I would highly recommend this facility!! I called and got my aunt right in. She was treated with such great care and kindness. She felt as if the nurses were her own family caring for her. She also enjoyed every meal and her room was very clean. Although the rooms could be updated a little, her care was so great that she didn’t even care about that.

I would highly recommend this facility!! I called and got my aunt right in. She was treated with such great care and kindness. She felt as if the nurses were her own family caring for her. The rooms are in dire need of refurbishing. The furniture is old & should be replaced. However, the hallways and gathering rooms are bright & cheerful. All areas are clean and comfortable. The staff is exceptional. They are friendly, kind and very patient. They work hard to keep their patients clean & healthy. They offer assistance where & when needed & communicate with the family with a positive attitude. They offer encouragement and support, which motivates the paticient to participate in activities that are geared to their needs and abilities. Years ago a social worker said, don't judge a rehab facility by the appearance, but by the services rendered by the staff. We chose this facility for my mom & I am confident that it was the right choice.

Run don't walk is very well said. This place is abysmal from the food to the care. I don't care that you are short staffed which I heard numerous times. Please do not take in any new patients as I heard you were short staffed within the first hour of arrival. The staff never returns calls cannot be be bothered with the patient or the family. The rooms and facility are dated and dirty. Not 1 meal in 2 weeks was what was ordered. Showering a 91 year old at 10:30 at night? Seriously? I was told by the facility and Doctor that CAreOne was the place for working the patient hard. What a joke. Two weeks and still waiting for 2 returned calls. The main office called me regarding my review and basically told me I was wrong on everything and did not listen to anything I said. Why bother calling if you do not care and will not listen to the customer. The state needs to be involved NOW. Do not go here under any circumstances as every patient complains about the care I spoke with. The staff does not follow the families request for anything.

the rooms where dirty, stains of who knows what on the curtain. ceiling leaking in several areas when it rained(mold) my loved one left worse than when she went in. she had a sever diaper rash from being left in urine and dirty diaper. She was not dressed and was left in bed all day. we did her care because there not aid to be found. he food tray was left on a table out her reach. We got her out ASAP. She would have gotten really sick there.

Run don’t walk . This place should be shut down by the state. Had my mom there for subacute rehab. She has dementia & 3 strokes. She was left in wheelchair for hours. They do not care about your lab work ups. I had them call the ambulance after I noticed her labs were off. She spent 14 days in hospital dehydrated- low sodium - very low glucose level.

Go anywhere but here. The place is dirty, bugs on floors, food is deplorable and staff do not care one iota.

The service at this facility leaves a lot to be desired. The staff does not attend to the needs of the patient in a timely manner. For the first twenty-four hours of my wife's stay, we found that the staff is not as attentive to the patients' needs as they ought to be. One of the problems is that the facility is understaffed and this may be the reason why the service is not efficient.

My aunt was in CareOne at Wall for over a month. First she was in rehab then her condition worsened to the point that she was on hospice care. I don’t even know where to start to describe the amazing care she got there. We looked at a few facilities and decided to keep her in Wall because of all of the wonderful nurses, aides, rehab professionals, speech pathologist, social workers, etc.. in Wall. Their compassion and expertise are exceptional. The facility is bright and clean and everyone always has a smile on their face with a positive attitude. They also have excellent Covid protocols in place. Our family has a great deal of experience with long term care facilities and this facility is one of the best we have seen. Shout outs go to CJ, Angie, Anne, Beth, Tiara, Frank, Amanda, Ashley, Billy, Carmen, Paul, Serge, Lov, and anyone else I may have inadvertently missed! The facility is so well staffed it’s hard to keep track of everyone! Thank you’s go to everyone for keeping my aunt comfortable, clean and pain free; and helping us go through such a difficult time. CareOne at Wall is an exceptional facility and I highly recommend it.

This facility has to be one of the most awful places I have ever been too. I have told nurse Asia that my mother was left in soiled clothing all day and she did nothing about it. My mother developed a decubiti as a result. I placed a mark on her sheet to see how often they were changing her linens. They didn't change her sheets for 2 days. The nurses and aides are useless. I tried to go to the director Juvie, but she was equally as incompetent as her nurses. Do not send your loved one to this facility. Do your research. There are much better places to send the ones you care about.

A lockdown is very difficult for both the patients and their families. It’s times like this that brings out the best and the worst in people. Kim’s understanding and compassion definitely helps ease my mind. She is never to busy to listen and help. I am thankful for her presence at Care One

Care-one Wall review I have nothing but praise for Care-one at Wall. In April 2020, when I was diagnosed with Covid, I really didn’t realize just how sick I actually was. After trying to fight it at home, I checked into Care-one for their care. I thought I’d go in for a few days but I was there for 17 days. The first few days are a memory blur for me, I was just so exhausted and feverish. But for the last 2 weeks I have to say the staff were very caring, professional and cheerful. The rehab girls were fantastic, encouraging me while at the same time being aware of my limits. Nurses came in to check up on me in between the times they needed to come in to give meds, etc. They, along with the doctor assigned to me, tried a couple different antibiotics to help me and finally settled on a mix of two, one being the Hydroxychloroquine type of pill. I really attribute those meds and Care-one for saving my life. I can only imagine what the end result may have been if I had stayed home and tried to deal with the virus myself. So, 5 plus stars from me.

My father left there after 90 days with cognitive deficits, weak and no plan. No matter how many meetings we had, therapy and nursing could not get their acts together. Laundry could not get clothes back to his room despite having his name on them. Therapy equipment was broken and there was no therapy follow through with the nursing staff. When the staff thought I was on hold twice!!!! I heard the nurse/aide yelling at other patients to just be quiet . So disrespectful . Would DEFINITELY NOT recommend!

Not my first stay in a rehab facility,but certainly the best. Physical Therapy staff were caring,attentive,and got me on my feet.OT and PT really knew what they were doing. Nurses and staff were great.

I visited the independent living level of CareOne at Wall, and I myself would want to join them. I really was very impressed. It was clean, it smells clean, the people I saw seemed happy and the rooms were very nice. The gentleman who took me on the tour was very informative and helpful with answering a lot of my questions.

Horrible experience for my family. We since moved my father and he's doing better in 3 days than he did in 23 days in care one. He lost 20 or more pounds during his stay. The communication between them and us was slim to none. My father was ignored most of the time. One night he wandered outside in the pouring rain and the doors locked behind him. No one from the facility notified us or provided an incident report. So many more issues that I could be here all day writing!! The OT and PT staff Nichole and Anthony were great and so was nurse Sue and Aid Kevin. This facility basically threw us to the curb. Told us to find a new facility because we were Medicaid pending. Within 2 days I found a new facility and received a call from the nursing director Louise saying now we could stay. I declined.. she also apologized for all the poor communication and everything that had gone wrong . A little too late for apologies!! Try told us my father was not salvagable and needed to go into palliative care. Hes now walking on his own in the new facility and gaining weight back.. any longer at care one and I would be planning a funeral right now not writing this review. Do not send your elderly family member here you will be sorry!!

My mother stayed at Care-One for one month under hospice care. The staff was so inattentive that I had to hire additional care givers to come in to take care of her because I could not rely on the staff at Care-One. Often times I would walk in to find the phone out of reach and the call button on the floor. The staff rarely, if ever, tried to engage my mother in conversation or even refer to her by name. The water my mother was given was from the tap. Her dietary requests were rarely met and even when they were the food was inedible. While all of this is horrible, it pales in comparison to the lack of air conditioning on the second floor. In the summer months the air conditioning is non existent. Three days before my mother passed away the temperature in her room was 85 degrees. It was inhuman to treat someone that way. While I was a strong advocate for my mother, many at the facility have no one to vocalize their issues and they are left to scream for help in the halls. In most cases, their screams are ignored. I'm surprised Care-One has not been shut down by the Board of Health or sued by one of their patients. Care-One made what was the most difficult time in my life and my mother's life even worse. If I felt my mother could have been successfully moved, I would have moved her to another facility within the first week. The management should hang their heads in shame treating the elderly in such inhuman conditions.

I have been associated with this facility for about 6 years now. My mom startred rehab here after having surgery. She now lives there in the nursing home section as opposed to the rehab unit. I have been happy with the care since the first time . They have a low turn over of employess which says alot about a facility of this nature. That speaks highly of management being in management myself. My mom of course has her struggles getting use to being there now full time but that is to be expected. I visit nights because of my work hours and they have always been open to that which is appreciated. I would refer anyone to this facility if they were in need of advice as my self being my moms caregiver for years and always was looking for resources and advice myself.

This facility is only for those that are ambulatory. If you need a high level of care stay away. My husband could not feed himself and was assigned a feeder. In one month he lost 30 lbs, was dehydrated and needed to be hospitalized to correct what was just a matter of being fed.
