basically had none left. Instead he was kept in his room all day and we were not allowed to speak to him when we called. We were always told he was in his room sleeping. My aunt went there after nurses refused to let us speak to him and told us that the place was a mess due to construction going on and that security wasn’t even wearing their masks. We took my father out of there after finally getting in touch with him and learning that one of the nurses hit him. Disgusting people, shame on all of you.

Truly amazing people. Im so impressed by how, from the minute you walk in till you walk out, everyone id attentive and caring. Everyone was so knowledgeable and helpful. I wouldn't trust any other place with my loved ones. The place is clean and I loved the activities set up.

At buenavida Rehab I can say that the furniture is In an attractive manner at the home.

I was really impressed with how clean the place was. I also really liked how loving and caring the staff is and they really know their stuff.

my mother is a resident. i live in another state and cannot reach anyone on the telephone (they all have the same number). I am not contacted about any issues. Her clothing has disappeared...every piece. been trying for two days to get in touch with someone. starting to regret the decision to place her here.

My father has 24-hour care at Buena Vida Center because he is oxygen dependent. He is comfortable, and he wants to be there. At first, he didn't want to be where he was at, but then he got to meet other residents and just got used to it. He is adapting, and he is getting used to the nurses.

My mother was in Buena Vida before. They were nice, and I liked them. The only problem she ever had was that when she wasn't feeling well and they were trying to make her do physical therapy. The good side of it was everything that happened to my mother, they were quick to call me. Like the first morning she was there, she fell, and they called me at 6:30 in the morning to let me know what happened. The staff was friendly and nice, and they all loved my mother. It was clean.

My father has had a very good experience at Buena Vida. The nursing home was very good. The care that they gave to him was very extensive. The facility is very clean. I cannot say anything but good things about his nursing home.
