Rooms are spacious and kept clean. The entire facility is kept clean by having a housekeeping staff that spends the entire day cleaning. This group of people are not just here for a few hours to tidy up rooms. . . they are cleaning all day long. On a daily basis, my room is swept, mopped, toilet & sink areas wiped down and sanitized, vacuumed carpets, cabinets and window sills are dusted and wiped down as needed. Trash is emptied multiple times a day. The laundry is labeled for you and returned clean in a timely manner after you send it out. There are many activities scheduled and provided for residents. There are several outdoor courtyard areas to enjoy the weather amongst various beautiful flowers and archways. Physical Therapy and Occupational Therapy are scheduled and kept to that schedule, but also flexible where needed. Therapy is targeted and not just a 'fly by the seat of your pants' plan. It is planned, and adhered to on a daily basis in order to reach goals. Goal planning includes the patient and family along with therapists, nurses, etc. The meals provided to residents are adequate & controlled portion sizes that are very tasty. Fresh ingredients are used, and made from scratch - No dumps from a can here! Nurses are attentive and helpful. The care given matches with patient needs well. Call lights are answered in a timely manner. Technicians & Assistants are helpful and quick to meet needs. I moved here from another facility that was just terrible, and the care I received here has been above and beyond the other facility, and I am so much happier here. This facility has Assisted Living, Short-Term Rehab, Long-Term Nursing Home, reaching all degrees of health care needed. I am here for Short-Term Rehab for Physical Therapy following surgery on my hip . . . I don't expect to be here very long, but if I needed to extend the stay, I would have no hiccups in choosing to stay here again. The staff seems to WANT to be here, they WANT to help patients . . . they are not 'just here for the paycheck.'

Prairie Lakes Health Campus was very nice. I visited my friend several times and she was very happy with it. Her room was small. She had a bedroom, a small seating area, a small kitchen area, and a bathroom. They had a room where they played cards or games, and I saw the eating area.

The care has gone down hill considerably. We have been patient due to covid but this is the reality we live in. Raise some wages if you don't want to be chronically understaffed. Which they are. The doctor that they contract with refuses to communicate with us and simply will not respond in a crisis. Unacceptable. In response to a situation with my mother having an episode of confusion and missing breakfast nobody got food or water in her. They opted to help dress her instead of going with food and water or calling myself or my sister. When my sister got there a little food and water helped. Very disappointed with them and moving is so disruptive.

My mother-in-law is in Prairie Lakes Health Campus. They're magnificent and I am so pleased with them. They're great. They take good care of her, she's happy, she's well-fed, and she's eating three meals a day. Every picture I get she's smiling. The grounds and location are fantastic, and it's accessible by a local bus. People are able to come and visit her. She's participating in absolutely everything, she does crafts, painting, morning exercise routines, and she's very happy there. I've tried their food and it's delicious. It's excellent and I'm very pleased that she's there.

Prairie Lakes Health Campus was a very nice facility. If we would've gone in an assisted living community, that's where we would have gone. It was very clean and very nicely taken care of. The staff was very nice to us, gave us a nice tour, and overall I was very impressed with it. It's a little bit out of town, and it's in an industrial area, but it was fine. They had a nice courtyard where mom could've sat and enjoyed the day.

My father is living in Prairie Lakes Health Campus and everything seems fine. They're very good. It's quiet and pleasant. Everybody is nice. It's easygoing. They could use some pointers on the food, but it's good. He says it's fine and he's not starving. They're just inconsistent. They need to warm the plates and they need to cook the vegetables a little more. The staff is wonderful. The nurses and the administrative people are all really good. I haven't met one I didn't like yet.
