I went to rehab at Lincoln Meadows Care Center. My experience was very good. The staff members were very friendly and nice. For the most part, they were quite attentive. They had physical therapy there, which was very good. They had a hairdresser. The place was always very clean and neat. It's an older facility.

I toured this place and immediately was greeted with a friendly receptionist. The building smells clean and presents well. We booked my family with them and happy we did. Therapy team is the soul of the place and they did group sessions. It was a nice place for my family. They also encouraged visitors and answered all our questions. Thank you

the amount of food is a joke. Then at night you can't get any decent snacks. The only drinks at night is water or coffee because they have no access to a refrigerator. The snacks the offer is not for diabetics. The nurses and cna's are really nice but there's not enough. For breakfast this morning I got 3 bites of under cooked eggs & oatmeal. I'm a adult being fed like a child. Plus you only are allowed 2 showers every 7 days.

My father was at Lincoln Meadows for 4 days. He was admitted for rehab after a visit to the Kaiser ER. The 4 days he was there seemed like an eternity because the care and attention was so lacking. Being monitored, cared for, fed a puréed diet, protected from falling and receiving rehab were the reasons (we thought)that he was admitted to Lincoln Meadows. His first meal was incorrect non puréed food -had my mom not been there to intervene I am certain they would have left my dad to try to eat it himself even though he requires assistance and could aspirate without the puréed texture. The meal staff brought afterwards was so horrible looking & unrecognizable that my dad refused to eat it-and he’s not a picky man. We were told my dad fell the next day but given no further details. There is not ample staff to prevent people who are fall risks from falling. My dads room neighbor fell 4 times while my mom was there on different occasions and she had to leave the room and search for staff on each occasion. She heard staff tell roomie to “promise not to try to get out of bed again” even though he was a memory care patient. At times there is no staff monitoring the entry/exit door to the facility. My dad was not given his medications on time during his stay there. My dad’ health and mental health visibly deteriorated in the 4 days he was at Lincoln Meadows -after 2 days we started the process to get him out in order to relieve him and ourselves from this disaster. To add insult to injury, the discharge sheet from Lincoln Meadows indicated my dads health had improved during his stay. This was a outrageously false statement. Please do your research and your loved one a favor and choose another facility if at all possible.

The rooms have great views to a peaceful surrounding sounds of birds Plenty of space- marble counter with sink Large bathroom with willing staff to assist during the most humiliating of times. My 89 year old had a 3 hour incident and Chris handled with her with dignity Staff are absolutely fun and always there although their communication system is now Being upgraded that is needed to know importance of needs when called. Ryan the interim director is working hard to make improvements .theresa the activities director has many tricks up her sleeve to get guests involved . The nurse practitioner on staff solved 2 problems of mine . Natalie was so kind to my 89 year old surrogate to me. I watched and listened to all interactions. The food was delicious but I owned a health food restaurant and it is far different than McDonald so if that is what you are looking for, they feed the guests quality food. I did not realize for the first few days that there’s 10 items that can be brought to you from 7-7 Egg salad the best ever and I served them in my store Soup the best when I found out about this option I had soup almost every day. The staff are so professional even though a guest is acting out. I they ere patient which is more than I would be . When giving showers they are thorough and keep you warm with what seems like 20 towels. Soap was better than I thought it would be . The staff greeted Helen my roomie with enthusiasm every time especially in the morning . Sherry is delightful and listens to eac guest with a smile ,hug and a song . I cannot think of an amenity missing Last the occupational therapists and physical therapist were better than private trainers. If your loved needs to gain strength and you need a couple weeks off from the arduous job of caretaking this would be the place To get your loved one the care they need and for you too. A beautiful piece of property the community would be remiss if they did not get involved thank you for your time You too

PLEASE do not send your loved one to this facility. My father was at Lincoln Meadows so he could receive an IV antibiotic prior to his surgery scheduled on 1/22/18. He never made it to surgery. To say that the facility is understaffed is only a part of the problem. Much of the staff we came in contact with did not have the skills necessary to be dealing with sick patients. Even more egregious, my father's chart/medical records did not represent the true state of what was occurring at the facility. It was a blatant attempt on the part of the facility to exonerate itself of any wrongdoing. We entrusted Lincoln Meadows with the care of our father, and they failed him in every respect. It is such a tragedy because it should not have ended the way that it did. The night that he passed, I called the facility three times, pleading with them to do something. He was dehydrated and his body was shutting down. I demanded that I speak to him, and when they held the phone up to his ear, he was incoherent. The nurse tried to tell me that he was sleepy as I argued that that was not the case; he needed to be sent to the hospital. We got the call five hrs. later that he had passed away at the facility. To add insult to injury -- the disrespect shown to my father's body, after the fact, was appalling. The facility had at least 30-45 minutes to "clean him up" prior to our arrival. IMAGINE walking into my father's OPEN room to find him completely naked, gown covered in vomit and twisted to the side of his neck like a noose, with his face covered by a towel. That is an image I will have to live with for the rest of my life. Now, IMAGINE that once he was "presentable", you heard your mother scream because staff had forgotten to unplug his bed and his lifeless body began to move, making her think that he had come back to life. IMAGINE that you are the last person in the room with him, holding his hand, praying over his body, and waiting for a funeral home representative to appear. IMAGINE that instead of a funeral home representative, you feel the arm of a Lincoln Meadows staff member brush up against yours as she attempts to offer your deceased father his meal tray. IMAGINE that you have been living with this nightmare since 12/29/2017. IMAGINE having to live with the guilt and anger of having put him there in the first place. My goal in writing this is to save someone the heartache of what we've had to endure.

My mother is in Lincoln Meadows Care Center. She was in Kaiser first in ICU for ten days before they released her to the nursing home. Her stay at Lincoln has been OK. She's doing OK. It's somewhat clean, but there were things on the floor. They were folded up and you think they are clean, but then you go to unravel them and they're not, which isn't very sanitary. You have some staff members that are really good, and some that are not, and she has kind of a little bit of both. There's been some staff members that are absolutely great and some that don't care. When she has to go to the bathroom and she has to ring her bell, sometimes it's 2 1/2 hours before somebody comes back to take care of her. That causes her to have other issues if she's sitting in a diaper, like a UTI. It appears that they have enough staff, but it doesn't appear to me that they have enough direction to take care of the patients. The food is terrible as well.

I was in Lincoln Meadows after I was hospitalized, and my experience was good. It’s a really nice place and each room has a window with a gorgeous view of the nice garden. They have this nice area for rehab. The staff is very kind. There wasn't a single person that I dealt with there that wasn’t fully focused on my needs. They just delighted people.
