Describe the overall population of assisted living residents at Jack Daws Place - CLOSED . What is their average age? What are their favorite things to do?

Accepted Answer

Since opening in December of 2019 there have been individuals who have enjoyed being served by JBs staff that have been in their 70's, 80's and 90's. Almost forgot our 105 saint! Due to Hippa Laws specific gender and exact ages cannot be revealed. We have served both men and women.

Things that folks like to do is enjoy home cooking catered to the likes of our residents, enjoy the activity of the day (provided by our activities director), enjoy watching the wildlife outside, enjoy going outside (weather permitting), enjoy weekly chapel, enjoy morning devotions, enjoy reading books and the newspaper, enjoy visiting with the nice staff. Enjoy playing cards, watching TV and movies as well.

What programs or activities does Jack Daws Place - CLOSED  offer to enrich residents' well-being?

Accepted Answer

First, Jack Daws Place has an activities director that plans a fun game or activity each day. Activities include: Bingo (of course), painting/coloring, word searches, exercise time, fun active games, board games, and many more. Second, because Jack Daws is licensed for only 6 individuals we know what each resident is capable of doing, ie, someone with memory challenges might not be able to play a simple game of cards but can participate in lots of active games (balloon volleyball for example). We cater to each individual because we know and love each individual.

What makes the chef or dining program at Jack Daws Place - CLOSED  exceptional? What kind of menu can assisted living residents expect?

Accepted Answer

In many other places of choice, the chef is cooking for 50+ residents. At Jack Daws Place our licensed food director is cooking for 6. Straight away a new resident (and family) is asked what the individual likes to eat and what they DO NOT like to eat. We center our menu around what our residents enjoy eating, and if there ever was something an individual did not find worthy, sandwiches, soups and salads are always available. Meals are the #1 thing that folks complain about and it is rare that we have complaints.

What safety features or security measures does Jack Daws Place - CLOSED  offer to assisted living residents?

Accepted Answer

The house is in a residential area and all the doors have locks on them. The front door turn lock was replaced with a remove the key deadbolt, and the door to the mud room (that exits to the garage and is our med room) has a combination pad that only staff have the code to open the door. There are also security cameras in the common spaces. If an individual is a fall risk there are a number of measures we can take: 1) each resident has a lanyard with a push button to alert staff that something is needed, 2) bed alarms, and 3) video monitors can be placed in rooms (with permission from family).

What safety features or security measures does Jack Daws Place - CLOSED  provide to protect residents who wander?

Accepted Answer

Our ratios are the number one thing that helps with wandering. Our staff is on hand and has the residents in their site. But let's say that one of the residents wants to go to their private bedroom to rest after lunch. If that person is a wanderer or a fall risk the above mentioned prevention tactics would be implemented: ie. bed alarms, room monitors, and the common video camera in the hallway.

What programs or activities does Jack Daws Place - CLOSED  offer to keep residents engaged? How does the staff engage residents' long-term memories?

Accepted Answer

We have found that family picture albums are valuable in helping folks connect to their long term memories. We have also found that music from the era of the individuals past is also helpful. We at Jack Daws get to the know person as he/she is currently but also the person they were in the not so long ago past to love and support them. Our staff is excellent at re-engaging individual when sun downing occurs or other challenging times happen. We have been very fortunate to have staff stay with us for long periods of time which also helps folks with memory challenges because even thought they might not recall the staff persons name they know their love toward them!

Who assesses residents' health and cognitive functioning? How often is that assessment repeated?

Accepted Answer

Each shift our care givers chart the residents day. What happened in the bathroom; how did they eat?; what did they eat?; how much did they eat?; did they take their medicine?; and what was their mood?. Each morning our Director of Nursing reads the notes and follows up as necessary with staff and/or doctors. These assessment are done daily and repeated daily. Jack Daws Place has RN coverage 24/7.

What ongoing training does the staff receive in dementia care?

Accepted Answer

When staff join our team, they receive 20 hours in dementia care. Additionally, each year there is a refresher module that is assigned along with monthly staff meetings where our Director of Nursing teaches dementia care and several other topics.

Describe the neighborhood around Jack Daws Place - CLOSED . Is it located in a residential neighborhood, a business district, a rural setting, or other type of locale?

Accepted Answer

Jack Daws Place is located in a lovely suburban neighborhood of Lakeville. The neighbors are very supportive of having an Assisted Living home next to them and have even vowed to pray for us!

Accepted Answer

Sorry to say we do not allow pets.

What ongoing senior care training does the staff receive?

Accepted Answer

Each year our care giving staff received well over 25 hours of training. Each month we have a Staff meeting where there is an hour of training and each month 1 to 2 hours of training is assigned online through a company called Educare.

How does Jack Daws Place - CLOSED  keep residents' families informed about their loved ones' health?

Accepted Answer

Each month there is a family newsletter with helpful articles, monthly menus, monthly activities and updates on staff. There is also a monthly "open" meeting that our Assisted Living Director hosts where people can attend in person or via telephone to ask questions and express concerns. Further, 4 times per year there is a family care conference with our Director of Nursing and family members. Phone calls are always welcome to the owner, the Assisted Living Director and the Director of Nursing.

Is Jack Daws Place - CLOSED  affiliated with a hospital if more care is needed?

Accepted Answer

We allow every hospital system to come and see residents at Jack Daws. We do have a Doctor who comes directly to our home and will do blood draws, X-rays, and monthly visits. She is an internal medicine physician named: Silva Hugec, MD and her company is called: Home Doctor for you.

What places are near Jack Daws Place - CLOSED  or within walking distance for assisted living residents to enjoy?

Accepted Answer

Lakeville, Mn is a growing suburb of Mpls/St. Paul. There are dozens of shops and restaurants that family can take their loved ones out for a fun outing. The house is located on a quiet street so if the resident is able to go for a walk, it is very safe to do so. Within a quick 2 or 3 minute drive family can enjoy several parks as well.

What transportation services does Jack Daws Place - CLOSED  offer to assisted living residents?

Accepted Answer

Our Licensed Assisted Living Director can arrange transportation to doctors appointments as needed. In an emergency situation, the Lakeville Fire Department is 1.5 miles away from the house. Family is welcome to transport their loved ones as well.

Ratios are perhaps the most important information one can find out for any place you place trust in to care for your family member! The lower the number the better care the resident has. At Jack Daws Place our day time ratio is 6 residents to 2 care givers! That is a 3 to 1 ratio! Simply put, you will not find a better ratio for Assisted Living than that!

At night there is one awake staff member and one staff member who is allowed to sleep but is awakened twice to help reposition two person lift folks.

Accepted Answer

Because we know our residents so we know if they have behaviors at mealtime. If they do, with the team of care givers, nurses, and doctors a plan is formed and then implemented. This plan will be different for each resident. For some, as they move down the road of memory care, they require a staff member feeds them. For others, they may need their meal cut up in small pieces. For others we may need a staff member to sit next to them. Whatever is needed, that is what is provided.
