My wife is at the Powder Springs Center for Nursing and Healing. It's run by Medicaid, so it's not going to be the highest quality, but they do a pretty good job. The facility is clean. The staff is okay. They do what they can. They have cafeterias.

I didn't like Powder Springs Center for Nursing and Healing at all. From day one, when they found out my husband was in another facility, all they were concerned about was getting paid and not caring for him. The staff didn't call me when one of the nurses pushed the bed to the highest level. Also, I had been asking for a rail from day one. They did not give me one. My husband actually hit the floor the first time he was there, but nobody cared. The food was like dog food. My husband had multiple strokes. He had lost the use of left side, but they put the food on the left side and not the right side. They should have put it on the side he can use. I broke in there a couple of times before they would let people in, just to make sure he ate some pudding or drank through the straw. They charge the nurse for them to wear masks. She was too busy trying to look pretty for somebody because she had on lipstick and everything. She didn't want to wear a mask because it would mess up her lipstick.

My brother has been a resident in the Center for 2 months and he has been taken care of very well. I also have a cousin at the center (oh my God) they love on her as if she was their own. What I like about the Healing & Nursing Center is that if you have any questions or issues they will be addressed immediately. They are a very CLEAN center and have doctors ready to attend to the residents whenever needed. Awesome activities; singing , cross word puzzles and more. I feel as family members we need to check on our family members CONSTANTLY that are residents. I have found when being consistent in visiting and holding individuals accountable everything work out for my loved ones favor. And the Director gives us her email so that if an issue is not resolved, she will step in immediately. I personally give them a 5 star.

Do NOT send your loved ones here, please. My brother has been there since the the end of December. It's been a nightmare. Now on the 4th Administrator since he got there. The level of care is horrendous. I cannot tell you how many of his belongings have "disappeared." I have never called the nurse's station and have someone answer. He's now under hospice care there and was not on his oxygen when the hospice nurse came to see him today!! Run as fast as you can.

When I go to visit this facility I have been very impressed with the communication. I have a cousin here and my questions are always answered. If I have concerns they are addressed immediately. They are short staffed and that is why it is VERY IMPORTANT for FAMILY and FRIENDS to VISIT, so that your Loved ones will know and staff knows you are there for your Loved ones. In lue of all of the viruses going around, this facility does call and inform families if one of the staff gets sick, unlike some other facilities. I like this facility.

My mother is in Powder Springs Center for Nursing and Healing. I don't really have any negative or pros at the moment because she's been in isolation. They've kept her in isolation because they've had a couple of outbreaks of COVID-19 recently. I know one nurse has been real good to her. However, every time I call to try to get an update or anything, it's almost like I'm imposing on them. So far the care is good, except for one other time where she fell. She called out for help, and they didn't come help her. The facility looks very clean. I know that they have a beauty salon that comes twice a week. They do in-house rehab, and they have doctors that come and see the patients. As far as the rest of the amenities, I have no idea because she's been in isolation. When you feel like you're imposing on people when you're trying to inquire about your loved one, it makes it really hard to get in touch with somebody up there. It kind of makes you feel like, "I really don't want your loved one to be there," to be honest. It looks nice on the outside, but I haven't really been that far into the inside. I don't know what the general population rooms look like.

I am a daughter of a decorated war veteran. My father was independent until his unfortunate bout of pneumonia. I'm not a resident of Georgia, so picking a rehab center was a little difficult. Pleaseeeee listen when I tell you. DO NOT BRING YOUR LOVE ONE HERE!!! My father has been here 3 days, no bath, barely eaten due to no one delivering food or no one feeding him. He is on swallowing precautions, and when he finally eats he has tough piece of steak with no dentures in. You cant make this stuff up. No one assists in to the bathroom. When asked everyone says the same thing, " Hes not my patient" No bedside commode, just nothing. My sister was going to buy one for him, although he has excellent insurance. Vietnam Veteran, 40 years postal worker and last 5 years Christian missionary work to Kenyan organizations. No one Deserves this Abuse. INHUMAN! I called 8 times in one day. NOT ONE PERSON CALLED ME. They put him in a room, and that was the end of communication with his point of contact person. I'm moving my father, as soon as I can book this flight tonight. Get YOUR family out of there or become his/her 24 hr nurse. FYI- Had I seen these reviews first, this placed would not have been an option!

They gave my mother the wrong prescription. The staffs took her clothes. She was never given a bath. The lady in the room with her they never give her any food or water and she had Covid and died. I had to get my mother out of that place. This place needs to be shut down. They will not return your calls or take care of your relatives.

My dad has been in Powder Springs Nursing & Rehab Center for 6 weeks and has not had 1 shower. We've talked 3 times on a shower day and no one would allow him to shower. Dad said that whenever he express his need to shower, he's told that his name is not on the list; so he returns to his room to uses his sink. Also, the misinformation, lack of communication from the beginning in regards to his transfer from therapy to long-term-care has been so much confusion. (When, where, if at all, for sure will be, will remain, will change when availability, no plan to change even if a room becomes available as been ongoing). During one of my window-visits, I saw his packed luggage as he waited to be transferred to his new room expected to be more conducive for long-term. Furthermore, my dad has been inside his room since his admission and sit by the window looking outside. I have been unable to reach the activity director as advised. NOTE: Last week, during my visitation, the new director introduced herself; I perceive that she would induce a caring and organized culture for this facility. Also, the receptionist and the nutritionist has been very helpful during this difficulty.

My mother was sent to this facility for rehab after breaking her hip and spent 3 weeks here. It was a horrible experience for her. She said the nurses were mean to her, the food was inedible and her needs were never met. One nurse yelled at her everyday I was told. She came home sick with a C diff infection which ultimately put her back in the hospital and she was unable to recover. She is now in hospice care.

Hello, I had a family member at the Powder Springs facility and it was a good experience for my aunt. The rehab department was excellent and the food looked better than my own. I can't say they were perfect, but they did work hard to resolve any concern that I had even the smallest things. The administrator is very professional and seems to care very much. The nursing staff were knowledgable and experienced and kind even when they were busy. I am very thankful for all they did for my family and I will forever be grateful.

I visited this facility

My father was in this facility for about 18 months and was very happy with the entire staff and the service provided. When I come and visit my father, the staff makes me feel very welcome, like I am part of a real family. I was very comfortable to leave my father in the care of such wonderful people. The facility is very clean and it smells good. The room is kept clean and the housekeeper is very friendly and detailed in her work. I like that a lot. My father enjoyed all the activities, specially bingo. He loved the food. I really have nothing negative to say. I thank the entire staff for being so compassionate and caring. I would recommend Powder Springs Transitional Care and Rehab to anyone. This is the right place for your loved ones.

Staff does not care about the residents. if a resident can't feed themselves the staff doesn't feed them. The doctor never comes around and the nursing staff makes their own decisions on what the residents need medically.

This place is terrible! Meds are not given correctly or on time. Food is ALWAYS COLD. Nursing staff does not check on patients when the call button is used. Promises are made to the patients by the nursing staff that are never followed through with. I would not recommend this facility to any one. Phone calls are not made to family members as promised.

My mom has been in this nursing home over 3 years and I can say finally they found an administrator this takes the time to listen and want to make a difference at Powder Springs nursing, We’re not asking for a lot only the best care for our love ones.. This place is a disgrace to the nursing rehab community. Horrible care, terrible food - even mean treatment to disabled residents. This place should be closed down.

I am/was a resident of this facility

poor Quality of care... why? Nurses- patient ratio is 1-30.. CNA always short staffed.. why? Nobody wants to come to work with 17 residents for 1 CNA.. pay? It’s not worth it. New administration just keep accepting new patients for money with less staff.. new administration is not willing to pay overtime for the staff to stay. I feel sorry for the residents.. sometimes there’s no supplies. Food are not good. So many complaints.. dirty hallway. It’s true that call lights are not answered in a timely manner because of short staff.. employees don’t stay longer..they are only good when the state comes for survey.. sad place to work and sad place to put your love ones... I wish they would increase their staff, provide supplies for the residents , change the old beds and renovate the whole building.. it’s more than 25 years old building.. cockroaches and ants everywhere even rats in one of the nurses bathroom..

I would not recommend this Nursing Home to anyone. The home is under new management and it is horrible. Nothing seems to be getting done and when items are addressed they there is no response. Residents are not taken care of properly. Seems like no one is there for the right reason. I feel like something seriously needs to be done with the new management and care of this home.

Just a few things I have seen with my own eyes - Call lights lighting up hallways, not being answered, a CNA not to be found. I have seen a residents light be on for 3 hours, while the residents bed is soaked, finally someone comes in, turns call light off and it still be an hour and 1/2 before that resident was changed. The food is horrible! Sandwiches are served every day for either lunch or dinner, sometimes both! Green beans are served every day, at least once and sometimes twice. Food is cold most of the time. I have seen residents in wheelchairs in the hallways trying to get to their meal that has been sitting on the cart for 30 minutes or longer. Have seen multiple ants in several rooms! Cops and detectives are present a lot, threats to call news stations etc as it is SO very hard to get things done that the residents need! Air/heat in residents rooms broken, SO old they need to be replaced and they refuse to do so! Most of the hospital beds for the long term residents are the old fashioned ones with the cranks near the floor at the bottom of the bed that has to be turned to control the bed, IF they can find an employee to do it! NO remotes to them at all. What facility still has these beds??? I have seen overworked, stressed employees arguing in hallways, and in turn, take it out on the residents. I have heard them be so hateful about having to change residents, which only happens twice a day, at shift change. If they need to be changed other than that, they have a heck of a time getting someone to do it. I heard a resident ask to get out of bed one afternoon, the CNA told the resident she was there to put her to bed, not get her out! It was 4 in the afternoon!! I could go on and on! This facility scares me!

Extreme lack of care and the staff show no desire to do their job. Staff are very unhappy and it shows in the way they treat their residents. So glad I got my loved one moved out of this horrible place. She suffered extreme loss in weight due to not being properly fed, and also bed sores due to not being cleaned. Unfortunately, I have nothing good to say about the facility, care, and staff at this place. STAY AWAY!

I visited this facility

I visited this place today and found out they aren’t doing what they are supposed to do with my loved one. When I was leaving a old man in a wheelchair was trying to come in and he fell on the ground and the recipient lady just sat and stared at him. The rest of the staff didn’t help him up either. A nice man leaving helped him up and inside the building . This is a place I don’t want to visit again. The nurse was rude.

STAY AWAY!!! My mother was in the facility for almost 2yrs.. The experience was so horrible it's hard to talk about... The CNA's not wearing badges.. Finding medication in my mothers bed, floor and inside of her mother... Mother couldn't speak for herself she is in the late stages of Dementia and Parkston disease,,, I was their every day and saw cross contamination where the CNAs wouldn't wear gloves... Administration is horrible they don't care about the patience's... They never answer call lights for HOURS .. They would turn them off and say, I will get someone for you.. Help never came... This facility needs to be SHUT DOWN until they place the right people in administration..

BEWARE!!! The management only cares about $$$. CNAs and nurses were always short staffed and over worked. Complaints made to management regarding the need for more CNAs, but nothing ever changed. Alarms in beds and on wheelchairs go off constantly without staff checking on peoples. Oh, and the night shift is terrible. Every morning people are soaked in urine and have clearly not been given attention since the night before. urine was soaked through the diaper, through the sheets, and onto the mattress.

My dad and mom were both at this facility. Mom had dementia and had already fallen two times before she was admitted. They put an alarm on her bed, and I was there many times when the alarm was buzzing and no one came to see if she needed help. I've sat in the lunch room and watched the staff pick food off another persons tray with no gloves on their hands and carry it to someone else. I've also seen the staff, who were supposed to be serving these elderly patients, standing talking and texting on their cell phones, and even leaving the room to take a call, and leave the cart of food trays sitting there for the longest. I've also seen the kitchen staff bring the cart of food trays to the hallway to be served to the patients in the rooms, and the trays sit there for more than an hour before someone started handing them out. Food was always cold, and they could never get mom and dad's food at the same time. My dad was vomiting one day and I ran to get the nurse to help me. She handed one of the staff members a wash cloth to give me and never came to help with dad. I was there every day and watched them neglect my parents. I would NOT recommend this place to anyone who cares about their loved ones!!

My Grandmother died a slow and painful death at the hands of this facility. When she was admitted into the facility she had slight dementia and was eating a little. We admitted her on October of 2015 by April of 2016 she was gone. We were there all of the time with her. I had several conference calls with all of the department heads. they are all ignorant to the neglect that goes on in this place. They quickly medicated my grandmother, telling us that it would help with her appetite which it didin't. She was able to go to the bathroom with help when she was first admitted, but because the staff was too lazy they forced her to where diapers. I would take her to the bathroom everytime I visited. My Grandmother was left unattended and fell out of her

I picked Powder Springs for my husband because it does have a nice Alzheimer's unit, where they actually put them in an Alzheimer's unit instead of putting a bracelet on them and mixing them with the rest of the people. The staff is very nice and very helpful. They really work hard for the people. The food seems fine.

I love this facility. My grandmother and my grandfather has lived here for over ten years and we hasn't had any problems. The staff is great and the nurses are very kind and friendly and caring. If there's ever a problem we are notified immediately. We can trust the staff at Powder Springs Nursing and Rehab Center.

I wish we had never stepped foot into Powder Springs Nursing and Rehab Center. I lost my sweet grandmother on June 19th and I believe she would still be with us if she hadn't been in this facility. She fell and broke her hip on May 1st and was sent to the rehab center after surgery for physical therapy. We stayed with her everyday during the daylight hours at the facility. We had a family member there everyday by 8:30 am until she went to bed every night because of the negligence in the facility. She wasn't allowed to get up by herself to go to the restroom because she had just had hip surgery and was at a great risk for falls. She had mild dementia and would forget she wasn't supposed to get up so we had them put an alarm on her bed. I was there numerous times when the alarm would go off for 10-15 minutes before anyone would come. I was hoping it was because they knew I was there but her roommate and her roommates family members said the nurses never came to help even with the alarm going off when my grandmother didn't have family there and she would always be left to go by herself. Thankfully she never fell but she did end up with c-diff. We told them for days that something was wrong and she wasn't eating well but they basically ignored us until I decided we were getting her out of there against medical advice. One of the girls in charge that day begged me not to take her out on a Saturday because it would be too much trouble for her since she didn't normally handle dismissals. We insisted we were getting our sweet grandmother out of there but before we left I noticed she had a fever so we took her to the hospital to get checked out first. They told us at the hospital she had contracted c-diff and started treatment for that but she never responded and kept going downhill. I really believe if the nursing staff at Power Springs Nursing & Rehab had really taking care of her and evaluating her then she would have been treated earlier for the c-diff and there would be a good chance she would still be with us today. I would never ever consider putting a loved one here.
