Old dirty smelly urine ,dirty bathroom ,One sheet on the bed rolled up in a ball!the staff is all in one corner talking about themselves to each other not caring about the patients or interacting with them, just sitting in a room or wandering around with no shoes on no socks and sometimes no clothes on. This place is filthy !they don't care about the patients. Enter at your own risk, danger

As a past resident I know first hand , place is filthy with bugs . CNA’s are very rude , food is terrible. I wouldn’t send my worst enemy there . Stay clear of this place ! BEWARE

I would not recommend this facility to my worst enemy. The rooms are dirty, furniture in the bedroom was nasty, & I found roaches in the bathroom. The staff is unaccountable in answering questions,.. Ex: Did he eat well today?, Seen by the Dr.? The answer was always, "I just came on". Activities for Memory care do not exist! Coffee served half hour before meals., qualifies for an activity. Dr. visits are rare. Nurses receive treatment advice over the phone.

Tom went in rehab was good. CNA mean , Nursing only 2 were professionals. He came home with raw bed sores, can’t even bend and walk! I’m not done with this place!

The level of responsiblity for the patients things is zero. My husband upon leaving this place was missing his dentures; pillow; blanket; 3 pr. pants; 5 shirts and 5 hats. After numerous phone calls we did get back his pants and shirts and 2 hats. No dentures!!! We now have to have new ones made - at our expense. In the memory care unit there are no activities during the day and the patients choice is to look at tv, sit in the hall or sit in their rooms. The enclosed patio has a lot of mildew on the floor and walls - needs to be power washed. He never had his sheets and blankets on his bed. I found his shirt in the dayroom and another time his glasses in the dayroom. I would never recommend this place to anyone.

My mother was at Daytona Beach Health And Rehabilitation Center. It was very good. The people were very kind and caring. She had a private room and it was not large. The food was fine. It was a very nice and very caring community. They were understaffed and need to hire more people, but I understand that in that industry, there's not a lot of money. It's an older place.

I am a patient's family member and nurses always seem to have an attitude. If you ask a question about the patient's health for some reason they take it as an insult. If the patient has any health problems I do NOT recommend this facility since the nurses seem to not have a lot of knowledge.

The care my loved one has received at thus facility has been exceptional. He has received the most compassiinate, high quality care ever. The facility is exceptionally clean. Wonderful atmispgere. Patient centered, personaluzed care given. Loving staff. Definitely a Godly place to be. Good food and wonderful activities.

My husband is in Daytona Beach Health and Rehabilitation Center. We choose it because it was clean. It smells good and the people are nice and helpful. My husband is in a private room with a bed, bathroom and furniture. They have very good food. They have a lot of activities, but my husband didn't participate there. What I like best about them is that I am confident that my husband is taken good care of. I would recommend this facility to others.

My father was at this facility for Dementia, to start the staff was not trained to handle a dementia patient whom had violent outburst instead of them learning to handle him they ignored him. Upon visiting this facility some days you would smell urine and feces and some days not, the CNA/LPN were note helpful but the RN would explain more one it comes to the medicine being issued to my father. The human resource staff and the psychologist whom was responsible for him "tried medication" on the hope it would work but nothing definite at which point I later learned the medicine they were trying was not supposed to be given to a dementia patient. At one point he had switch medicine like three times within a month which left him in a very lethargy state which the nurses were fine with. The setting of the center is in a very quiet and peaceful area, very sunny area and the front was keep clean and orderly another negative is couple times visit the staff had music playing that was more of an aggressive nature and the TV playing with shows that were loud and aggressive also in nature, I felt this was bad for the patients because it could keep them on edge.

This facility is nasty and needs to be shut down in my opinion.It has mold and is disgusting.The management of this facility is horrible they discriminate against certain races in this facility. The food is normally nasty I have been to the facility multiple times and they would serve a dish that is suppose to be warm freezing cold. As for patient care call bells will go off almost all night I am not saying all the nurses are bad or other employees they need to be under investigation for neglect in my opinion. They allow anyone to be there as long as they want no matter who it is. they have people allowing people to go out and get drunk and to come back into there facility and cause problems with other patients that are wanting to sleep or relax. I have seen nurses tie down patients to keep them from moving around I reported it but nothing was done. do no recommend anyone for this facility no one should endure the stuff i have seen here.
