After hearing good reviews and taking a tour, we moved my mom into Heritage Woods in August 2023. I go to visit often and honestly can say each time I am there I am continually impressed with their facility. Residents and staff are friendly, caring and welcoming. There are 3 nutritious meals a day, housecleaning, laundry services and plenty of activities planned to promote community and getting to know each other. Most comforting is knowing there are CNAs there 24/7 so if my mom does need something they are there to help. They check on her often and I'm extremely grateful for them. Every time I go to Heritage Woods, I'm more convinced this is exactly the place my mom needs to be. Todd M Freeport IL

Our community provides a wonderful alternative to a nursing home or to those struggling alone at home. Among the services included in the surprisingly affordable monthly fee are three restaurant-style meals each day, plus snacks; housekeeping and laundry services; health monitoring; nursing assessments; help with medications, and assistance with activities of daily living.
Designed for adults 65 and older who need some help to maintain their independence, Heritage Woods is located on more than five acres of property on the southwest corner of West Pearl City Road and south Forest Road in Freeport.
Heritage Woods of Freeport is certified to operate through the Illinois Supportive Living Program, which enables the community to serve older adults of all income levels.
The community was developed through a partnership between Gardant Management Solutions, the largest provider of assisted living in Illinois, and the Stephenson County Nursing Center Foundation.
Financial assistance is available for those who do not have the resources to afford the monthly fee. Older adults who are on Medicaid or who only receive Social Security benefits can qualify for residency.
The emphasis at our community is on providing residents with the Love, Compassion & Dignity they deserve and on helping each resident maintain as much independence as possible for as long as possible.
Heritage Woods of Freeport combines residential apartment home living with personal assistance, support services and opportunities to enjoy the companionship of friends and neighbors and participate in a variety of social, educational and recreational activities.
The two-story building houses 76 studio and one-bedroom apartments, each with its own kitchenette, private bathroom with shower, and emergency alert system. Community areas include an activity room, beauty/barber shop, convenience store, dining room, exercise area, library, multi-purpose room, outdoor patio and television lounge.

I was very happy from beginning to end of the tour very well explained, we were greeted by everyone we walked past and given an activity calendar to see the variety offere. This community has all we are looking for.

My mother was a resident for 5 years.. she received hurried nursing care, was belittled, and made to feel like an outcast. Management doesn’t promote a care staff. You get the great place song and dance! Just beware- underneath it’s awful!!

Heritage Woods of Freeport is very nice and friendly. They have Bingo, exercise class, and crafts. My mom loves the food, and she's very satisfied. My mom hass a small but very nice studio apartment, and it works for her. The people are very friendly and helpful.

My mother was at Heritage Woods of Freeport. The nurses, one in particular, were very abrupt, which just shut my mom off. She went into the dining room and was told you can’t sit here because they’re not able to sit wherever they want. It was a constant battle every time I went down there, and I couldn’t even get her to go down to eat. I could tell that there was something going on with her, and they were not giving her meds properly. I was there on a Sunday and she had not had her morning meds, which was a mood stabilizer, and the doctor said they had to be given at a certain time in the morning and in the afternoon. They told me I could run upstairs and give it to her. Well, I gave it to her and probably shouldn’t have because that night she had a little bit of bad behavior, which turned out to be a big mess. They came to the hospital and told me that she would not be able to come back to Heritage Woods or to any other Heritage Woods. They would yell at her and the other residents. Now, there's a new administrator there. The cleanliness and the newness were the positive things about Heritage Woods.

Heritage Woods of Freeport was very nice and was my dad's first choice. We liked it a lot. It was a pretty new facility, and I can't say anything bad about it at all. The staff was all very helpful in showing us around and giving us details of what would take place if he moved in there. I'd rate it as top-notch. The cleanliness stood out very much. The dining room was beautiful, looked like a restaurant, and they can order off the menu every day. He was very pleased with the studio apartment we looked at. It had a lot of natural light, and I think they had an outdoor patio. I'd give it an A+.

My first impression of Heritage Woods of Freeport was very good. It was very clean and affordable, and the staff was very good. The apartment was not like a nursing home, and was carpeted, nicely painted, and had nice fixtures and good lighting. They offer bingo and card games. They have a library, a little general store there, and a van that takes the residents to WalMart and Goodwill. They have people come in to play music for the residents and various activities going on. The food and the dining area are excellent.

I already had my mom at Heritage Woods. It was actually referred to me by somebody who lives there. So far I'm very pleased with the staff. They are very friendly, very thorough, and I can call them anytime. My mom can call me if she has concerns about something and I can just call the nurse and she will go right to my mom. The rooms are pretty small, but it's fine for her. The facility is brand new so it's absolutely beautiful. The food is good and I've eaten there several times.

Heritage Woods of Freeport was just getting ready to open so it looked like it had been complete at the time. It was in a nice area. There was easy access to main shopping areas within close range of the facility if you are able to get out. They had nice amenities, it was clean, and everything was good.
