My room was nice, it was kept clean, I made my own bed (out of habit). I had plenty of room for my belongings, my laundry was done by staff. My interactions with the staff were positive, I made many friends among the staff. The service was good, my needs were met, My therapy was well taken care of. The food was good and suited my tastes. I did not avail myself of te activities, I kept myself engaged. All in all, I had a satisfactory experience!

My husband is in Woodland Grove Health and Rehabilitation. I would give it an absolute zero. I would not recommend this place to anyone. The staff are noncommittal to taking care of their patients. The only thing they're worried about is following orders. His room is shared and they do not keep it clean. The food is horrible. The therapies are wonderful though. They have activities, but he doesn't take part. If you go in on the weekends and you walk down the hallway, it stinks. The common areas have lawn furniture, and it looked like it has not been painted in ten years. It looks like the place needs to be mowed all the time. It's awful. The place is rundown. It's not kept up. People are paying good money for this and they're getting ripped off. When we took the tour through there, they wouldn't let us go into a room. We could just walk the hallways, and now I know why. The faucets in the bathroom look like they've been there for 40 years with corrosion around them. It's just terrible.

This facility should not be rated or advertised as a physical therapy option. My mom is 81 years old, but in great shape mentally and physically. She lives completely independent. She went to this facility to receive physical therapy to resume her life. She was neglected, left in diapers for hours and left unattended in her wheelchair for hours. An abrasion she received in the accident was neglected to the point that she had to be taken by ambulance to the emergency room and treated for cellulitis with intravenous antibiotics. She was talked to like she was a child. When she questioned the medication the pharmacy tech tried to give her, she was told to just take it. Fortunately I was there to follow the tech out to the hallway to go over the medications, and they were NOT my mother's. I guess there was no system of checks and balances to make sure the patient was getting the right medications. When the doctors finally came to look at her arm, they reprimanded her for letting it get so infected, even though she asked numerous times for someone to look at it because it was hurting worse than her fractured bones. Nobody should be treated with disrespect or lack of dignity just because they are old. I realize the the facility has more patients suffering from dementia, and maybe the staff is better equipped to handle those cases, but for someone who has all her faculties and suffered an accident, this place was terrifying, demeaning and inhumane. If your family has to be cared for in a facility, please do plenty of research and spend as much time in the facility with your loved one as you can to make sure they are cared for. My mother would have died from negligence, wrong meds or depression and broken spirit if it weren't for us being there at the facility to watch over her care.

I would never put anyone you love in this facility. The rooms reak of urine. Air conditioning ducts are filthy. Bathrooms look like something from a “flip flop motel”. My husband says the food is awful. I have to make an appointment to visit him and am only allowed 30 minutes per day. My husband is in there for rehab not for a nursing home. No activities for residents. The sheets on the bed are so thin, you are sleeping on plastic. This place is horrible!

My loved one is staying at Woodland Grove. He called me last night complaining a lot. The place seemed fine, but I'm concerned about the service and what they are or are not doing. The staff seemed friendly. The administrator was really efficient in getting us in. The place seemed clean and didnt have a smell when I went in; it was not super clean but it was adequate.

Mother is at Southpoint Terrace, and her care is going fine. The staff is wonderful; they’re doing their job. They have a lot of things in there for the whole month of different days. They play bingo, they do different kinds of crafts -- like she made a pumpkin -- and they have music and church services. It’s a very clean facility. They keep the floor shining and buffed. When they clean the rooms, they take all of the furniture out, clean the rooms, and put the furniture back in there. Her beautician would see how clean it is. She has a beautician that would do her hair for me. They have a beauty salon on site, but my mother is very particular; she won’t let them touch her hair -- no one but her old beautician who's been doing her hair for years. She has no complaints, and she loved the meals there. My sister told me how she enjoyed her dinner yesterday; she ate all her food, she got a good appetite, plus she has gained weight since she’s been there.

DO NOT USE THIS FACILITY - If your relative needs to go here for short term rehab be advised that the "estimated time of discharge" will be the exact 20 days that the Medicare might cover. You will be told that the patient fell into a "plateau" state (meaning he/she will not get better) and because of that Medicare will not cover anymore. The food is HORRIBLE. The staff could be friendly buy they DO NOT help the patients if they cannot eat. I saw a poor lady just sitting there staring at her food, nobody came to help her and after a few minutes the nurse came to pick up the tray (she did not eat at all). The same day came to visit my mother and she was sitting on her wheelchair all wet (she peed on herself) and NOBODY came to change her. Plus, if you complain they get upset. Do not take any relative that you love here.

I would not recommend this facility. My father spent 6 weeks here in Aug-Sep, 2013. Although the Therapy team (PT, OT, ST) are excellent and the CNA's have great bedside manner, the quality of administration and overall medical care of this nursing facility are horrible. On several occasions the attending physician ordered vital tests and outside treatment which were not carried out by the nursing staff until AFTER the family complained. At least two such incidents involved performing tests to determine the cause and extent of potentially life-threatening conditions. We were forced to bypass the facility medical staff and have my father admitted to ER in a local hospital when Southpoint staff failed to follow through on doctor requests for additional testing when my father had been tested positive for internal bleeding....something he almost died from two weeks prior to being admitted to Southpoint Terrace...they were aware of this. Once admitted to ER we learned my father had a huge blood clot in his left leg that required surgery. Each time the nurses failed to perform tests/procedures requested by the physician the Assistant Director of Nursing made up excuses. On one occasion the attending physician ordered an examination by an orthopedic surgeon...two weeks later nothing had happened and we had to make arrangements on our own for the exam. The administrative staff is new and does not appear to have the proper skills and/or training for their important job thus putting patient care at risk. I interviewed several CNA's and nurses who said they planned to look for another employer due to the poor management of the medical treatments of the patients. Before choosing this facility ask to interview the nursing staff and their supervisor about their procedures and controls that ensure doctor-ordered treatments/tests are carried out. If you listen closely you'll find they don't have proper monitoring controls in place to ensure such things are carried out.
