For an off-and-on total of eight weeks, my husband was a patient of skilled nursing here. Each time, he visibly flourished within the first 24 hours. The physical therapy department is stellar. He was up and moving within hours of arrival. Physical Therapist Margaret Stone is fabulous. We're lucky to have found her and her talented colleagues. The nursing staff is warm, kind and capable, with some employees having worked here for 24 years and 18 years and other long-lived tenures. Dot, the singing nurse, has a voice that once graced the Rutgers choir. Sharon, a CNA who helps encourage dog visits, is always cheerful. And then there's the kitchen staff who produce really good meals for residents -- and their guests. The dining room has two grand pianos and dark furniture and feels like an elegant restaurant. The patient rooms are capacious, with huge windows and open (literally) to views of elk and deer and the occasional bear. A young buck laid down in the grass outside my husband's room one day, about ten feet from his bed, and stared at us as we stared at him. A kiss from God -- as is this wonderful haven of healing.

My mom ended up in rehab and she was at Life Care Center of Evergreen. She had a PICC line in, she had a UTI, and they were having her on antibiotics. They did the PICC line, they got her well, and they gave her antibiotics on time, so the medical care was just fine. My mom was diabetic, and on one Sunday morning they gave her pancake syrup and orange juice. I made a point of calling up there several times during different eating periods just to ask what she had. I'd asked the staff what she had, and then I also talked to her afterward, and they paid no attention whatsoever to the fact that she was a diabetic. Many times they fed her a meal that was very high in carbs with almost no proteins. My mom didn't like the food. I had one bite of a turkey-something once, and it tasted OK, but salty. They weren't following any kind of a dietitian-type plan, even though they claimed they had one. The staff-to-patient ratio was much higher. The nurse was on staff and sitting in a room that had 20 rooms around her that she was supposed to be watching, and that was one person on the weekend. She could call someone if she had a question, but one person for 40 people is insufficient, especially if my mom is a two-person lift. Overall, I wouldn't take my mom back to the Life Care of Evergreen ever. The facility itself was not immaculate but pretty clean, and very old but remodeled. However, I wouldn't recommend anybody go there for any reason. My mom was in isolation all the time. They brought her in and they had her on COVID isolation, so 14 days she never saw anybody except the nurse that was on duty.

I am a relative of their patient in for rehabilitation. The patient was at a hospial for treatment following a stroke. I visited her daily at the hospittal. This was an uplifting experience for her since she was ellated to see me on each visit. Background: the patient is a 91 year old person who previously was very active in good weather to work in her beloved garden and she expressed her desire to return home to continue working in her garden. This nursing facility refused to allow me to visit her because of policy. i expressed my opinion that her emotional and mental health may be in danger with her expected visit from me. To no avail, they did not seem to care. Policy is policy we cannot deviate from it. . At this facility. policy is more important than patient health. I can only hope that she will prevail inspite of their POLICY!

My mother-in-law was at Life Care Center of Evergreen, and they were wonderful. She was there for quite a while after her fall, and they made sure everything healed properly. Its very nice and doesnt need updating. They had physical therapy there for the seniors.

We liked the environment at Life Care Center of Evergreen. The rooms were very nice. It had the size that I needed. It was very clean. The people were very nice. They had created an environment that I appreciated greatly, and I appreciated the conversation that we had there. They had substantial activities everyday. They were doing things outside also.

My mother is presently at Life Care Center. I visited in March of 2016 and my sister just returned from a visit this August. The CNA's are very caring and generous with their help. The dining room staff have gone out of their way to provide vegetarian meal for Mom and all of her family when we visit. They accommodate us even to allowing us to bring frozen entree's to keep in the kitchen cooler. The PT people have helped Mom regain strength and mobility after her July heart attack. I cannot say enough good things about her care.!

Visited this facility and happened to see a couple of mice. The residents were sitting around looking bored and unhappy. It seemed to need a good cleaning. Be advised the staff are not concerned about the residents and are there for a pay check only.

My mother was in Life Care Center of Evergreen for about a month or six weeks. It was very good. Everybody was very attentive, very good, and very caring. The room was bigger than with other single rooms. They had exercises, ice cream socials, and puzzles. My mother was there for rehabilitation. They had doctors right there, which was nice.

I've been at Life Care Center of Evergreen. This is the place where I took my dog and visited the people there. They had a nice physical therapy room. The entrance was not large, but it was nice and there were some residents that were sitting out in the reception area. I saw some residents and they seemed relatively happy. It's a pretty setting. It has a good-size dining room and it was attractively done. I saw a single and a double room and they were of good size. The staff seemed to be very good with their patients and they were willing to give the residents some freedom.

I loved everything about Life Care Center, and I should have stayed there. They have computer and art classes. The staff was very good, very efficient, and very caring.

I have always had great experiences when visiting the Life Care Center. By working with our church they help elders that are Catholic still receive communion. When I have been there everyone seems happy and comforted. The employees are happy, comforting and informative. It is not one of the nicer facilities I have seen which is disappointing. I think that there must not be money going into the upkeep of it. It is good that those living there seem to be happy though. They always seem to have activities to keep them occupied and they make visiting very easy. It is in a beautiful location, Evergreen is gorgeous but it is secluded which is what makes it less accessible. It is a great place for someone who has family in the area or enjoys the great outdoors. The winters in this location is tough too, lots of snow! But for me that is not a bad thing. Overall its a nice place.

Even though I do not care for nursing facilities, this one is exceptional with their rehabilitation team. My mother broke her femur and has been at this facility for 3 weeks and has made tremendous gains. She hates being there, but they have done an extraordinary job with her getting her back to walking, going up and down stairs with the goal for her to go home.
