This facility should be under review.. From the moment you walk into the main lobby, it is unwelcoming, unclean, old and tired. The majority of the associates are non-English speaking, weak communication skills, unprofessional ,unmotivated, and many are uncaring. Having said that there are a handful who truly are caring, attentive, and gentle with the residents, and, those who are admitted for rehab. I also found many of those who are admitted to the rehab section of the facility can walk around without help or assistance, dress – bath themselves, go outside for walks, smoke, run their own errands, shop, go home and come back to the facility once twice or more a day which makes me wonder why many of them are there. The kitchens on the floors staff only are dirty, counter tops soiled, floors sticky, dirty, refrigerator’s filthy. Culinary services are uneatable, cold food, melted ice cream cups,( which are put back into the freezer – serve safe 101! cold coffee/soups. Many occasion I have entered the building late in the evening and or early in the morning to find associates sitting in the dining area on cell phones, heads on the table, talking to one another while two call lights were one, visible over the door and at the nurses’ station. One being my family members when I entered the room my family member informed me she had been waiting 10 mins, I had to go ( this was more than once) to the dining room ( dark) to get an aid to assist my family member. A male guest was wondering the halls, entered a female guests room at 2 am sat on her bed awaking her, nothing was done! When the Administrator was called it took him three days for him to return the call in regards to the male sitting on the females bed at 2am. Again, this speaks to the lack of care and attention. My mother was having difficulty breathing when an aide was assisting with changing her as I stood outside the room I heard the aide who spoke very broken English tell her it's ok it's ok stop!, as I entered the room upon hearing my mother’s labored breathing and the aides response my mother was gray and gaspingher oxygen was low she was not breathing correctly I not the aide called for staff. Four nurses then paid attention, had to regulate her breathing, give meds 30 mins caring for her because an aide who should know when someone is in distress kept saying you’re ok it's ok..after this incident two aides and one nurse told me if this happens again get our mother out of here! Another time I was told by my mother she did not get her medication twice in one day after asking multiple times. When I addressed the issue, a nurse looked me in the eye and lied to me, I spoke to the person whom she said was there and a witness who said she did not witness it, and would not lie my mother did not get her medication. Some maybe asking why she stayed, this was in a weeks’ time!!! ONE WEEK! At one time this was a wonderful, clean, caring facility, no longer….for the health, well-being, and safety of your loved one, thought twice before you allow him/her to enter this building.

I only have the highest recommendation for this skilled nursing home, Our mother was at another facility the had a great reputation, but it was anything but great for our mother. She ended up with a terrible bed sore, and was not recovering like they said she would. I ran into a woman who works for Autumn Village and I asked her how their rehab program was. After speaking with her, we decided to move our mother ...in what seemed like no time, her wound was healed, and she was up walking and getting the right type of therapy. She is home now, thanks to the caring staff at Autumn Village!
