My mother likes The Oaks at Avon. They're very personable. She has made friends with all the nurses and CNAs on the different shifts. They're very nice. The facility is very clean. Everything's kept up and they'll fix something when you mention that it's not working. They have bingo, a lot of church services, and people singing. She receives occupational therapy and physical therapy. She enjoys being there.

I believed I had chosen the best facility for my Dad's needed physical therapy & nursing care after his surgery & being in the hospital for a month...boy was I wrong! The bad situation was I could not visit my Dad due to the covid which is understood however I did not expect the lack of responsibility of patient care my Dad received while a patient at the Oaks. They lost his clothes, they did not give him baths, in fact he went for 2 weeks without having a bath until I raised the roof about his care. That was the whole problem no one really took care of my Dad like he should have been cared for in all ways, the food was terrible according to my Dad, I even told them what foods he could eat which apparently fell on deaf ears & in the process my Dad lost over 22 lbs. I got my Dad the heck out of there was not a good place for him nor anyone else if you have loved ones. I am still trying to figure out how they are rated Four Star????

The Oaks at Avon has good workers but the administration needs a lot of work. There are not enough people to work so they're understaffed. My mom is doing rehab there and it's good. Food is basic and bland. They need to improve staffing and a change of administration as they know nothing and misguided me on every step of the way as far as what to do.

My husband was recently discharged from Oaks At Avon. The nursing staff was overall very nice. The aid Nathen and Nurse Jenny are the best and always helped when needed. Therapy is great at getting you going. Food is ok, laundry services could use some work as they lose some clothes. Overall I'd recommend this place.

The Oaks At Avon is an older facility. The cleanliness and the overall staff were fine. The only problem was I had a miscommunication with the social worker. I had a problem trying to get some information from her and I was not pleased with how that situation went. The food was OK. I know they have some games and other activities. They need to improve their communication with the relatives of their patients.
