I am pleased to leave a review for Lexington where I recently stayed. The room was spacious and totally clean. The food was good and nutritionally healthy. They offered a menu selection daily. The serving sizes were adequate and they offered snacks during the day. Most importantly, the staff were excellent and attentive. I was checked on several times a day and a nurse was always available when needed. The call button was answered within five minutes no matter what time of day or night. I will request to stay at Lexington next time I require a rehab and gladly recommend it to anyone I know.

My 99 year old Mother was transferred here from the hospital for rehab . Room is very clean . She is required to have puréed food, which looked perfect . PT started right away . All of the team leaders came to the room on day one to introduce themselves. ALL of the staff has been very friendly !

Very upset and disappointed. Few years back I had a wonderful experience so I tried again . I was suppose to go to rapid rehab but wound up on a unit. I was treated poorly. I have not had Covid vaccine due to my history with blood clots. The nurse, I’m assuming infection control nurse, told me I was getting one whether I liked it or not. Luckily I was transferred to hospital with blood clot in leg before this could happen. Wonder what would have happened if I stayed. Place has really gone downhill and so disappointed in staff attitude

Please DO NOT SEND YOUR LOVED ONE HERE!!! My mother was sent here for Physical therapy following hip replacement surgery. The nursing staff is very nasty. They gave my mother all of her medications at once, even if it was supposed to be twice a day. They didn't give her the thyroid medication for a week. The nurses yell at you from the hallway. Nurses never answer your questions as they are already walking down the hall. They had my mother so heavily medicated she didn't know what day it was and she couldn't get out of bed. She developed gout.- her feet turned purple- yet were rarely looked at. Physical Therapy took a few days to get started. Then it was 15 minutes and they never knew when they would be back. The food is inedible. Always served cold. The nurses do not answer the call button. They left my mother to lie in her own filth for hours!! The only saving grace was a CNA named Simona. Sadly, she wasn't there every day. On the last day the nasty nurse was screaming at my mother. Telling her she couldn't leave. She had been discharged and transportation was arranged. She threw flowers into my mother's lap- splashing water all over. She made my 85 year old mother push her own wheelchair to the exit. I can not say enough negative things about this place. There is no compassion. No caring. I could go on and on. I tried to have her moved to another facility. They told them that my Mom was ready to go home- Not true. She had to stay there for another 2 weeks. Even the doctor said that they only send patients here when all other rehabs are full. Please do not send your loved one here! All of the negative comments are true.

I was mistreated from the day I checked in. I went for rehab for my fall, arrived on a Friday afternoon. Never saw a doctor who handling my medication. I was receiving more medication then my private doctor prescribe with any update of what they where for or why. I was on private insurance and should have a private room, I was told they “ they don’t do that here” which I know is not truthful. I was never given any form of transportation to get out of bed. I was what I was called trapped in my bed. We were given raw food that was not fit to be eat. I was unable to use the restroom I would call for a bedpan and it would take 45 minutes. I had soiled my self several times one nurse came in and “ hell no” and just left, after about a 1/2 hour someone came in to help me. My sheets were never changed after all this. I would ask for some water and was told “we are out of cups”. We were given lunch at 11:30 and dinner did not come till 7:30 and mention above it was raw and cold. There website states there are snacks, activities during the day. I was never given any snacks and no one even mentioned any activities to me. After 4 days and still no rehab or doctors visits. I ask for extra blankets as it was cold in there and again was told they don’t have any. At one point I was given a dirty bed sheet. The staff is very rude and unhelpful. I never any good experience here and had to check myself out as I was not “allowed” to talk to anyone in authority with my concerns. I would not even let my dog stay at this place. They should be shut down. I am still having night mares about my time in there and still have nit-mares of my time in there. You would be better off at home then going in there. I am sure if I had stayed there I would not be as far along as I am now,

The staff here are lazy loud been waiting 3days for my meds. They just ignore you.i came here for PT so I could get around my house. Well I've passed!! And they can't tell me when I will be released from this hell house. They treat the older residents like shit yelling at the and rough with them crying in pain This place is filthy they now have a case of covid. I have severe COPD I can't be around any of that!!

My dad is in Lexington Health and Rehabilitation Center. It's a little older and somewhat updated. The rooms were a little small with 2 beds in each room. It's just kind of hard to move around. The staff is pretty good but the thing that I do notice is his clothes keep turning up missing and that irritates me because I got his name written on his pants and his shirts so I just don't know why they can't figure that out. They give him to therapy to try to get him to walk better. I would say their therapy department might need a little work. My personal opinion, from what I'm seeing is, and it's probably all of them, I think the people that have these jobs are just kind of tired of the same old situation every day. It's easy for us to sit out on the outside and say we wish they would just be a little bit happier about going to work. I think what I'm seeing is they just want a paycheck and they do the least amount that they could do. I don't know all the ins and outs or what everyone does though. They're starting to use the dining areas a little bit more but COVID took all that away too. That was in-room eating only but in the last couple of weeks, I've seen them eating in the cafeteria. I don't know how they do all that. There are some great people there but then there are some people that are just there for a paycheck.

My mother spent 25 days at Lexington to rehab from a fractured T12 vertebrae. The only positive thing I can say is that the PT and OT staff were very caring and concerned about my mother’s rehab and one of the nurses, Nancy, was a huge help. The rest of the staff didn’t want to be bothered. However, the thing I want to make known is that an employee of the facility met with my 91 year old mother (who has Alzheimer’s) on the 2nd day after she arrived at the facility and told my mother it would be to her financial advantage to disenroll from her current insurance and proceeded to have my mother sign paperwork to accomplish this, all done without my knowledge. And on the day this was done, my mother was hallucinating and mostly incoherent. I am my mother’s POA and her Health Care Surrogate and Lexington was advised of this the night my mother arrived by ambulance and was admitted to Lexington. My mother doesn’t have the mental capability to understand what they asked her to do nor to sign a legal document.

My Dad is in rehab for hip surgery and can't walk, so he is at the mercy of the staff. He had an accident in the bed and called for assistance and the nursing staff left him all night to sleep in his own filth and didn't change him and the bed, till morning. They took his hospital gown and didn't give him another one and just threw a blanket over him with nothing on his lower half and a dirty T shirt on his top half. This is not care or dignity. Please don't send your loved one to this hell hole. They are under staffed and the staff they do have are unhelpful and have attitude. My Dad got a quesadilla for lunch and it was just a cold tortilla with a teaspoon of shredded unmelted cheese in it, not enough for a child and awful. His food tray fell over because the wheels are messed up on it and he called for help, someone came in, saw it on the floor and said someone would come and pick it up and walked out. She could have taken 2 seconds to pick it up. The staff is unresponsive and passes the buck. Awful place. It breaks my heart that my Dad is trapped there for another week.

I was a patient at this facility for about a month. The room was kept spotless and comfortable. The aids were helpful I would especially give two aids , Ms Simone and Ms S Guy, as outstanding in their care of me.They both made me feel special I will always think of them. Should the occasion arise that I would ever need care I will give the ambulance driver the way to you.

This is an excellent nursing home. I've had two family members as residents in this facility. They are extremely clean and professional. They also keep in touch with the family which is very important.

Excellent rehab at Lexington! The concierges was an unexpected service, and they have a phone line to call. There is also outpatient rehab if you want to continue therapy with the same therapist at home. There is also transport available which is convenient.

I visited a friend at the Lexington and was tremendously impressed with how clean and well appointed the facility is. From the rooms, to the outside sitting gardens, to the music room to the workout facility, the whole facility is marvelous. Staff was friendly and only a step away when I asked a question. The place is beautiful. The rooms are large and well planned. I can't find one thing I would change about the Lexington.

My aunt was at Lexington Health & Rehabilitation Center for about six months. The people were nice, but the place is not as clean. They had a lot of people to take care of and it was a nursing home type of environment. The food was good and they had Bingo.

Wow please run fast this place is a dump donot put your loved ones here very unprofessional the facility is not clean and the food is awful my mom didn't eat for days showers disgusting and my mom was basically left to take care of herself thank God we moved her after being giving the run around for 3 days on the relocation.. please donot go here my mom is so happy now she was depressed

Unprofessional rude young staff. Admissions department was a horrible process. CNAs are angry and miserable. Parking is poor. Not fully staffed. Food is my mom's number 2 complaint.

My mother would have qualified for Lexington, but since she's gone down so fast, we decided to forget it. It's assisted living, nursing home, and rehab center. The rooms were nice. My aunt was there in rehab, so we went there and to see since it was close to the house. The rehab looked like a four-star, the assisted living is also a four, and then the nursing home was about a three. It was clean, but there were a lot of patients and from what I saw, the nurses couldn't give what I wanted my mom to have. She needs somebody 24/7. My mom is legally blind and can't eat by herself, so she has to be fed and that's what we were looking for. More like someone that has only about five patients and she could go from two or three rooms between them. That's not what they have, and I understand that.

We got our grandmother out of Lexington Health and Rehab. We took her out early and took her home. She wasn't fond of the place at all. She didn't like the way the nurses treated her. They put her in a room with someone who has dementia who would try to climb her bed. She felt like she was thrown in with crazy people. At that time, she had a spine injury and couldn't really move or help herself. And the nurses weren't attentive enough, and were kind of talking down to her. They weren't really treating her as if she was competent. They weren't really compassionate. Although the physical therapists seemed okay. But I wasn't really impressed with a couple of the nursing staff. We found her an apartment in Connecticut and she's moving up close to me so I can help take care of her.

The facility was clean,organized,and clear of safety hazards.The facilities staff was always with in protocal and regulations.This facility was always providing activities and treatment for the patients.I have witnessed goals being established towards patient for better physical health.The only thing real negative thing that I seen was that the patients mental treatment or psychological therapy was not taken into acount in most cases.The facility didnt seem to to take a patients mental state into concideration nor did I witness any theraputic treatments or groups tahat would help the patient more psychologicaly.Te facility was good on training me as a volunteer and letting me know what my responsabilities were and were'nt.It was good a good volunteer experiance.This facility helped me to better understand people and patient care and also helped me to realize what I wanted to do with my future.
