My husband moved into the Autumn Woods Health Care Facility. He's happy there. They have plenty of staff. They're very friendly. The facility is clean. The food is much better than what he was getting at the other facilities, so he's fine with the food. They have a hair salon, physical therapy, a little aviary, and a game area where they have a game table. They have games, like puzzles, and things for them to do. They have activities all the time. They usually have a dog come in for visits once a month or so. They have church services. Everything is there. The staff members are always there. They're always responsive and helpful. They don't make you wait a long time. Medicaid and Social Security pay for it.

I just got out of Autumn Woods Health Care Facility. It was good. I liked the activities they did. I had therapy in the morning and activities in the afternoon. It was alright. I had a single room, and I had a toilet and sink. The food was alright. We had a place outside where we met other patients just to talk or smoke cigarettes, and it was alright.

My dad is at Autumn Woods Health Care Facility. The place is not good. You have to tell them to come because it always smells like pee and poo when you go in there. It's not clean and not safe. The staff members are OK. There's one nurse who was like she doesn't want to be bothered. She's not a good nurse. My dad doesn't like the food. We always have to take something for him to eat. We tasted the food, and it was not good. They don't do too many activities with them. My dad has a roommate. The facility should be more caring and more active with the patients. They should be cleaner with their environment.

My father-in-law is currently staying in Autumn Woods Heath Care Facility. I don't have any complaints or problems for now. It is clean, but I think they can do better. There is a smell -- all nursing home smells sometimes -- but I don't know. They're really nice and friendly. Every time we go there, someone stops by and asks if we need anything. I don't know if they're going to start doing some exercises for him, because he told me that he's weak in his legs and lower body. I never had a meal or saw him eating, but he didn't complain. He was happy with his meals. He's ninety-seven years old; I don't know if this is going to help, but I wish there were someone who could sit with him and feed him when it's time. I know some people don't need the service, but he needs it because his hands are really weak. This is not a complaint; it's just something to think about. There are a lot of covered places; they call them gazebos or something, and they are really nice; I like it. The landscape is nice. The facility is clean and nice, but I think they still can do better.

This place is THE worst facility that you could ever stick a loved one into...If you want to get back at them for something they did, stick them in here! My father is a vet and was placed in here covered under his VA insurance for 2 weeks for rehabilitation. He had been hospitalized and in intensive care a few weeks prior with 2 strands of pneumonia and sepsis blood infection.. While in here the first week contracted an intestinal infection and severe dehydration... From the moment you walk in, the smell of urine permeates your nostrils...The rooms arent thoroughly cleaned, nurses/aides come literally 2 hours after call button is pushed, they only have water and the ice is kept in a rubbermaid cooler in the hallway for all nurses, aides, guests and patients themselves to get on their own, and in a 2 week period my father was bathed only 3xs even tho he was having bowel issues because of his illness...Staff was lazy and rude and never around and the Aide Supervisor named Jerry was downright mentally abusive. In 2 weeks time, not once did I see anyone out of their room and the food was the wrong color and/or the patients were feed dr sandwiches for dinner. I would only send your worst enemy here!

This place is horrible my sister was there they did not even give her a sponge bath for 2 weeks she was suppose to go to physical therapy they never took her she was so severly dehydrated and there was no iv the rooms are dirty and so are the bathrooms do not put your loved ones in that horrid place I have more to say but this isn t the place

My sister was a resident here at Autumn Woods in Warren Michigan. She recently died in July 2018. Prior to this I had made several complaints. I had an advocate from LARA working with me. They had dropped my sister twice. She was blind with special needs. They use to just let her stay in bed many times with soiled diapers. After the advocate they began to take her out of the room daily. BUT they would leave her alone sitting in the lounge for hours. She would get so cold. Many times no sweater, blanket, or cover, wearing night clothes. It is so sad the way the residents are treated. MY MOTHER WHO IS 101 YEARS OLD SHARED THE SAME ROOM WITH MY SISTER. NOW SHE IS GOING BLIND. THE FAMILY WANTS TO BE NOTIFIED OF HER EYE APPOINTMENTS SO WE CAN TELL THE DOCTOR WHAT’S GOING ON AND FIND OUT HOW THEY CAN HELP HER. NO ONE EVER NOTIFYS US. NOW THEY RECENTLY PUT A WOMAN IN MY MOTHERS ROOM WHO WAS SCREAMING AND YELLING ALL NIGHT. MY MOTHER SAID IT WAS HORRIBLE. THEY DID REMOVE HER. ALL OF MY MOTHERS CLOTHES HAVE BEEN STOLEN FROM HER CLOSET. ITS CRAZY WHAT IS GOING ON. THIS IS NOT THE PLACE TO PUT YOUR DEAR LOVED ONES. PLEASE THINK TWICE. There is really very little communication unless we get an advocate involved. I’m opening another case so I can get help finding out about my mother going blind. She is always saying “It is so dark in here, I pray I am not going blind.”

This place is a living nightmare. It breaks my heart I cant have Mom at home with a private, caring nurse. Todays visited consisted of being ignored by 3 separate nurse aides. One looked directly at me when I asked for a dinner tray and walked away. Mom had no blanket, the walls are nasty!! There is no excuse for these loving elderly people to endure this. Im so glad Mom will be out of here soon. Please before you move your love one in here do more research. They have so many violations through the state for improper care & hazardous conditions. I must report her filthy room daily but end up wiping and sanatizing more than they do. My mom was left without a blanket 3 times. Once I popped into to visit in the evening and my Mom had no pillow, no blanket, a hospital gown on (she had warm pjs hanging in closet. The air was on in her room and she was shaking and trying to use her blue pad that was under her as a cover. It broke my heart.

The food at Autumn Woods is good. The place is clean. There are attendants around in case you need any help. The staff is very caring toward the patients. The rooms are skilled nursing and double occupancy. It is swept down and mopped daily; nothing fancy. It's not the Waldorf. The grounds are well kept and very nice. It's very easy and accessible. They could have more staff on the weekends as sometimes they're a little short-handed.

Autumn Woods Residential Health was kind of chaotic. The people that run it are haphazard. One doesn’t know what the other one is doing, but the nurses are attentive. Mom had a lot of food that was ground up because she had a stroke, but she tried to eat a sloppy joe. I think that’s a bad food for someone who’s had a stroke. It’s hard to manipulate a sloppy Joe and keep clean without it getting all over. The community was clean. They had bingo. They had activities, but if you knew the residents and if they got everybody, it would be another thing.

So far so good. Our friend is adjusting well in the three and a half months of his stay.. The staff is very accommodating when a concern arises.

When it comes to placing a family member here and then having the "family" complain about the facility is a bit irritating.You can not complain about the living space especially when you agree to have them live there AFTER you've done a walk through.You see the entire facility so you know EXACTLY what is in store.My mom is there and we are short on funds.Every time I come in contact with an employee they are always very nice and actually take the time out to ask about things that interest my mom.Yes it is a bit outdated but she gets meals,snacks,baths,TV and a warm place to sleep.

My loved one was placed at Autumn Woods , looks great on the outside, but when it comes to the family members room-it is the worst . The rooms here are dirty, her bed table has not even been wiped down, the furniture in the room is old, the floor did not even look clean, the beds are right on top of each other,the room smells like urine, the walls in her room need some serious painting, they only provide one chair in the room for family members to sit-due to the fact the rooms are very very small-yes there are other areas to take your loved to have a visit with them- which all in all is a lot better then the loved ones room,- they do provide a beauty shop- it looks very nice - But I am not impressed with the staff either- I recommend visiting at all hours of the day and night - not impressed with the staff at all!! At least that have not impressed me yet!! Clean up the rooms, make it a pleasant place for my loved one and others!! I cannot even give this place 1/2 of a STAR-but unfortunately to submit I have to choose 1 star----- I will work very hard to have my loved one placed elsewhere!!!!!

Mom was in this facility for 6 years after release from hospital with complications from severe stroke. I found the staff to be competent, caring and compassionate - BUT to get the best care for your loved one, visit often, but on a bit of an irregular schedule, that way staff gives the best care -they never know when you'll roll through that door! Also try at all times to work "with the system", as part of her caregiving team (you are), rather than against the facility. A lot of this journey came down to changing my attitude, to be more positive, and not so reactive toward the staff if something went wrong - after all, we are all on the same team!
