I am/was a resident of this facility

The rooms are dirty. The equipment is very up to date for 1950. The beds are very uncomfortable. When I arrived I needed to use the bathroom. First the light bulb had blown out. Second. Someone had used the toilet and it had not been flushed. The nurses are terribly under staffed. I was there for 5 days. I was suppose to receive physical therapy twice a day. I received it 2 times total. The food is edible. That’s the best I can say about it. I needed an air mattress on the bed as the workings of the mechanical bed were pressing on my surgery. They found one. It was an old air mattress with a motor to blow it up and the motor had to run constantly. Loud enough that I couldn’t sleep at night. Please stay far far away from this place.

I am a friend or relative of a current/past resident

my father lived in this facility for a few months. on multiple occasions he was hospitalized due to infections, lack of assistance in part of those who work there, etc. they not once called my mother or i to tell us he was hospitalized. he had blood transfusions without us being told, he was sent to hospitals without his cell phone and without any of his clothing without us even being told. many times he hit his button calling a nurse because he had to go to the bathroom, but unfortunately the “care takers” do not show up for minutes after the button is hit so he had to do it himself. 2 times that he had to help himself, he unfortunately fell and hurt himself. one of these times he ripped his drainage tube out of his recently amputated leg and was driven to the hospital for surgery without our awareness. just a few days ago he had a stroke and was rushed to the hospital due to too much potassium and lost his memory. he cannot speak or move and does not remember his family or friends. he cannot even eat by himself. we were not called not even once to learn that he had a stroke. we found out days after, from a family friend who unfortunately went to visit him and learned he was hospitalized. his brain is bleeding. the people at this facility don’t care. the food is terrible. residents are mistreated and not cared for properly. my father is unable to do anything, his wellness has declined. not to mention that the rooms are incredibly dirty and frankly unlivable. do not send your loved ones here if you truly care about them.

this facility needs to be on the radar of the state, nursing review board for compentecy, under staffed, dirty, md refusal for services, medication mismanagement, 45 min on average for staff response. I wouldn't send my dog here!

My mother was at Kindred/Greenbriar for about 2 months and in that time she got progressively worse. There wasn't enough nursing staff and what staff was there, about half didn't care about the patients and a handful actively talked about "problem patients" in the hearing of other patients. One wing was closed because of the lack of nursing staff. Her therapy sessions would be cut short; she'd go for a 30 minute session and only get 15 minutes or go for an hour session and only get 40 minutes. The food was horrible; the chicken and fish was often served dry, potatoes would be raw, pork would be over cooked, or the food would be served cold. She would often be left without a call button; once she was on the commode for 40 min without a call button. It would take the nursing staff 20+ minutes to answer her call light when she did have her button. And she was sent home with a prescription (antibiotic, liquid form) that couldn't be immediately filled; the earliest any local pharmacy could do was 2 days later. Do not go to Greenbriar, they are not the same facility they were 5 years ago.

The administration part of Greenbriar is awful they never returned your phone calls Pay It Forward gets misplaced or lost they're always too busy we'll get back to you and never do the whole place needs revamp from the top level down they're out to make money not patients needs worst place I've ever been at

Nurse to patient ratio is low to the point it's not safe. Kindred doesn't pay their staff enough thats why they can't keep people. The nurses here are very nice the Licensed Nurses Assistants are some of the nicest. In fact they are the one's you see the most of. My room was dusty I've watched housekeeping while they clean they just clean around the sink, sweep the floor and sometimes mop and thats it. The food at Greenbriar is the worst, the vegetables are boiled to the point that all the nutrients have been boiled away. Had a Chicken patty the other night, tasted like it was a frozen Supermarket type, nothing appears to be made in house, dinner is served at 4:30 in the afternoon so your hungry again at 9:00 PM. No snacks are offered durning the course of the day so if you're a diabetic you blood sugar is all over the map. Staff needs more training, I had a picc line put in so they can administer I.V's and draw blood more easily This morning a nurse came in to take blood I asked her why she wasn't using the picc line she said, "No one here is certified to due that.". KINDRED WAKE UP. a patient expects their nurses to be trained and trained well. Overall , Greenbrier is not the place to go, but it's not the staffs fault it's the managements fault.

I have a family member who was at Greenbrier Kindred in Nashua, NH. The first night he arrived we were there, His room was dirty and still is, dust and cob webs all over, and the room smelled. House keeping had come in the day after and all they did was wipe down the sink and bathroom and took out the trash. I THOUGHT A PLACE LIKE THIS IS SUPPOSE TO BE SANITARY????????? IT'S NOT BY NO MEANS!!!! He's had wait for more than 45 minutes to an hours for a nurse to come to his room, when his rings the bell. The windows were not closed properly and locked. Any resident there could and if they did could get out the windows. The food the served is awful. One evening we were leaving there was a gentleman in the lobby unattended standing without his walker and when I walked past him he yelled at me thinking I was a nurse and when the nurse approached him, she said his name and I thought she was rude about it. He tossed his walker away. She whisked him away.

The care at this facility has gone from bad to worse. I am in the healthcare profession and I know exactly what I am am talking about. Due to the continued, and significant decline in care and staffing patterns, I feel it necessary to look elsewhere for placement of my parent. Call lights and safety alarms mean nothing. And even when they are heard the aids do not respond, and at times, are UNABLE to respond because they are the only aid working on a unit and are busy trying to care for another resident. Generally speaking, the nurses and/or other staff do not help answer call lights or alarms...even to assure patient safety. I spend many nights concerned about my parent and praying to God they make it through another day safely. Talking to the Administrator and/or DON is a waste of time. They are fully aware of the situation and would rather have unsafe staffing ratios than bring in agency staff to fulfill their staffing needs. The aids and nurses are leaving left and right and they can not retain the staff they do manage to hire. The aids they are able to get are all too often, of poor quality, uncaring, insensitive and/or inexperienced. There are a few nurse and aids I trust and I feel do their best to provide quality care, but they are frustrated, overwhelmed and are too few in numbers. I fear it won't be long before they quit as well. It's a very sad state of affairs at this facility.

I won't speak to the entire staff because some nurses, aides, and especially the activities workers were truly fantastic but we had very unpleasant experience with Greenbriar. A relative of mine was there for a while to rehab but my family found much of the nursing staff to be inexperienced with the needs of elderly patients and at times completely inattentive. If you are calling for assistance to use the restroom 45 minutes is not an acceptable time to wait. I witnessed several hygenic issues with regard to the way medicine was administered and the living space was cleaned. It reached the point where we were uncomfortable leaving my relative without family there for any amount of time. We understand the nursing profession is difficult and stressful at times but the management needs to find a way to have their employees better equipped to handle the needs of their patients.

Don't send your loved ones here. The Nursing to patient ratios are so low its dangerous. Nurses are leaving left and right and administration does not care about it in the least. Greenbrier was once an excellent facility however the newest directors are taking this place to a whole new low. The facility looks pretty but that's about it. Go somewhere else!

I looked at Greenbriar (Kindred Transitional Care and Rehabilitation), and everything looked good, but it was not going to accommodate the things that we needed. However, they had a rehab for the people who were off from surgery as it is a nursing home. They answered all my questions.

I visited this facility

Kindred Greenbriar was nice. It was well staffed. The first place I looked at I liked a little better, but this was a very cool second. They had it down to a very fine system. They were very efficient, yet you felt very comfortable when you walked in. The customer service was awesome. It was very well maintained. It was immaculate.

This place is a ripoff. They billed medicare to the max,and after they lost my moms dentures refused to pay for them,refused to pay for one half the fee for new ones.

I am/was a resident of this facility

I recently had occasion to spend 4 wks at Kindred Transitional Care & Rehab -Greenbriar (KTCR-Grn). When I arrived, I had a violent case of intestinal issues and a very infected Venus Statis Ulcer on my right calf which I'd let go for over a year. The very qualified, experienced & kind doctors, nurses & aides on all 3 shifts provided me with excellent care. In fact, the wound care I received for the ulcer was so thorough, the plastic surgeon felt no skin graft would be necessary. Also, I had tied myself to an electric motorized power chair for over 2 yrs and could barely stand up on my own. Today, I can stand to do my dishes, vacuum and cook with only the use of manual walker for some support. As I continue on my road to independence from the "chair", I owe so much to the physical & occupational therapists at KTCRGrn who worked so dedicatedly with and for me. And during what could seem like hours of empty time in my room, the Activities ladies would show up with their smiles and invitations to join a group for Bingo or a musical performance in the Main Lobby. They would pick me up in my room for the activity and deliver me back, too. I never felt alone. I KNEW always that help of any kind at any time of the day or night was a close as my "call button". I don't want to omit mentioning the laundry staff who brought up my clean clothes with a cheerful greeting. If my parents were still alive, I wouldn't hesitate to place them in the care of these exceptional professionals at KTC&R-Grn.

My husband's got dementia and is staying at the Kindred Transitional Care and Rehabilitation because it is the closest to where we live. The food is very good. They have entertainment coming in throughout the week, movies, Bingo and Blitz. The staff is very nice.

The nursing staff in this facility is second to none. They take very good care of the patients. Checking a patient out for an afternoon, lunch or a day is very simple provided you provide the staff with the necessary information such as destination, time of return, etc. The nursing staff is willing to answer any question that is asked by visitors (while protecting the patients' privacy) and treat each patient with utmost respect.From the patient point of view, we have been told several times that he is extremely well cared for. The staff members keep the patients active and involve them in games. This is above and beyond what many facilities that offer care for patients with dementia offer.Overall, this facility offers plenty of activities for patients, liberal visitation policies and they take excellent care of patients. Having been there to visit both in the rehabilitation area and the long term area, I rate their service as extraordinary.

The facility is home based. Very clean and comfortable atmosphere. The staff is one of the best. They are right there lending a helping hand if they are needed.The facility is in a very good location and is near by. We do not have to travel far for the facility. The staff and very friendly the minute you walk through the door there is always someone there to great you. They come in and check to make sure the patient is comfortable.
