Do not recommend the facility in Middleburg FL. No special dietary menus. Diabetics fed what everyone else’s was. Excuse was not enough $, which is baloney! Facility lost brand new pants, 2 pair & 1 dress. Mom got to wear them less than once before they disappeared. We sent her by UPS 2 more new pants & 2 dresses. The package arrived & was signed for by the front desk on Friday the 9th, Mom was released on the 15th & she never received her package! Night assistants were nasty to patients & refused to answer their calls. Leaving patients on the loo up to an hour!

My mom moved into Brandon Health And Rehabilitation Center in September. I must say that this facility is satisfactory. She can get the help that she needs on a full-time basis. The staff seems to be pretty OK, but they are shorthanded, so sometimes they can't get to you when you need them to. But I'm satisfied with it. Her room is semi-private and it looks OK.

Parent sent there from hospital for rehab. Have found parent left soiled more than once, call lights lighting up hallway and no staff insight. The care provided is disgraceful. The patient’s are Neglected!

This facility is not worthy of one star. Unfortunately that is the lowest rating you can give. My father was a patient there in July 2021. It is a miracle that he survived! Luckily he was only there four days. During that time he received no pain meds, water, food, therapy, or personal care. He looked like he aged about 10 years in those 4 days. I am sure Medicare was billed for all kinds of medication and services. It was worse than prison. If this place serviced animals and treated them how they treat the human patients who are trapped there people would be protesting outside the building on a daily basis! Luckily we were in a situation where we could take him home and have my sister who works in the medical field could take vacation time and care for him for several weeks. However it was not an easy process to get him released. It took about 6-8 hours. No one in the administration was helpful or knowledgeable. Brandon Health and Rehab should be shut down. I don’t think anyone would ever choose to send someone here. Unfortunately, when someone is released from the hospital like my Dad was you are told this is the only facility with a bed. If this facility serviced animals It would have been shut down long ago ! Sad that animals have more protection and rights than sick or injured senior citizens.

Do not put anyone you care about in this facility. Many reviews tell about what atrocities happened to their loved ones that came here for care, I’m speaking from my own experience of being a prisoner here. I only needed to build a little strength in my legs after a week in the hospital. It has been 3 weeks, 3 weeks of hell and counting. I’m in better shape now and have been for 2 weeks than before I went to the hospital, but because my insurance keeps paying, they won’t release me. They finally held me in this dangerous place long enough that I now have Covid. I am a diabetic, I worked hard to get my A1C to a healthy 5.8 since I been here my glucose is seldom below a dangerous level of 300 and usually close to or over 400, they refuse to give me the proper amount of insulin, I can barely see out of my right eye because of high glucose levels. Personal hygiene; none, there is no access to towels, washcloths, wash pans, soap or anything other than a wall mounted soap dispenser and brown paper towels, the type found in gas station restrooms. We are scheduled 2 showers a week, but 1 of my days is on the weekend and there is never enough staff for a shower, in fact on weekends the patients are ignored altogether or treated rudely. As I mentioned, I’m diabetic, yet all meals are nothing but starch and carbohydrates, very little protein and usually with a sugary desert and 6 or 7 packets of artificial sweetener, 1 packet of salt and 1 pepper. Because I now have Covid, the day nurse wouldn’t bring my morning meds, fortunately the night nurse was still here and I was able to take my meds. There is a lot more to disclose but you get the idea, stay away from this place

Inept staff from the administration down. Believe the bad reviews. I saw them but hoped for a better outcome. Rooms were filthy. Molding falling off walls, curtains with blood (?) or some form of fluid on them. Took them 2 weeks to return my dad's clothes from being washed. Administration cannot answer any questions. Save yourself and your loved one a headache, choose elsewhere!!

Believe all the negative reviews. The room was disgusting and dirty. The employees were rude and dismissive. The care my dad received was inhumane. He went there after hip surgery for short term rehabilitation. He was put in diapers with a post surgical catheter and left in soiled diapers for extended periods of time. The PT did not start when it was supposed to. In the first week he developed a sacral pressure ulcer and an additional one on the left heel. It was given a grade of 3, 4 is the worst. He was diabetic and the blood sugar had never been so high, consistently. They did not feed him a diabetic friendly diet and seemed to have a difficult time keeping him hydrated. When he would tell them he couldn't eat what they brought, they either told him the kitchen was closed or brought him a sandwich and threw it on the tray. We witnessed this on several occasions. They told him his room buzzer was broken and would not answer the call button, which we found later to be a lie. They would close the door and ignore his cries for help with the pain. We were there every single day for 3 1/2 weeks trying to make sure he was taken care of. The worst was the nights and weekends. I expressed my concerns to the Social Worker, Director of Nursing and anyone who would listen. On the day my dad was taken via ambulance to the hospital, he was incoherent, shaking uncontrollably and lethargic. It took us almost an hour to get the ems there. The nurse on duty at the time kept on insisting nothing was wrong. My Dad was Septic. He had 2 infections, was dehydrated, electrolytes were non existent, blood sugar through the roof and had septic encephalopathy. After this all took place, my Dad was in the hospital 7 more times with so many infections and never came off the catheter, which was supposed to be removed the first week in this facility. My Dad passed on January 2nd because his organs were so damaged from all the infections he endured. He was treated horribly at this facility, so bad that we were questioned by the hospital about elder abuse.

Please DO NOT put your family member in this place its terrible. My loved one was in there for a little over a week was getting depressed. They were not taking care of. I HAD TO FIGHT TO GET THEM TO BATH HER. She wasn't getting the Physical Therapy as I was told. Communication is horrible. The day receptionist is beyond rude and disrespectful. Little worms crawling all through out the hallways. Don't leave your loved ones in move them if possible. Its heart breaking. Lack of communication the list goes on. Do not put your loved one in this place. If it was possible to give negative stars I would.

This was the worst facility I have ever seen. I visited my Uncle. This place was in complete chaos. No one knew what was going on. I waited over an hour to see him. I had to keep asking where he was. The front desk person yells into a loud speaker to bring patients out, but no one listens. The food is the worst thing I have ever seen. Patients are neglected. I watched one patient sit in the Florida summer heat in their "court yard" waiting for someone to come bring her back into a room for over an hour. I saw a man with a wrapped leg that had fluids leaking and it had been there so long it had turned black. The staff is rude. I witnessed an exchange with a staff member and a family that was very upsetting.

Listen to the negative reviews. My mother left Brandon Regional able to sit up & walk 15 feet & left this facility bed-ridden after 14 days later. I was told when she entered this facility, they had a 14 day quarantine policy to follow CDC guidelines (even if patients are Covid negative). My mother has Stage 6 Dementia - know your rights because they told me there were no exceptions & no visitors - there are - Compassion Care Visits - Look it up. Day 1 & 2 I got updates, after that, I had to call if I wanted information and was given basic - she's okay. Day 6, I got a nurse on the phone that informed me my mother was refusing food/drink, resisting care, it was taking 2 people to change her, the only thing she had eaten the day before was 1/2 a pudding. Keep in mind she isn't aware she takes meds, they are supposed to be given to her in pudding, crushed. I had to drive there, on a Sunday, argue with the receptionist to be put on speaker phone with the same nurse (because no one else was supposedly available) , who finally sent the Unit Manager to the front to speak to me. The Unit Mgr. had no knowledge of any of the issues, said the notes weren't updated, told me my mother had been downgraded to pureed foods days before, with no one making me aware. I was still told I could not see my mom, NO EXCEPTIONS. Day 7 - After 3 hours of trying to speak with an Administrator, I went in armed with CDC Guidelines, CMS Compassion Care information, Gov. orders on Facility Openings and demanded to see anyone that was in charge. I spoke with the Risk Mng. I explained I was not leaving without seeing my mother - It was her right and mine. Under the Compassionate Care guidelines, a patient does not have to be dying, they can be having a hard time transitioning, refusing foods, etc. My mother met almost every need on that list. They allotted me 30 minutes. It took them 10 minutes to clean her while I stood outside her door. She was severely dehydrated, weak, over-medicated and had terrible leg pain. Did I mention outside her room was a leaky air vent dripping onto the floor into a puddle, with no caution sign & with patient walking around. I filed complaints with every government department I could. I visited her every day after that, with no time limit. I had to tell them to change her meds because she was over-medicated, I had to tell them to check her leg pain and when they did, they discovered a blood clot - the day before they planned to give her the J & J Covid Shot. I had to tell them they tested her eating capacity and down-graded her to pureed foods because they tested her over-medicated and half-asleep. I had to keep telling them to crush her pills. I had to tell them to bring her water and straws, because she wouldn't have any, couldn't reach the cups if she did or the table would be 4 foot away from her. Again, she's now bed-ridden. Would you have had to do this in a good facility? No. If your loved one cannot speak for themselves, DO NOT SEND THEM HERE. I could go on & on, but do I really need to? My mother is safe at home now, under her Primary Care Dr. I didn't even trust them to set up her at home care. No need for a facility response, there's nothing you can do to make up for the lack of care, service and compassion you promise.

DO not send anyone that needs care to this facility. My husband was here for 5 days before I was able to get him out of there. He waited up to 4 hours for anyone to come and assist him on any of the days he was in this facility. They also did not do the rehab as suggested, they gave him the rehab to do on his own in his room. Find another place if you love the person who needs help. This is not the place!

This facility is in desperate need of rehab in itself due to the lack of empathy for its patients and there family members. My mother was discharged at noon and by 2pm on her way back to the ER via ambulance due to a bleeding pressure skin ulcer. No treatment listed on her discharged medication list. How sad and furious this makes me feel. No body should be treated like this nobody period.

The staff on the phone are absolutely horrible! Lacie and Moana are so unprofessional, I really worry about my Mothers care while she is at the facility. During a Pandemic we are limited to visits and rely on the staff to communicate with our loved ones. But, these individuals are absolutely horrible and should be fired by speaking to individuals on the phone they way these two do. Including putting me on speaker phone and mocking me because I have a speech impede. Which is super Tacky ! I would hope unit manager Lacie and Moana are reprimanded for their actions and how the speak to family members. This place doesn't even deserve 1 star !

My Mother has been a patient at Oakfield Health & Rehab Center in Brandon for 2-months. She is constantly made to feel ashamed & embrassed that she soils her adult diaper and told to wet herself or go #2 more than once in her diaper before they will change her. She lays in a dirty diaper for an hour & half before anyone will change her. We've reported a particular nurses ad who seems to be the culprit of the abuse & no one does anything about it or even gets back to our family about our conerns:(

You can ring the bell at Brandon Health and Rehabilitation Center, and it’s 30 to 40 minutes or maybe an hour before somebody comes and sees you. If mom needed something fixed or adjusted or her pillow fluffed, they’ll say they’ll be back, but it might be 3 hours later before they can come back. Their rehab facility is excellent, and the people there are great. They really care about their patients. I would definitely take her back to Brandon Health and Rehab again if she needed complete rehab because I do love their people there for the rehab part. Even a lot of the nurses were good, and their staff was good. However, 50% of their CNAs were lousy. They could care less about the patient. The other 50% cared. I told the CNAs that if they didn’t like their jobs, that they needed to go find another place of employment because that was what they were there for was to help the patients. They don’t change their bed linens every so often. My mom was there for 20 days but had only 2 baths. I kept telling the CNAs that she needed to be bathed and her hair needed to be washed. Her last bath was the day before we left, and a nurse ended up having to give it to her because the CNAs were passing the buck. They would sponge her sometimes in the bed, but that, to me, is not a bath. She has a shower right there in her room that they can just roll her right into. It’s not like they had to lift her or pick her up. She has a catheter and she needs to be kept clean down below. I was very disappointed in the CNAs. Their office staff was very good and very kind, but after her 20th day there, they made a big booboo. First, they told us that she was completely covered by her insurance, and now they tell us that we went 2 or 3 days too quick. She’s not covered, and we owe them $3000 and some dollars. That’s poor financial accounting ability there.
