My mother moved into SKLD Plymouth. They are very caring and very thorough. The facility is clean. They have knowledgeable staff members. The food is good, and my mom is eating it. They do have activities, but my mom's unable to participate. I love their group dining. You can get your hair done. They have crossword puzzles and coloring. I haven't seen anybody unhappy. Everything is covered because my mom is a dialysis patient, so we don't have to pay anything. They make adjustments where needed. If something is off track, they try to ensure that it gets fixed immediately. For example, when my mom gets back from dialysis, they try to make sure everything is warm and ready when she gets situated. I appreciate that.

Room small and crammed with furniture. Constant falling, which I wasn’t notified unto I discovered cuts and bruises then notified nurse on duty. Constantly crushed meds that include ER meds. Constant mobile residents men and women in Her room stealing anything edible and taking her private blankets. I visited every other day for 4 to 5 hours during 5 year residents. No one from Yehuda’s admin departments called or emailed or texted condolences after my sister passed. Once the payment stopped so did any caring. All of admin uncaring and unsympathetic to my heart breaking loss. What a bunch of money making hypocrites. If it weren’t for the caring staff this place would rate a zero. Mo wouldn’t put her on activity program / said it wouldn’t work.

Do Not, under any circumstances, bring yourself or any family member here. Nursing care is horrible! Physical therapy was excellent, but general care needed much improvement. Staff never answered bells, forcing pts to attempt going to bathroom on their own, causing falls. Then the staff yelled at patient for getting up alone. My mother was never given a choice with food selection and was forced to eat in her room. They lied about costs. Told it would be covered by Medicare. Forgot to mention the "copay." Ended up to be $4000. And she came directly from hospital after a fall where she fractured her pelvis and wrist. Never again! If I could rate zero stars, I would.

Worst discharge for the patient. We were lied to about post care & equipment.
