I am a friend or relative of a current/past resident

Maybe I should give it a few more days, but my mom has only been there 1 day, the facility is nice definitely compared to the last 2 rehab centers my mom stayed at. But having someone respond to the nurses call, is sometimes unacceptable. My mom would have waited over 40 min to have someone help her to the bathroom if I had not arrived. When it did it wasn’t a pleasant situation for me or my mom after having to clean her up myself. When I mentioned it to the nurse, basically I was told they have lots of patients and it’s not like being in the hospital. Well not a good answer. I think they the center needs to hire more staff. Nice location, short staff doesn’t make a pleasant stay.

I visited this facility

I toured Evergreen. I didn't set up with one of the staff and do a tour that way. I went in a different capacity before it so that I could see it firsthand. They can schedule and be prepared for you to come to visit it, and it's not what is gonna go on daily. I wanted to see their day-to-day activities, without being planned, without being rehearsed, and everything else. I wanted to see it firsthand. Overall, it was good. It was clean, it was decent. The time that I went was in the midst of morning activity. You had some smells and so on, but like I said that is to be expected. The staff are not paid enough. They work, and they did quite a bit, especially in the morning to get everybody situated and prepared, fed, dressed, and cleaned. I did not try the food, I was just told how the dining setup worked. The outside is really beautiful. The front is beautiful and I know that some of the residents go out in the front. The entry is nice, but there's not a whole lot of room in the back because that's like a parking area and it's like an athletic club. So, there's not much going on in the back, but the front is beautiful.

So far, our experience with Evergreen Health and Living Center has been pretty good. Overall, I think it's a nice place. It's pretty clean and they seem to take care of my mom pretty well. The rooms are nice, clean, and looks like they're doing some reconstruction on certain areas to make it bigger and make more room. The staff seems pretty good also. Every time I talk to them about an issue, they seem to be on top of it. So, I haven't had any problems with them. They had a good menu, they do art activities, and they have a nice big activity room.

My father was a resident of Evergreen Health and Living Center. It was very nice, the physical therapy is wonderful. I love Evergreen, the only issue that I've ever had with them was that my father did not like the food. My dad was in there multiple times, and the only time he did not like it was during COVID. Maybe it's the lack of visitation, I don't know exactly what happened, but he began to have some dementia. It may have started because he was there so long and had been in the hospital so long. But outside of that, Evergreen's services and rehab are wonderful, I would recommend it to anyone Their physical therapy was great, always an awesome job. My dad and aunt had been there before. It's very clean, and the staff is friendly. Speaking with someone was no problem. Another issue I had, I would take him food, and the maintenance person would throw it out when cleaning out the refrigerator. But outside of that, it was definitely awesome. They had activities, they had bingo, and ice cream socials. They would have singers and musicians come in and play. They made sure their participants wanted to participate. They would come to the room, get them, and take them out to the community area so that they could participate, for those that lived there and those that were just there for rehab. They were very inviting, as far as family members participating. I haven't tried the food myself. It wasn't bad, and it wasn't horrible, he just didn't like it; he likes southern cooking. When I informed the staff that he didn't like the food, the nutritionist did reach out to me; you can have selections, you can kind of pick what you want from their options. So, we did do that, but my dad was just a funny bird. He wanted southern cooking, and a corned beef sandwich. The insurers always covered his bills while he was there. We just had to pay for the telephone or cable, which was a nominal fee; that wasn't anything outrageous, maybe $50 dollars or something like that, so I didn't feel robbed. I have never gotten a bill that was a concern.

With nursing low staffed, you need to learn how to help yourself the best you can. Be assertive and alert and tell them you can assist yourself. Let family or friends know if you need anything. Room is OK, but I do not like the beds and sleeping is not easy for me here. Some of the staff try their best, while others may not understand why you are here. Nurse gives your meds and lets you review them before you take. Food is OK, make sure your tray is for you with the paper slip having your name. Pressing your red button for help may take awhile for them to come to you so try yourself to do as much as you can for yourself. Patient phone is useful as your tool to use for other departments.

I filed a complaint with the state of Michigan against the facility because the midnight shift staff on the weekend has failed on several occasions to change my brief. Plus, speaking with the facility's supervisor & management about this problem has fallen on deaf ears. Being left in soiled unchanged brief for 12+ hours is down right unsanitary & 'nasty' & no picnic. 2 days after the state substantiated my complaint the midnight shift staff on the weekend 'again' failed to change my brief. What is more worse, the staff failed to change my brief 2 nights in a row. I blame the CNA on duty each time the failure to change my brief occurred. But I blame management more because management has failed to do anything about this problem that keeps occurring. My perspective is management keeps allowing the CNAs to get away with doing this so they don't care & condone it. Also, this happening again days after the state investigated my complaint says management doesn't take the state, my complaint, or me serious. In addition, since residing in the facility there's been a failure to provide both of my weekly showers on my shower days. In other words, I get a shower once a week.

I will never recommend Evergreen to anyone. I just picked her up a few hours ago after being there less than 1 week. My entire family are very disappointed with the horrible care this facility provided. My sister and I cried so many tears while she was in their care. The Staff just did not care. Please if you are looking for a facility to really provide care your love one needs and deserves, Evergreen is definitely NOT the place. As I read the reviews tonight, I see the promise of change has still not come into play. I was promised and it never happened. Even down to bringing my sister to the front lobby WITHOUT A MASK!

This place use to be nice. If you want your loved ones to be treated with respect and receive PROPER care don't bring them to Evergreen. My mom was here and it was HORRIBLE. Her surgical site wasn't clean nor checked & she got an infection. Never got a shower. Some days would be in the bed all day. Because of Covid they can get away with the neglect

RUN...don't walk from this facility before placing anyone you love or care about in there! PLEASE check their reviews thoroughly and any violations they may have. We placed our loved one here for two weeks for rehabilitation after he was discharged from the hospital after having a severe accident. There were so many things wrong here I don't know where to begin! The staff attempted to serve our loved one FOOD HE WAS ALLERGIC TO which could have caused dire consequences had we not been there to immediately take it away from him. (IT passed at least three different employees' hands before reaching our loved one so there was absolutely no excuse for the "mishap"!). The allergy was also written (and highlighted in PINK highlighter) on our loved ones meal receipt each time he was served something. It took at least 20-37 minutes for the nurse to arrive after the call button was hit when our loved one needed something. Our loved one fell from his bed on the dirty floor and remained there FOR OVER 20 minutes AFTER the call button for the nurse was pushed because nobody came to his rescue! By accident, an aid was walking by and assisted our loved one as the nurse, again, NEVER CAME, despite the call button being pushed. The cough medicine our loved one was taking (WITH CODEINE) had apparently "RUN OUT" according to the nurse so he could not have the required dose until they "replenished" the medication! ARE YOU KIDDING ME? HOW COULD THIS HAPPEN? The bathroom was disgusting. Our loved one's roommate defecated ALL OVER after he was left sitting on the toilet for OVER 20 minutes with no nurse coming to his aid AFTER he pushed the call button numerous times. The bathroom was not thoroughly cleaned up for literally 6 days with feces in the front of the commode and the back! There are so many other issues with Evergreen...too numerous to mention here but PLEASE people...check out this place thoroughly before putting anyone there. Thank you.

My father was discharged from Beaumont Hospital R.O. 2/3/2020 to Evergreen Health & Rehabilitation Center. There was no edema in my father's lower extremities (legs and feet) bilaterally. When I arrived to visit my Dad 2/5/2020 at 12:38 pm, there was noticeable swelling of the lower extremities. My Dad has congestive heart failure and fluid build-up can kill him. I inquired with the nurse practitioner. The NP informed me that there was nothing she could do about the swelling in my father's legs. I asked her if she would contact the medical director (doctor) on staff to obtain an increase in diuretic or compression socks to combat the swelling. She stated that there was nothing she could do for him. I inquired with the staff nurse about elevating my Dad's legs and she stated that the wheelchair my father was in was not equipped to elevate his legs. I then asked if she would contact the medical director (doctor) to obtain an increase in diuretic or compression socks. She stated that she had called the medical director (doctor) but the medical director (doctor) had not returned her call. I then called the medical director (doctor) but there was no answer and the telephone disconnected after I heard the message, "thank you for calling, good bye". Every visit with my father, I noticed that both of his legs, ankles and feet were swelling more by the day. When I arrived at Evergreen 2/8/2020, my father's ankles were pitting +4 and his legs had blisters, open sores and were weeping. I also took pictures of my father's feet and legs. I complained to the staff nurse again and she stated that the medical director (doctor) was not responding to her call. I had to believe her because I called to speak with the medical director (doctor) three days in a row and received the same message "thank you for calling, goodbye" prior to being disconnected. On Saturday 2/8/2020, I placed two pillows underneath my Dad's feet prior to putting him to bed. When I arrived Sunday, 2/9/2020, I noticed that some of the swelling had gone down. I then realized that the staff nurses nor cnas had been placing a pillow underneath my Dad's feet to decrease the swelling. It was then that I knew I was removing my Dad from Evergreen before they caused irreparable harm.

Other than the OT, PY and ST people, the resy of the nurses(?) and aids suck!!! 30 minutes for assistance for basthroom help. By thne, he had wetted his pants and had to sit in it. Food service was more than rude in the dining area which according to the workers, the day room. If someone gets the person out of bed in the morning, they are dressed (kinda, still missing clothes that were supposed to be labeled and returned after discharge) and wheleded into the "day room" until breakfast lunch and dinner is served. Do not take a memory challenged person here!!!

We found out today that my mom does not have to return to Evergreen to finish her rehab. I’M SO THANKFUL!!!!!! They’re going to do it at the hospital. Many but not all of the nursing staff and aides aren’t doing their jobs —- just barely enough to get by. Word to the wise - make sure that you consistently and frequently check on your loved one. Otherwise, they will suffer because staff aren’t doing their jobs.

My mother entered Evergreen Health and Living Center on April 30, 2019 for rehab and was there for over three weeks. My observations are mixed. On the good side, it's a beautiful facility, and most important, her physical therapy and occupational therapy were very good. I live far away and was able to come in to town to see two OT sessions. The therapists gave her exercises that pushed her and the facility mimicked a home situation: bathroom, kitchen, stairs. They also arranged a special session with me there, where I showed them Mom’s bathroom setup. If my calls to staff weren’t answered right away, I did receive a call back (especially from Heather, the head of the therapy department), with detailed answers to my questions and a detailed description of Mom’s progress. I also received helpful information on Mom’s medical condition from the medical records department. Mom said all the staff were very kind, and the ones I met (e.g., nurses, attendants who brought meals) were extremely pleasant and attentive. One problem: even though on Mom’s whiteboard in her room it said “no coffee, only hot water with meals”, the staff kept bringing her coffee. A few problems did arise that related mostly to administration. I called Evergreen on May 1 to see about Mom’s rehab. The person at the nurse’s station was new, and said that Mom wasn’t on a list for rehab, and that they didn’t know anything about her. That disturbed me, and I left a message for the therapy department; they called me back later and told me that with the new staff mistakes were being made, Mom was being evaluated, and that an update would be called to my sister (primary contact) in several days. On my mother’s pre-determined discharge date, one of her caregivers came to pick her up, and no one at Evergreen was prepared for her discharge according to both the caregiver and my mother. No one had her paperwork ready, no one helped her pack, and both women had to wait at least a half hour for an Evergreen employee to pull the paperwork together. This is unacceptable. They should have had the paperwork ready, her belongings packed, and should have had Mom waiting at the door in a wheelchair, ready for her caregiver to pick her up. Her caregiver had to do all this for her. So I’d give three “grades”: a “5 star” for her rehab, a “3 star” for the on-floor staff, and a “1 star” for the administration.

Well as you will read, Evergreen is not the place for your loved one. It is very difficult to believe this facility is affiliated with Beaumont Hospital! With a few exceptions, the nursing staff was indifferent and most were downright lazy! My mother was there for one week and that was long enough! I visited everyday and found her still in bed, sometimes not bathed. As her condition began to deteriorate, the doctor finally ordered lab tests. The unit clerk did not call for the samples to be picked up until 4 hours later, after I asked about results. The weekend staff is lazy, rude and only willing to do the minimum. I had to bus her trays, change her bed and get her cleaned up. One of the CNA’s had the nerve to have an attitude because I asked for another blanket. Evergreen is severely understaffed (nursing) and customer service is nonexistent. The bright spots: the therapy team and the maintenance crew. Avoid this place at all costs; you could be saving your loved one’s life!!

My husband was there from August 10th and discharged on September 13th, 2018. The attendants in his wing where good to him (maybe because we were always there). They were informative when inquired about his day. The PT and OT were good. However, upon his release on the 13th he seemed weak. Got him home and he refused to eat. The next day he was weaker and would not eat or drink. He was taken back to the hospital by an emergency vehicle and admitted with an urinary track infection. Why didn't they know. This did not happen overnight. Now, he is worst now than he was when he first had his stoke. The caregivers gave good care (because we were there a lot). Never saw a doctor (called and asked to speak with her). and the PA seem to be in LaLa land. 5 stars, DON'T think so.

This is my dad's second trip here for Rehab after a serious infection. The only true good thing here is the PT! Everyone tries to pass the buck of blame, they are either short staffed or have hired people that just don't care about the quality of the care they give people - and that is on every level but Physical Therapy. When you do inquire about an "issue", you are told it will be taken care of - but it never is. Multiple phone calls go unanswered. It took 4 days to change a soiled pad on my dad's wheel chair - 4 DAYS! That is unacceptable. When my dad was discharged the first time, they wrote that he didn't need assistance with anything - but he NEVER did anything without assistance! Because he "refused" a shower - he was sleeping and they woke him up (they showed up a 9pm at night finally to bathe him when the schedule says 9am-3pm) - they put he didn't need assistance with hygiene on our discharge papers. Yet they had to have 2 people assist him with showering and not once did he ever do this himself. I asked them to correct it and they gave me a hard time about it. In case anyone thinks this isn't an issue? When you try to apply for VA or Medicaid Benefits - they look at what the facility had to say about his ability to care for himself. IT MATTERS!!! I reached out to the Administration about the issues we faced - it took 3 phone calls and 4 days until someone finally called me back. This facility is the only one close to our home which is why he is there. But this is NOT by any means a 5 star facility. I am truly disappointed that Beaumont rates this as one of their "top" facilities - makes one wonder if Beaumont ever bothers to look at the multiple reviews of problems this facility has logged?? I find it interesting that , of course, they only show the best, glowing 5 start ratings they received - but dig deeper and you will find that this facility only gets a 2.5 star rating.

This place is awful. My mom was there. She had bed sores, wasn't eating or drinking. She was not taken care of very well. She was discharged not long ago to go to a different nursing home, the discharge nurse was extremely rude and she argued with me. She died yesterday and I feel that Evergreen is responsible partially because they didn't do their job. I would recommend not sending your loved one here.

Evergreen is a very high quality run facility under incredible administration. I have had several relatives stay there. My grandmother stayed there after receiving a pacemaker surgery when she was 84, which had left her very weak. She went to evergreen for physical therapy to regain strength , and received very high quality care and assistance for all of her medical needs. Upon her discharge from the facility, she felt stronger and healthier than she had in years. This is just one example among many of the high quality care given at evergreen. My great uncle and my best friends grandfather also stayed at the facility, and we have all been extremely satisfied with the care and hospitality that is present at evergreen!

My grandmother spent 2-3 weeks here on three separate occasion, her and our family were very satisfied with everything the facility offerred. The nurses and staff are very attentive. Even helped sterilize toys for gramma's youngest visitors to keep them entertained while providing excellent care. I would stongly recommend giving this place a chance for your loved ones. Leo

My grandmother stayed in this facility on three occasions, each for 2-3 weeks after hospital stays. Great place for recovery or staying if your loved one can't go home (yet). The place is clean, residents and staff are friendly, nurses attentive. I had two toddlers with me, both required entertainment. The nurse helped sterilize some toys for them to play with while accomodating the patient's various request. After experiencing other similar facilities, I would strongly recommend giving this place a chance.

I am currently a patient at Evergreen and would like to share my experience so far. In a word, it has been nothing short of INCREDIBLE. When I arrived, I was barely able to walk 20 feet and certainly not up the stairs to my apartment. My therapist (Ron) has helped me tremendously. The nurses and aides are all very helpful - even when I can be a bit grouchy myself. I'm looking forward to going home - but only because I know I'm going to be safer and less likely to fall having worked with all of the great therapists here.

I'm so thankful my mom is home and doing well, after being in Evergreen Health and Living Center for 2 1/2 weeks certainly I owe them "no accolades"" People please if you read this review, do not put your loved ones in this facility. The medical care is inadequate, the food is lousy, and most of the CA's that are staffed here are lazy. I dediced to place my mom here after I was told by Botsford/Beaumont in Farmington Hills that she needed to be in a rehab facility to become stronger before returning home, and it would be easier for her medication to be monitored if she stayed in network with a Beaumont facility. I looked at 5 of the 7 facilities Beaumont offered, and this appeard to be the best out of the 5. "WHAT A JOKE", this was one of the most ill-run facilities for elderly patients that I have ever seen. My mother did not even see a doctor, until I took off an entire day from work, called Dr. Winters local Livonia office, and called Nurse Manager Stephanie at Evergreen Health. He then appeared on scene after she had been there about 9 days. My mom is a diabetic, her blood sugar was not monitored, her vitals were not taken reguarly and when they were it would be 4 or 5:00 pm. I eventually brought her glucose machine from home and we took her blood sugar ourselves. Dr. Skoures (my mom's primary doctor) called and gave instructions to Evergreen on how to monitor/and do her insulin and they could not even get that right. The dietcian showed up about a week after my mom was admitted, took her food order for 2 days, and after that she never saw her again. The food that was served was cold and most days not edible, so almost everyday I visited, I would bring her dinner from home, so she could have a decent meal. She had 3 nurse assistants, that were kind, considerate and caring, Stephanie, Nicole, and Angela. The others were noncaring, noncommitted and seemingly just there to get a paycheck. A few nights she put her help light on and waited over 2 hours for assistance to go to the bathroom, and when ALMA arrived, she told my mother to hurry-up, I have other patients to attend to. UNBELIEVEABLE!!! My mom was truly sent to this place for rehab therapy, so a shout out to AARON for all the help he gave her. He did his job! The facility is pretty and very up to date, but like the old adage says "NEVER JUDGE A BOOK BY THE COVER", and please by all means avoid this place if you love your loved one. My mom was discharged on October 19, 2017 and is doing fairly well, she is 78 years old. I wonder what happens to the elderly people at Evergreen, that don't have close family and friends to speak for them and don't have a voice???

My mom is a diabetic, her blood sugar was not monitored, her vitals were not taken reguarly and when they were it would be 4 or 5:00 pm. I eventually brought her glucose machine from home and we took her blood sugar ourselves. Dr. Skoures (my mom's primary doctor) called and gave instructions to Evergreen on how do monitor/and do her insulin and they could not even get that right. The dietcian showed up about a week after my mom was admitted, took her food order for 2 days, and after that she never saw her again. The food that was served was cold and most days not edible, so almost everyday I visited, I would bring her dinner from home, so she could have a decent meal. She had 3 nurse assistants, that were kind, considerate and caring, Stephanie, Nicole, and Angela. The others were noncaring, noncommitted and seemingly just there to get a paycheck. A few nights she put her help light on and waited over 2 hours for assistance to go to the bathroom, and when ALMA arrived, she told my mother to hurry-up, I have other patients to attend to. UNBELIEVEABLE!!! My mom was truly sent to this place for rehab therapy, so a shout out to AARON for all the help he gave her. He did his job! The facility is pretty and very up to date, but like the old adage says "NEVER JUDGE A BOOK BY THE COVER", and please by all means avoid this place if you love your loved one. My mom was discharged on October 19, 2017 and is doing fairly well, she is 78 years old. I wonder what happens to the elderly people at Evergreen, that don't have close family and friends to speak for them and don't have a voice???

When walking into the facility it appears very nice. Everyone is courteous and smiling. The rooms look okay, but there is very little supervision about how many visitors a person can have at one time or late they can visit. My room mate had about 7 people at once, all laughing and trying to out talk the other. Needless to say I did not get much rest that evening. I experience another traumatic occurrence that had me so upset and sickened that I fell an injured my hip replacement. Nothing was done about the situation until I complained the second night and was so upset and shocked at what was going on in the room that I could not go back into the room. It seems some people who have been in the facility for a long time do not have to follow the rules about visitors and when visiting hours begin and end.. The nursing care service is nearly non-existent, the food is practically trash. The only things I felt safe to eat were pre-packaged items, like crackers or cookies.

My Mother was a patient for 20 days. In that time, the PT people were wonderful but beyond that, the food was sub-par and the aids were Horrible. Because she required ambulatory assistance to go to the toilet they left a commode at she waited more than an hour for assistance on several occasions, and the commode was left filthy for more than 5 hours after repeated requests to clean it. It took me 3 Days, dozens of phone calls and a direct order from her primary physician to get her medication in order. I would NEVER consider allowing my Mother to stay the EVER AGAIN!

My mother in law was taken to Evergreen Health & Living Center after her hip replacement was done at Beaumont Royal Oak Hospital because her first choice rehab nursing home was full. We figured since it was owned by Beaumont that it would be good, but we were sorely mistaken. They have only ONE RN on duty at a time for the entire facility (hundreds of patients). The "nurses" supposedly caring for the patients were all LPNs. This is their way to provide fewer patients per "nurse' as they can pay the LPNs far less money. The drugs are distributed by the assigned LPN, but most of the patient care is provided by aides. My mother in law needed soft food because Beaumont lost her top dentures when she went in for surgery. She would receive food she could not chew. I would complain and she would then receive the right food. No one was monitoring how much the patients were (or were not) eating or drinking. I was there to advocate on her behalf 12 hours a day and she still had lousy treatment by the staff. I pity those without someone to advocate for them. Her first roommate had severe dementia and a broken foot/ankle that was in a cast. She kept crying that she wanted to get out of the wheelchair she was strapped into and kept asking me why the cast was on her leg. I would tell her repeatedly that she broke it and needed to stay in the chair. I later advised her assigned LPN of what she was saying/doing (not that I was complaining, but that I was concerned about her). The next day she was so drugged (while strapped into her wheelchair) that she only woke up when she was roused. The aide walked in, woke the woman up, set her meal in front of her, and walked out. She walked back in the room an hour later, found she had not eaten a bite, woke her up, asked her if she wanted to eat any more, the woman said "no", and she walked out with her food, untouched. I advised the LPN who acted like I was being a busybody and she told me to worry about my mother in law. The next day my mother in law was moved into a different nurse's group who had fewer patients because they were more seriously impaired with the explanation that my mother in law needed more care (not that the other woman needed more care). I complained to the one RN in charge of the nursing home and she apologized. Unfortunately for my mother in law, she became dehydrated and weak from the lack of nutrition and fluids and was readmitted to the hospital after about three days. She later died in the hospital and I believe that the nursing home was complicit in her decline and later death. The physical building, grounds, and furnishings are in nice shape. The physical therapy they provided and the people in the physical therapy department were great, but if the patient cannot fend for him/herself, it is a dangerous place for them unless they have an advocate sitting there with them (and even then...).

My mother was in an assisted living facility and needed a higher level of care. Evergreen Health and Living Center's staff and facility have met her needs. Medical attention, care, and food have both met and exceeded expectations. This was a decision I had to make on behalf of my mother, I am reminded on each visit, why I selected it.

My 90 year old mother went there after knee surgery. Normally, she almost never complains as she wants to be liked. When she started complaining to me about this facility I knew something was wrong but by then it was too late to put her somewhere else. She told me she sat in her urine three nights in a row. No one would come change her. She ate peanut butter and jelly sandwiches because the food was so bad. She was in extreme pain starting at 8 pm and the doctor would not adjust her meds .. She tells me she will never go back there. I tried to talk to her nurse a couple of times. Left messages and no one called me back. My mom also asked for saltines one evening as she is a diabetic and did not eat her dinner and 2 hours later someone came by w a bad attitude and gave her the crackers.. This place is affiliated somehow w Beaumont Health and Beaumont should disengage immediately. Their standards cannot be these standards. I asked that the director of the facility call me. I am very disappointed and distressed. I not only do not recommend this facilty I warn against it.

This is the ABSOLUTE worst place. The communication is awful. NO ONE knows anything. You ask a simple question of the receptionist and she is unable to give an answer. She wasn't even aware of the staff and their positions. They are not familiar with the patients and are always referring to the chart. I am not sure if they just rely on the family to not know any better? The night staff is too busy chatting about their lives instead of assisting patients to the facilities. My mother was told on a daily basis to just go in her "diaper" because there was no one available. Several members of the staff are telling her different release dates and have her so confused when we are already dealing with a VERY CONFUSING time. I truly wish that undercover boss would come and see what type of place they are running. I would scream from the roof tops to run the other way!!!!

Evergreen offered excellent care. The PT and OT dept was excellent. Whenever I had a concern, it was addressed immediatlely and a solution to the problem. Nurses were great, ass't were great and came very quickly when you pushed the bell. Food was very good. Menu had variety and they accommodated your requests. Everyone was very friendly. Overall, the care was very good. Would have liked to seen more activities but there were about 3 each day. Ice Cream social hour daily.

My 88 year old mother fell, spent a few days in the hospital, nothing serious just a bruised shoulder, then to Evergreen for rehab. First day there there was no therapy, second day there was the evaluation, that was a Friday. There was no therapy on the weekends. Mom called me Sunday afternoon saying she wants to come home. They left her laying in bed with Saturday and Sunday. Zero physical activity other than bathroom breaks. I called Evergreen and told them that I was taking her home, the head nurse (are they really RNs? I don't think so) gave me the hard sell why she should stay, she has actually very rude. When I picked her up the head nurse again gave me her speech in a rude condescending tone. I told her 'I know but it is what mom wants'. Next week my mom had a representative from Dept. of Health & Services investigating me for not acting in my mother's best interest with removing her from Evergreen. People - stay away from this place.

My mother was a resident here for several weeks following a hospital stay at Beaumont. This, luckily, was our first experience with a facility of this type. We knew numerous people who had been in this facility for rehab over the years and selected it on that basis. First, the good: the physical therapy staff is excellent. I doubt you can do better in the area. The meals were better than I expected. The rooms were OK; my mother could not get a private room but the design of the room made the shared room less objectionable. Some of the nursing staff and assistants were competent and caring, Now, the not so good: my mother, who has never been incontinent in her life, was put in a diaper on the first day. It soon became apparent that the reason was that the staff was taking an incredible amount of time -- half an hour or more -- to answer the call button. Pretty soon, she quit eating and drinking so that she would not be embarrassed by her inability to get to the facilities. Similarly, her chair and bed alarm repeatedly came on and no one ever came to check. At one point, she needed medication; I personally rang the call button and waited two hours for it. Care was, at best, chaotic; I never had a sense that anyone had a sense of why my mother was there or what she needed, Items we took in related to her care disappeared almost instantly. They were supposed to do her laundry; if they did the laundry, her (labeled) clothes disappeared into the lost and found, but for much of the time, the dirty laundry stacked up so long we took it home and did it ourselves. My suspicion is that this place is neither better nor worse that similar places of its kind. But our family had to resort to keeping someone there most of the time because of the lack of care. I would not recommend this facility for someone who is unable to care for himself/herself and lacks family in the area to take over for the shortcomings of the facility.

I did not like Evergreen Health and I would not recommend it at all. I was not impressed with anything. It is an affiliate of Beaumont Hospital, and it was where mom was sent when she fell. She was in the intensive care, so I figured that would be a really good place to put her because I had a good experience with Beaumont Hospital, and my brother heard good things about it. I think its fine for people that are recovering from a broken leg or hip replacement or something like that, but for an elderly person that can’t get out of a wheelchair, absolutely not; I think it’s terrible. They were understaffed. Two rooms are divided by about three quarter lengths wall so they are fairly separate, and then those people share a bathroom. I wouldn’t say it was too updated but it wasn’t dirty or filthy, but they update on the furnishings and the carpeting and the paint in most of the facility.

My husband has been in Evergreens for two weeks now, and I go there every day. My husband has Parkinson's, and he was there two years ago for rehab. He is happy there. He loves the food and likes the staff. He also likes the physical therapist; that's why he went back. They are great. They keep him very clean, and they do a lot of therapy with him, like speech and occupation, so he has a lot of attention, which I like. He is well cared for, and he is walking better. He has help with taking his shower and a lot of other things. He is in a semi-private room, which is nice. It is a very nice facility. It is all on one floor, and I am not fond of high-rises. It has a lovely dining room and beautiful courtyard. They have activities, and they are having bingo today. They have an ice cream social every day at 2 o'clock. They have jazz players come in and play twice a week. They have trivia pursuit games. They have a lot going on. It is a nice place and he is really happy there.

This place is terrible. My elderly father had to go here for rehabilitation after a fall. He absolutely hated it at Ecergreen, he asked to go to the bathroom and the aid told him to "go in his diaper". It was humiliating. We couldn't get dad out of there fast enough.

Evergreen was wonderful. When we brought my dad there, we didn't think he would make it but they did wonderful with him. When he walked out of there he was better than when he had his heart failure. The accommodation and the staff were good. It was very clean and the doctors were very informative.

This place is horrible. They keep the residents sitting in a room all day at a table just kind of staring at the wall, and my poor dad is convinced he's in prison. They also have lied to us on multiple occasions. We found a wonderful assisted living facility to move him to, but they waited till the very very last moment to tell us we needed a mrsa test for him to get out. Now they have been giving us the run around, lying to us that he didn't pass the test then we later found out they never even gave it to him, and they keep changing the rules on what tests he needs every time we talk to them. Dad hates it there, and he's going to miss out on a wonderful place cause they can't get their act together. He says too that when he pushes the button, they don't come, sometimes for hours. Don't let anyone you care about go there. They should be shut down.

OMG! My friend is there and they havent cleaned her room at all. I looked down and there was a dead mouse in a mousetrap that looked like it had been there over a week atleast. There were spiders and several ants also stuck to the poison. I took a pic of it. Her mail was under the bed including bank statements, and there was cups and candywrappers etc under the bed. The mats are there to prevent a fall but people end of walking on them and spilling stuff and they dont seem to have adequate cleaning people at all. Even after complaining I had to leave and they still hadnt really cleaned the room, they seem to not be able to see the whole picture, one of the nurses said they arent allowed to touch the mice. All i know is for $100,000 a year the local holiday Inn would give you better service. They need to hire more people and stop trying to please their shareholders. This place is a MRSA breeding grounds. And how about giving my friend some new water once in awhile. I hope i never get old, the nursing homes in this country are just awful.

My mother has Alzheimer's and dementia. After her hip replacement, she would forget that she couldn't walk. Evergreen Rehab Nursing Center is very large and they couldn't watch her all the time. So when she was in a wheelchair, she would try to get up and several times she ended up falling and splitting her head open and having to go to the hospital. It's a good facility, but they can't watch the residents 24/7. A lot of them were frustrated, my mother included, because the residents were stuck in a wheelchair and they pull them up to a table. And that's where they are most of the day because they weren't allowed to stay in their beds. The staff are really friendly and the physical therapy is great. They have everything you need there. Every room has a nice view outside. It's really nice. It's just so huge and there is no way you can take care of that huge group of people and watch them 24/7.

After hearing great reviews of Evergreen Rehab Center, I carefully took a guided tour for myself. Very nice and clean facility. Staff seemed friendly, especially when a guided tour is being conducted. My father was transported from Beaumont Hospital to Evergreen on a Friday afternoon. My family felt comfortable prior to leaving that night. However, when I arrived at 9:00 a.m. the next morning, my father had dried up stool in his diaper and also within the crevices of his buttocks. My father told me he pushed the call button after 12:30 a.m. and the aide came 1 hour and a half later. He informed the aide that he was sitting in his stool and asked if she would change him. He also asked the aide if they were short staff and she replied no. The aide left the room and never returned. My father sat in his stool for over 9 hours. I informed the administrator and the nursing supervisor that this lack of care and concern is totally unacceptable. Feces left on the skin causes risks for bacterial infections, viruses, and bed sores to develop. In addition, my father could have fallen or had chest pain, and to wait over a 1 hour to respond is INEXCUSABLE. I asked if this aide had other complaints filed against her, but neither the administrator nor the nurse answered my question. They apologized and assured me that this would not happen again. I insisted that this aide not come near my father again. Unfortunately, the ratio is 14 patients per 1 aide and 1 LPN staff who only administers meds but does not give direct patient care. Evergreen needs to have a measurement tool in place to qualify the type of direct patient care provided by the aides especially during the late to early hours. I am terribly disappointed with the care my father received on the 1st night of his stay at Evergreen. We are certainly in the process of locating another facility to provide quality care at on the midnight shift as well as in the day shift.

I visited this facility

This facility seemed to be one of cleaner, warmer, less clinical, more involved place for my grandfather. They do regular social activities, have beautiful outdoor courtyards, and very attentive nurses. Even though he had been acting very unruly and not himself, they removed a certain medication we weren't familiar with and he got better after that. He was being very picky about his food and what he would eat or not. The nurses encouraged us to bring in any of his favorite foods and treats so they could get him to eat more of what he liked. They made space in the pantry and fridge and gave him whatever he wanted. Ice cream social day is nice... Visitors can enjoy a cup of ice cream with the patients and stroll around the facility and courtyards. The only thing bad that happened during his stay with them was that he rolled out of the bed. Not sure who was more at fault, there wasn't any thing that could have been done to prevent it besides large bed rails that stay up.

When I first walked into Evergreen Health and Living Center, I could tell that my relative would enjoy it very much. You are first greeted by a front desk assistant who directs you toward the room you're trying to visit. All hallways lead through the dining room, a large, well lit room with a player piano. This room is lined with open windows, as well as couches and chairs for residents to visit with their family. When visiting my relative, I always noticed that her food was delivered to her consistently and she was treated with great care by the staff at the facility. My relative also had to undergo physical therapy, which was painful and difficult, but the doctors and staff helped her through it tremendously. Before I knew it, my relative was walking and living on her own after breaking her hip months earlier.

I am a friend of a resident of Evergreen Rehabilitation and Nursing Center. In general, I thought that the facility was a nice place for a love one to live or to receive rehabilitation. The facility had great décor and the facility was very clean. The facility did not smell and the food looked good, and the dining area was formal. The facility also serves the residents and their visitors various flavors of ice cream between 3:00pm and 4:00pm 7 days a week. Evergreen Nursing and Rehabilitation Center was beautiful with all of the décor matching and coordinated it was very tasteful to the eye. The grounds of the facility are beautiful and well maintained and very well manicured. The common areas of the facility are beautiful and well structured and the surrounding neighborhood is beautiful with lots of land and a large structured parking lot in the front and the back of the facility. The residents' rooms are very contemporary and all décor and bedspreads matches the wallpaper on the wall. The staff and management of the facility are professional in all areas of their job duties. I think the residents get excellent care there. The visitors can also order, lunch or dinner and eat in the dining room with their friend or family member. While I was there, I noticed that in the early afternoon the facility had a musical group singing for the residents in the formal dining area, and I noticed that in the evening after dinner some of the residents were watching movies on a large movie screen in the recreation room. I enjoyed the time I was there visiting a friend of the family the facility was not at all like a facility but more like home and some of the residents rooms had one to two couches in them that matched the wallpaper on the wall. I would give Evergreen Rehabilitation and Nursing Center a five star rating. Overall, Evergreen Rehabilitation and Nursing Center is a great place for a loved one to be placed. The staffs give the resident's great care one on one and they communicate consistently with the residents and I would recommend it to everyone/anyone, who is looking for a placement for their loved one; because I believe that their love one would do well there.
