this place is a complete joke. Basically if you love your family member and want even just a little bit of dignity to remain in their last years of life you won't send them here. Let's start with the administration. They do not have the residents best interest at heart. Whatsoever. They are quick to fire CNAs who take care of the residents everyday but can't even keep a bare minimum of supplies available for them to do their job. Laundry is not being sent out nor washed in bleach so your grandma is washing her face with the same washcloth I wiped another residents private area with. Nurses are CONSTANTLY over medicating and making CRITICAL medication errors to the point where residents are being sent to the hospital on drug over doses. Not to mention they don't care to act quickly and effectively in serious medical situations landing residents in the ICU. It's called "Nebraska Kill" for a reason. Don't get me wrong there are nurses who do their job great it's just very few. Nurses who are making critical errors are not being handled in the right manner resulting in them still being able to work the floor AND med carts passing meds to your loved one. Residents are subjected to looking at a graveyard for breakfast lunch dinner as their everyday view. It's just a pitiful place and don't send your family members here !!!!

I had to be rushed to the ER with a life threatening blood clot that I had to plead with the medical staff at this facility to pay attention to, I was rushed into surgery for a filter and they explained before the procedure it may already be too late to save me. I told everyone at Nebraska Skilled, all the nursing staff (you are never allowed to see their doctor expect on admission), the social worker, admin staff I was in incredible pain and needed help. Finally on a Saturday there was a different nurse and she actually got me transferred to the hospital. I have since learned that people have actually called 911 from their rooms. I have known two other people who also went to Nebraska Skilled to rehab after the hospital - they are dead - they died there. I was lucky to make it out alive.

I would give this place no stars in possible. My mother was very ill when she went there and almost died as a result of their care. They never took her vitals and we had to rush her to the ER because of very low oxygen. She would have died had we not forced them to take her vitals. Do not allow your family to go here.

Everything is so-so. My life is okay though. The food seems to be overcooked or undercooked. I've been living here in Nebraska Skilled Nursing & Rehab for seven years. The rehab and the exercises help a lot -- it's the best thing that we have. The evening staff is not the best. From 2:00 to 6:00 is the problem time, but morning and overnight are good. They have art, reading, various types of projects like sewing, you can get out and walk to exercise, and play bingo, which I really enjoy. Sometimes I have trouble and sometimes I don't -- it depends on the individuals that I work with.

My dad was at Nebraska Skilled Nursing and Rehabilitation Center. The staff was friendly when I visited. The rooms were pretty good. There was a bed, a couple of chairs, and a table. There was a little couch, a TV, and a dormitory size refrigerator in the room. The security and safety seemed to be good. They had cameras at the entrances. I would probably refer this facility to my friends.

Don't go there. I was there for three days after an operation for rehab. They put me on a bike for 5 minutes and that was it. I pleaded to have them escort me to walk everyday which was my doctor's requirement, but only once did that work. Being very tall they repeatedly made fun of my size and once an attendant knocked on the door and, while giggling, asked me to stand up so she could show me off to her friend whom she brought along. Going down the hall I was hooted and shrieked at. Understand I know about professional courtesy when it comes to the medical profession. I've had 6 operations and have been in numerous medical facilities; this was humiliating and a waste of Medicare's money. They demanded I pay $500 up front and now they won't return my $400 balance after 4 months. I'm actively reporting them to the authorities including the State Attorney General, BBB and Nebraska DHH. I was told they once lost their state license.

This place is awful. No one will take responsibility for anything. My patient was ignored and the staff would only say that she never asked for anything. Communication is non-existent. This place collects medicare for warehousing patients. Social workers will not tell you if a place is bad only if there is space available. Shame on social workers!

TERRIBLE PLACE! I wouldn't leave my dog there! Dear friend had open bed sores. I found her on several occasions lying in her own feces and wet. The call button was broken, and the heat would not shut off. It was stifling. I found my friend naked on her bed while they went to find some other bed clothes for her. She had cancer and couldn't talk or move much. I was appalled at the lack of care, despite reporting it to them. How they are allowed to stay open is a disgrace!

The staff were incredibly kind, and worked very hard. Unfortunately, they seemed to be incredibly understaffed, and that meant waiting longer than was healthy. My relative was dealing with incontinence issues and difficulty walking, and it was very hard on him when he had to wait a long time to get to the bathroom. The rehab was great. They did a great job of getting my family member stronger over a fairly short amount of time. Communication was our biggest complaint about this facility. Most of the time it didn't seem that the staff understood how to communicate with us-the family members. Once they even tried to take my family member for a doctor's appointment that they had told us nothing about. This was incredibly frustrating, because it turned out that it was a completely unnecessary appointment with a doctor that had never seen my family member. This was very frustrating. If they would work on their communication and get a few more employees, this place would be amazing.

Absolutely horrible place to send a loved one! They refuse to get necessary medical equipment and neglect patients! The only helpful people there are the therapists! They have not followed doctors orders there and my Dad now has sores from them letting him sit in body secretions and it looks like the skin is being eaten away. They refuse to get him up each day (he is paralyzed on one side) and have refused to get him a wheelchair to have him sit in. Also make excuses for this and the nursing director has flat out lied to the insurance company about all of this several times! There is no reason they should have a license to care for people! They see a paycheck in each bed, not a person!
