My mother-in-law is now at Countryside Rehab & Healthcare Center. I would recommend them. She has been there for three weeks now. I chose it because it was close to my home, and the doctor that saw my mother-in-law in the hospital, that's one of the places where he practices. The overall experience has been very nice. It's like they're a family. It is very clean. The dining hall is always very cheery looking with either flowers on the table or something. It was decorated for the 4th of July. The staff are very friendly, very attentive, and very professional.

The communication was nonexistent. A few workers spoke English as a first language. When you questioned something the employees became defensive. He is diabetic but was only monitored occasionally (his last blood sugar was over 300 and the did nothing). Employees, even RNs, were unresponsive to requests for assistance. He is diabetic and was given orange juice and regular jelly at breakfast. When I questioned this I was told this was his carbohydrates! I told the staff, as did Nick, the head of rehab, he kept leaning forward and would fall out of a chair. They put him in a chair and he fell out. I was told it was not their job to watch him. Overall, the food was disgusting, the room was nasty, the ‘skilled nursing’ he received was inadequate at best, and the nurses were rude and condescending. This is a substandard facility that needs to be closed.

I would not recommend the facility. There isn't enough staff to the patients in their care. Press the call button and it's a minimum of 30-45 minutes before someone shows up. I witnessed a patient on the floor calling for help - no one came until I went to the room screaming for help. Leave messages for a return call to get updates on the care - no response. Most caregivers speak only Spanish and hard to communicate what is needed. Food is terrible - had to bring food in most days. My mother developed pneumonia while in facility - my mother and I had to keep insisting on help with her breathing.

Cna on duty delayed my medication by not telling nurse I was requesting medication. After calling several numbers nurse showed up with medication and said she was busy. I called the main number and requested a supervisor and was told there was no supervisor was on duty.

My Mom was transferred to Countryside rehab from Mease hospital since we didn't have many choices under the Humana plan, after 10 days she was getting worse not better and when I inquired to get her back to the hospital the assigned Dr. Levine said she was getting all the care there that the hospital would give, by midnight that night they were having her transferred to Mease ER where she passed the next morning. We will always regret not getting her out of there and will have to live with that. There was 38 patients to one RN so her vitals were not monitored as should have been, the staff was inattentive to my Mom and disrespectful to the family. DO NOT SEND YOUR FAMILY MEMBER THERE!

Right now, my father is in rehab at Countryside Rehab & Healthcare Center because he had an accident. The facility is very clean, and they're very diligent with the medications. Anything that you have, they would explain it to you. They've been good, so far. The staff is very respectful and very professional. The food is great and he likes it. The building is big and spacious, and it looks new. Maybe they should just get more personnel and more CNAs.

Countryside took wonderful care of my grandfather during his stint in rehab. Admissions was smooth, communication was great, discharge was efficient and therapy was outstanding. He even liked the food. Thank you for all that you did.

I will recommend Countryside Rehab center for your loved ones CNA staff is absolutely wonderful , caring. The nursing staff is well experience , caring. The place is very clean smells nice. They will wash ,dry delivered and hang your Love ones laundry. The food is very good. The Rehab department is well experienced and very caring. Great maintenance staff in house . A lot of wonderful activities for your loved ones . Everyone in this facility works very hard to take care of your loved ones 24/7 . If you're looking for a place for your loved ones to be well tooken care of Countryside Rehab is the place.

The people at Countryside Rehab & Healthcare Center were nice and the food was good. They worked hard at helping my husband. They weren't allowing him to go to the bathroom by himself. The place was lovely, nice, and clean. The staff was very pleasant.

My dad has been here for a few months. The place is clean, people are nice, my Dad is always clean, fed and cared for. The nurses keep my and my mom up-to-date on his condition. He gets physical therapy and speech therapy. I've been to numerous nursing homes and this one felt homey and it didn't smell. It has lots of windows to let in natural light. There is a courtyard that the residents can go outside and get fresh air. My dad's only complaint is the food. Due to swallowing issues, he is on a soft and ground food diet. No place is perfect. Mom and I take turns going every day. This way we can keep track of him.

My Mother was referred to Countryside Healthcare by another facility. In two weeks she went from good health other than recovery from minor hip surgery and Dementia, to critical condition and had to be rushed to the hospital unresponsive. Her litany of issues included Pneumonia in both lungs, fluid around her heart, severe dehydration, malnutrition, low blood pressure, low heart rate, low temperature and partial renal failure. She ultimately died from complications due to her total lack of monitoring and care by Countryside Healthcare. I find it deplorable that this facility continues to operate. The staff spends their time texting and Facebooking while they collect money for patients they ignore. DO NOT SEND ANYONE YOU CARE ABOUT TO THIS FACILITY.

I think Countryside is one of the worst facilities on the planet. My husband almost died there; all they did was drink Ativan and sit on the corner. Everybody is duped on drugs. Their CNAs walk around on the telephone, texting and playing games. When you question about your loved one, it is horrible. I wouldn’t tend my dogs there. They must take good care of the residents.

My mom was in a hospital and needed rehabilitation and they suggested that she went to Country Side. They basically forced us to go there. This place should be shut down. I got her out in three days. The staff is a nightmare. They are unprofessional and the whole place was low class and horrible. It looked like a crazy house. The rooms were old and run down. I wouldn't send my dog there. They have no activities.

I got the a chance to tour the facility when I was looking for a place to stay. The facility was nice but I decided that I need to be around with people more. I've been to couple of facilities, apartment, and retirement communities because I was not really sure where to go but I decided to chose another place.
