My brother had a severe arthritis attack and spent a month here.Nice,incredibly clean room with nice furniture.He needed extensive physical therapy,and received world class care from the therapists.We have been through this before,and I know mediocre from great.In less than thirty days he went from can not stand to can and able to come home.Every staff we encountered -rehab,social services,nursing,dietition,case manager,cleaning,office staff,doctors and nurses were fully engaged and immediately addressed any questions or concerns.I talked with a lot of happy residents every day as well.Highly recommend as best choice in Jacksonville.

A dear friend and companion was placed at this facility after being released from the hospital for rehab post stroke care. He was only there 10 days in that time he developed a UTI with sepsis, pneumonia, pulmonary embolism and a subdural stroke from a fall the denied happened. He now in ICU . This is an absolutely horrible facility if you care about your family do not send them there it’s dirty, nursing care is nonexistent.

I was a resident of Harts Harbor, and while most of the staff were okay, the front office staff left a lot to be desired. Even though I repeatedly told them that there was no way I would be staying there once I got my settlement from my auto injury case, the front office kept on pressuring me to sign the paperwork that would allow them to take the lion’s share of my SSDI. They also put me in with a roommate who was clearly nuts, and one of the agency nurses also tried to have me Baker Acted for having a meltdown after being kept up by my roommate who was trying to crowd me out. It was one of the regular staff nurses who finally got my troublesome roommate moved back to her old room, and I got another roommate who was less of a pain to deal with. I can only say good things about the physical and occupational therapists. They kept me working, and they even made it fun. That being said, I wouldn’t want anyone to send their relatives there full time,because other than the PT and OT staff the other staff were not so caring.

ABSOLUTELY , do not go to this facility, They're just there for the money....don't care about you personally, the nurses are a joke.... the food sucks, the place have bugs in the bathroom, they treat you like Dirt !!!!!!

Everyone I've come into contact with so far has been very welcoming. When it mattered most the staff went above and beyond. I just can't thank you guys enough. Those caring for my mother are top notch. Thank you guys for all you've done to help my family navigate all the barriers, questions, and concerns that we came with.

My mother is at Harts Harbor Health Care Center. It was wonderful because they're really taking the COVID situation serious. We're able to see her, but they're maintaining the six feet apart, and just making sure that she stays healthy, and they're taking care of her well. The staff is kind and any information that I needed they were able to give it to me. They're pretty much business like, but as far as anything personal, they're on point. They're not letting anyone in the facility right now due to the COVID situation, so when we met with my mom they had us all outside on a patio. Everything was nice and neat. The patio was well manicured.

On the evening of Friday, 4/14/2017 I had been waiting for pain meds since 7:30pm. I rang the bell twice, at 7:30pm and again at 8:00pm. At 8:30pm I went out to the nurses station. "She's on break", they told me when I asked for my nurse. I waited and about fifteen minutes later she showed up. I asked her, "didn't anyone tell you I've been ringing the bell, I need pain meds?". She said, "I don't know, maybe." Then she said, "we have three nurses, any of them could have given you the meds". I said, "who, none of them offered to help me?". She began questioning me as to why I did not come out to seek her sooner and I told her my feet were hurting badly and it was very difficult to leave my bed. I am very dissatisfied with this experience and with Harts Harbor in general. This is only the latest in a series of many unpleasant events that have occurred to me since I was admitted several weeks ago. They have accumulated to such a point where I can be silent no longer. From aides who never inform nurses of requests for help (they shut off the red light and leave), to rude and insensitive aides (and others) who show no respect for closed doors or curtains, to the small portions of barely palatable food (usually with at least one item missing), to incorrect medications given to me quite regularly (once a completely different patient's meds, might that have given me a stroke or worse?), to aides who call me "baby, sweetheart", etc. despite my repeated requests for them to stop, to the never-ending loud aides and nurses who show no concern at entering my room at wee hours, putting the light on without permission and leaving (sometimes without closing the door), to stolen laundry (by aides, not residents, it does not matter that my name was written in marker), to non-availability of needed medications and medical supplies. Shall I continue with more examples? I understand the difficulties in this industry as a whole. However many of the things I have seen here and been subjected to have no excuse whatsoever. I am doing everything possible to transfer to another facility. If the balance of my time here remains as awful as it has been until now I will be taking steps to inform appropriate oversight government agencies and the media as well. I consider myself a civil and agreeable person but I have now reached my limits with the mis-treatment I have received here.

The patients diapers are not changed for hours. Items are stolen frequently. Staff gives lip service no results. You tell laundry not to wash cloths family will launder items. Cloths end up in laundry and lost. All items of clothing had been marked. Food is below average. When a complaint is registered things do not improve but get worse.
