My mother moved into Boca Raton Rehabilitation Center six weeks ago. The care is very good. There are numerous CNAs and nurses around the clock and a great patient-to-caretaker ratio. She is properly cared for, and the physical therapy program has been excellent. The nurses, the staff, and the doctor had been very responsive, and her care has greatly improved. She's in a shared room which has been wonderful for her. There are a lot of activities by the Activities Director, who comes in regularly, and all the different dates, like Cinco de Mayo, he had a little party and brought around some mocktails and some chips. They have activities that the residents can attend. And then there's outdoor time and conversation talk three times a day. The physical therapy program is fabulous. They're working on returning her back to being mobile with physical and occupational therapy. The staff is great. As far as the physical appearance of the facility, it could use some fresh paint and landscape, but we're there for the care.

Staff very helpful and friendly. Therapy team was very helpful. Especially Rick and Jennifer. Meyha was also very kind. thanks to Scottie for his help in my stay,and treatment.

A friend referred this place to my dad and I, she said they had the best rehab and I must say they had my dad walking again after a bad fall , they have a nice gym for the residents and the staff is great very helpful!!!

I had my mom there and they didn't give great care to het and neglected her. She died. Please don't put your mom or dad on this place please

This facility is the worst I have every seen. On their website they state that no residents have covid but what they are not telling you is its because they ship them out to the hosp asap. My husband was tested before leaving the hosp, once he got to rehab he was tested again (all negative) and a week later they called to say his nurse got covid but i shouldnt worry because she wore two masks. They refused to do a test for another week because protocol dictates testing every two weeks. once he finally did get tested he was positive and they shipped him out so quick to the hosp (to keep their numbers down) your head would spin. Two weeks later he was dead. Oh and when they called me they said they were sending him to the hosp for a uti and oh by the way he is covid + now. when i said im not surprised you never put a mask on him when you wheeled him to the lobby, the nurse told me "these things happen". They wouldnt have if they followed protocols and put masks on the patients. He would have been better off at the dog pound and would have gotten better care. The receptionist and most of the staff have accents so heavy you cant even understand when they say the word hello. Ifyou want your loved one to live do not send them to this despicable facility. I would have given them a negative 19 stars if it was possible.

DO NOT TAKE YOUR RELATIVES HERE! My father was transported here back in March, every 2-3 he was going to the hospital ( only during night shift) We get good reports but it was hard to reach anyone. Jacob or the Shift nurse! We was able to find a Nurse Director that would answer. Dont get me wrong the day staff some are great! My father lost so much weight, they call us to put him in hospice and we told them no! He was approve for longterm care but we was told they have no facility for that. He never received his personal clothing that were taken there! Later in the months we get another call around 130am he is not breathing well going to Hospital only to find out he was being OVERFED and aspirated! He later passed away! The night nurses are lazy, don't pay attention to patients, keep them on meds to sleep while they ignore the phone sleep and do everything else.

Only one RN per 40 rooms and up to 110 beds it takes several minutes before anyone acknowledges a help light is on Most staff are somewhat knowledgeable but some do not verify correct medicine doseage. They give you a calendar of events but it is up to the patient to get to any. Any therapy even though Doctor is ordered are followed through on. 2 or three times a nurse brought me the correct doseage of my nighttiime meds, Example I am prescribed 150mg of seroquel and a nurse only brought me 50mg she then stated she thought the pill wa s150mg when each one waa only 50mg I wa only one of most patients whi actually knew what i was to take. they do not have a menu for diabetic patients

so far this is fine--I do not know about the food but the people in the business office have been on top of things and return my calls--my cousin is cared for and the staff is for the most part doing a descent job caring for her--although I am not there my cousin goes there looks after her and reports that is fine--

My wife is in Boca Raton Rehabilitation Center right now getting good care. I’m satisfied. The have a very good staff. Her room isn't luxurious, but it is acceptable. It's a clean, medium-sized facility. My wife and I are not impressed with the meals. It’s like everything comes on a sandwich.

This facility was horrible. The building was clearly not designed for use as a rehab. Patient rooms have only bath tubs, so all must use a not-so=clean community shower - and no towels are allowed unless the patient is on the shower list that day. Furniture was broken, with drawer fronts dangling to the floor. The food was heavy on carbs. My diabetic friend was able to order a salad instead - BUT the same carbs came, because "we do not have any lettuce!" The room was cold, but the patient was denied the requested blanket because, "there aren't any." But, the physical therapist said, "they're lying" and delivered one. Promised, and needed because of recent anesthetic, help for constipation was never delivered for days! Medicine dosages were wrong. I had to purchase pillows for the patient to use, because the supplied one was so uncomfortable. No promise was ever kept. I never saw a cockroach, so I guess that's why they pass inspection..

I visited this facility

This place is disgusting! I visited a family friend and was appauled in the condition some of the patients were in and the type of people that worked there. It smells! Very unorganized.

The great staff makes up for the not so updated building! Everutime I go i am greeted with smiling faces from residents and staff! My friend just told me she feels at home here and wants to stay instead of transferring somewhere else. To me that speaks volumes!

Horrible place . they dont care about their patient my mother was there early November 2016 she buzzed the nurse for help and took 2 hours to help she was laying in urine . very unprofessional. Horrible. Place

Do not bring your love one there. Facility delapaded but we overlooked it because we assumed my mother in law would get superior care there because it is right next to Boca Community Hospital. Treatment of patients is exactly as bad as the place looks. My mother in laws uti got so bad while there she had to be re admitted to the hospital. AVOIDE THIS PLACE......

The day shift is great especially the physical therapist. The night shift 3pm to 11pm I am not sure if they are short handed or the people on the shift just do not care. I explained to the nursing staff that my husband gets confused at times and does not remember things. I had to call twice before someone finally would go change my husband after he pooped and also had to call twice and report that my husband was in pain. Last night my husband said a nurse walked in his room and turned around and walked out without helping him. Even the evening shift supervisor does not seem to be forcing people to do their job and take care of the patients. If this continues I am going to take him out and put him in another rehab, My husband is supposed to be in contact isolation and none of the nurses seem to be observing it.

My dad was at Boca Raton Rehab. The administrative people seemed clueless, and the supervisors didn’t seem to know what was going on. I wasn’t there the whole time, but I never saw any efforts for socialization. My dad had a stroke, and it was very difficult for him to talk. The day we moved him in there, there was a double room with a guy in the other bed laying in a pile of feces. You could smell it from down the hall.

I visited this facility

I have visited many nursing homes and this has to be the worst one I have ever experienced. The nursing staff is poorly trained and poorly managed, they did not have enough basic supplies like diapers, gauze etc. It takes about an hour for the call light to be answered and when they enter the room they come in with an attitude. The Director of Nursing does not address your concerns she seems overworked, lost and confused, everything I said she just kept on repeating it. On top of that they billing me for services I did not receive. Oh and make sure you check your medications thoroughly because several times i caught nurses dispensing the wrong pills. It is not safe, I would not recommend it at all.

DO NOT GO TO THIS PLACE!!!!! If you love your loved one don't DARE!!! I had my mother there August 6 2015 we arrived at 6 pm we had no care for 6 hours! No meds, water or food. After begging someone to help me that my mom had bleeding blisters from the tape and needed pain meds from her surgery 5 days before, I couldn't get anyone. They told me they nurse went for an break for and was gone for over an. The place was total chaos .Nurse were yelling to each other from drown the hall asking each other for things they needed. There were my psychiatric patients then rehab! There were men laying in there beds with No cloths on, a lady sleeping on the floor. Awful I TELL YIU!!! My moms file couldn't be found. No clue! The place was disquesting ! I tied to be clam and trying to understand but I knew this was going no where I called 911 and they came and we took my mom back to the hospital where she had her surgery. PLEASE DONT TAKE ANYONE TO THIS PLACE!! I will be filling a formal complaint

My grandmother suffered not a great place for your love ones the nurses don't care for the patients no passion it's sad when other nurse's tell you don't send her back because some of their worst patients come from there my grandma called me in so much pain crying I called the nurse station by the attitude I received from the nurse let's me no that they are not doing their job if u love your family don't send them here

My mother stayed there for three weeks . I loved the therapy staff. They were all excellent, truly caring. I especially liked Nick, Menesha, and Mary. But if your parent is a fall risk, has dementia, or is very weak I wouldn't recommend this place. My mother fell because of the lack of safety measures exercised by the nursing staff on duty the day she fell. There are a few nurses and CNA's that truly care but unfortunately there are ones that are just there to make a paycheck. So proceed with caution if your parent is uncommunicative, weak, frail, or a fall risk . You would be better off somewhere else.

Boca Raton Rehabilitation Center was the right choice for my mother. We chose this place is because of it's location and everybody has been very pleasant to us. The staff is fantastic. My mom has been there since the middle of April. Every room they have is immaculately clean. Also their level of care is pretty good in that there's not a real long wait for when they ring the help button. The food is pretty good too and they serve it with a nice presentation so it looks pleasant.
