I am a friend or relative of a current/past resident

My sister is currently residing here. Some staff have been very disrespectful stating "I'm done with you" because my sister is unable to walk or stand without assistance of two people and they have one woman assigned for mobility assistance. She does not like the food..always cold. She has lost 40 pounds in 2.5 months. I wish I could fine alternate arrangements. At $280 per day, semi-private, not a great value. They are billing medical services "out of network" This is a complete disaster. Administrative staff doesn't/never return phone calls.

My stay at Applewood has been unexpectedly well. The food is delicious and very satisfying, the staff is delightful and very caring, and the nurses are very knowledgeable. I am originally from Flint MI and relocated to Trenton and have made my first friends here at Applewood.

I am/was a resident of this facility

I'm in Symphony Applewood for rehabilitation. I have applied for Medicaid, and I'm working toward becoming a resident here. I like best the therapy. The vast majority of the staff are very nice and they take care of me but there's an occasional person who doesn't seem to care, but that's not as often. The food's not bad. It's better than other places I've been. The cleanliness is about average. Generally, it's OK. They have a dining area. However, I'm bedbound, so they feed me in my room. I know they have certain programs here, too. Occasionally, they'll have an ice cream night or a social night where they'll have cocktails or beer available if you want it. Sometimes, they have people come in and speak or do music and they might bring donuts or something like that.

A friend of ours was in the nursing home at Symphony Applewood. It was total chaos. That would be the best way to describe that place. The staff was mediocre. Their food wasn't particularly appetizing. They gave a lot of hot dogs and grilled cheese and stuff like that which I don't think is very healthy for people. It was not a nice place, but that's where they had to go because that's what their insurance company approved. I never saw any activities going on at all. They had like a dining room there, but most of them seemed to be hanging in the hallways. It was not very nice. The building seemed to be clean, neat, and not falling apart. It was well constructed.

If you are looking for a facility with rehabilitation, you definitely might want to keep looking. We have been to Applewood previously and it was excellent and the PT really worked their patients, and cared about their goals they wanted to achieve so they could get home. Since the new owners have taken over it is a disaster, I honestly can't believe that they are charging my family members insurance for making her do bed exercises, when she should be in a wheelchair at a exercise machine and trying to stand up with her walker. They are very short staffed and she has sat for hours in her dirty brief on multiple days. The food there USED to be excellent when it was APPLEWOOD, they do have an ala cart menu you can order off {very limited} if you don't like the slop. The bathrooms are very dirty, and I never saw anyone mop the floors in the rooms. This is all very disappointing because before Symphony took over this place was 5 star. The good thing is that what staff they had was pretty darn good for the most part.

The staff very good and treated my husband with respect. The physical therapist was excellent and improved my husbands strength in the short time he was there. The staff was always polite to me on the phone and even though I couldn't visit I was comfortable knowing my husband was being taken care of. The applenotes is the FaceTime program and they always had a time available when I wanted to see my husband.

My mother is in Applewood Nursing Center. Right now, everybody seems to be very helpful. You can call and talk to the nurse at any time about your patient, I've even had the nurse Facetime me, so I could see my mom. The outside is very well-kept, and well taken care of. Even people at the hospital and the social worker said "that's a good place." They have a lot of activities for them and dances.

This facility has excellent caregivers. Both CNA‘s and nurses are kind and work very hard. A few of the staff members have been rude on occasion but you will find that behavior anywhere. My problem with the facility is that they are always understaffed and patients must wait way too long to get help after the call light has been pressed. It is not the fault of a nurse or an aid when they are already busy helping so many others. I am not sure what the nurse/aid to patient ratio is in there but I can tell you it can’t be legal what they have on afternoons and midnights. Since my grandma has been there she rarely gets out of bed and when she does she is put in her chair or a wheelchair. There have been many times where she has sat in a wheelchair for 6 to 8 hours at a time. Since she has been in the facility she rarely gets any exercise and therefore most of her muscle mass has atrophied. Many times she is waiting to go to the bathroom so she tries to get up on her own and my fear is that she will fall. If I am on the phone with her I have used my other phone to call the nurses station and tell them that she is trying to get up and somebody needs to get down there as soon as possible to prevent a fall. Several times this has happened and she has still had to wait 10 to 15 minutes before anybody could go down there knowing that there’s a possibility of her falling and injuring herself. Again, these types of examples would not happen if more staff was hired. I can only commend the staff that have stayed around during the COVID-19 pandemic. They are true heroes. They can not be blamed for continuous short staffing that leads to unsafe practices.

Applewood Nursing Center is more of a short term/long term nursing facility. My mother-in-law is a resident there. It's truly for residents with more limitations. Their medical team changes over quite a bit. I would rate their cleanliness a three. There's a lot of wheelchair limitations, and mental limitations. The physical therapy program is great, and I would rate it a five. It has a nursing home feel which is not as upbeat as assisted living.

I visited this facility

Looked over the case and said we were accepted to attend. As the transfer was being processed, they stated that they changed their mind and were declining admittance. No explanation would be given. Don B., assistant manager, was the one who told us we were accepted and the one who decided to deny us. We asked to speak to him but he refused to speak with us at all

My mother passed away while she was a resident there for just a few short months. This place had me in tears in the care they were providing for my mother. I came in later than usual one evening only to find they forgot to feed her! When I complained to management they made it seem like it was of importance yet nothing really changed. I don't understand if they hire ignorant people with no experience with the elderly and dying or they are extremely short staffed; we waited anywhere from 20 minutes to an hour to have her call light answered at all times. Didn't feel safe leaving my mother here, disappointed she spent her last few months here.

They have very friendly staff, Plenty of staff to watch over the residents, good security, plenty of activities and appealing food. The only complaint for me is the rooms are very small and you have a roommate so there's very little privacy.
