My mother,92 and cognizant, has been @ Habana since 2016. I rarely have complaints b/c I have things dealt with immediately b/c I visit 2-3 times a week @different times. I also bring in goodies for the staff. That may sound like bribery, but it really is b/c I understand they are not paid well, it’s not an easy job, and deserve recognition. I have had issues like a cna not showing compassion. I immediately calmly spoke with her and explained my mother’s need. It changed her behavior. I found her clothes were not taken good care of, so I do her laundry myself. I have no problem having things fixed by maintenance when needed. In other words, patience, understanding and kindness go a long way to making my mother’s time there relatively good. I will say that during COVID things have not been great and I can’t get in to make things right, but when my mother tells me something, I call and it’s remedied. I’m sure their jobs became even more difficult and frightening with COVID. Lastly, I will say that the night staff is less diligent in my view. Why? I don’t know. There job is just a bit less hectic. Oh, and the food is probably the least likable aspect. It really needs to improve.

the nurse call baton is usually on the floor and not reachable.. there is an attitude problem in some of the CNA personal.. the food is a problem ,very poor quality.,

I went to Habana after a 3-week stay in the hospital. They were the only place my insurance would cover (that should have been my first clue as to what I was in for). My second day there, they woke me up at midnight and said I needed a blood transfusion and took me to a nearby hospital by ambulance. The hospital ran its own tests and said that a transfusion wasn't necessary. I was later billed $401.40 for the ambulance ride and Habana refused to pay. The facility is dirty and run-down, the staff is uncaring and rude (with 2 exceptions), and the food is lousy. If you love your family member, DO NOT send them to Habana!!!

Please, if there are ANY other options, PLEASE take your loved one elsewhere. The place is absolutely filthy. My bedrails were so dirty, I could not grab them to pull myself up in the bed. I'll chalk the food up to my taste buds. Maybe one of my meds made food taste bad. For the most part, the food looked good! Although 2 mornings they just forgot to bring breakfast. I only found out breakfast had been served, cause they came for dirty dishes. I was there for wound care. Supposed to be changed 2x a day. Once a day was all they could manage, and one day it went for 26 hours before changed. I have no idea what some of the meds they gave me were, cause I don't speak spanish. I got sick one night in trash. They never changed trash and never brought something for stomach. Apparently the call nurse lights didn't always work, cause sometimes they came, and sometimes it took 2 hours. When I decided to leave, I signed out, but got no assistance in carrying bags. I walked out myself with 2 carry-on size bags. I left by Uber! I feel sooo bad for the residents, who can only afford these type of places.

This place has poor maintenance and a medical staff that refuses to follow doctor's orders. I did not have a call button or a TV or a working light over my bed when I was first admitted to this nursing home for rehab. The medical staff failed to keep me on a heart healthy diet, failed to keep me on water restrictions, insisted I drink more water, and failed to increase my lasix when my weight rapidly increased. The nursing staff also refused to increase my advir and postponed doing an ultra sound until the day before I was to be discharged. I ended having to be admitted into St Joseph's hospital within days of being discharged from Habana Health Care Center for edema and cellulitis because I had retained too much water during my stay at Habana Health Care Center. This place needs better medical staff to go along with their good rehab staff!

If I could give this place negative 10 stars I would! First off if you love and value your family member DO NOT put them in this hell hole! Where to start well first off right from the get go they put my grandmother in a bed that had another person vomit on it! When my grandmother pointed it out the cna told her she’d “get to it” well a whole entire day went by with my mother calling the nurse telling her the bed had another persons vomit on it to which the RN replied “I don’t know anything about it”. Next on the list the ENTIRE time she was there she did not have a working phone my mother and her complained over and over to no avail. Everyday we’d call and the phone would ring and ring and ring half the time they either wouldn’t answer or they’d say they were “too busy” to bring her to the phone. “Too busy” funny for a place that left their patients in dirty, soiled diapers and then when my grandmother would tell the cna that the woman in the room needed to be changed she was told “mind your own business” and then would shut the door. They did not shower her for an entire WEEK what kind of disgusting people would do that! My mother brought clothes by and voiced her concerns to the nurse who repeatedly rolled her yes and gave an attitude! Let’s not forget my grandmother is a dialysis patient who requires a special diet they knew this upon taking her. On the days they would actually pack her a lunch they’d send her with a peanut butter Sandwich and a soda!!! Both no no’s for a dialysis patient! They never had or salt blockers that she needed for dialysis either! When my mother complained to the supervisor she laughed on the phone. I’m so glad that you think someone following a dialysis diet a must-for her well being and health, is funny! Her eggs were always cold and if she’s ask to have them heated up she’s be told to “deal with it”. Would one of you eat cold eggs? The cnas would never answer the call button and My grandmother would have to scream for help to which an attitude ridden aid would eventually respond. If you want to visit tour loved one you can make an appointment and see them for 15 minutes outside! The people at this establishment have taken full advantage of the current pandemic and the family not being able to be hands on right now. This place is a sorry excuse for a healthcare facility and every single person should be ashamed of themselves! Shame on you for treating the elderly like this! This place and their sorry excuse for staff are garage! I work in healthcare and it is my JOB to uphold the highest of standards not one staff member does so here! Run as far away from this place as you possibly can as I will never ever put another loved one or my enemy for that matter in this place!

While a resident of this facility, my mother was given a shower with cold water by a CNA. This caused a terrible cold for which she had to be treated. While there were many loving and caring CNA's, who treated my mother extremely well, this incident is unacceptable and I will be filing a formal complaint.

This is by far the worst nursing home I've ever been to! Staff members are unprofessional, patients are just sitting around looking lifeless with no kind of entertainment. So many violations were seen. Parking is ridiculous! I can go on and on. Receptionist is suppose to be the light of the company yeah right. Lazy and unprofessional. None of their staff really speaks English. Just a horrible place. You want to really know the inner workings of a nursing home? Schedule a tour between 4pm and 7pm. You'll have all the information you need to run for your life!

Simply put, horrible! My uncle is in Habana Health Care. There are 4 residents that share one bathroom. There is tepid water at best and both knobs have to be turned to get any type of water pressure. The toilet itself was nasty. Dried excrement around the toilet seat and I could not tell you the last time, the bathroom was cleaned. My uncle did not receive his pain medication as he should have, with the nurses saying that yes, they gave him the pill, when in fact they did not even after I had been sitting there the whole time during the time the pill was supposedly given. When I initially called, the front desk put me on hold and then when finally came back to the phone stated that my uncle was not in the facility, when he was! As you walk in the lobby, there are residents by the elevator asking people for cigarettes and the smell of cigarette smoke is overwhelming. There was one nurse to 16 people on the day I was there. The rooms are small with no chairs for people to sit on when visiting. You either stand or have to sit on the patient's bed. Meal delivery is another story with my uncle receiving half of what he requested to eat. I seriously overheard patients talking also about buying medications that were not their own prescription. This is just the tip of the iceberg. If the patient does not have family or friends checking on them, the patient is royally screwed!

This is one of worst nursing homes, I have come across having worked as a nurse in a nursing home and also in a busy medical hospital. They do not answer their phones I have an Aunt in this place. I live in England and like to speak to my aunt who's all alone since her husband passing , they answer the phone then put you on hold. I am ring now been holding for the last 30 minutes. The Saff are so rude

My mother has been here for weeks after coming out of a sugar coma. This facility is not properly cleaned. She shares a toilet with 3 other people and it is not properly disinfected after use. But for a few CNAs, most staff are unprofessional. I have found a pill in her bed and inconsistency in her care. They doctors and staff tell you (the family) that they will provide a service and unless you are there every step of the way, these things do not happen. They are understaffed and ill prepared.

Do not take your loved ones here. My grandmother is in the hospital hanging on by a thread because of their blatant neglect. WE had to call 911 to help her. Unacceptable!
