My sister is currently in the Plaza Healthcare. It seemed like a beautiful place and looks well-run. The doctors seem very good and very caring. So far, my sister has been getting really good care, and she's happy. The staff members are very good at communicating with my brother-in-law, and letting him know about my sister's condition and what she's facing. There are a number of doctors and therapists working with her, which is also really good, so there are a wide variety of people working with her. They have physical therapy, there's a really nice gym, and my sister is recently being allowed to go there in a wheelchair. They make her stand up, walk a few steps, and sit down again and again. Their gym seemed very well-equipped, and there were people working out at the gym, along with one-to-one nurses or trainers. They have a beautiful courtyard with trees, fountains, and lots of birds, which was really nice. They also had some cages with birds, and really nice fish tanks, and if the weather is nice, you can go and watch them. They also have a really pretty chapel there, they have a number of nice visiting rooms so that the patient and visitors can sit in a different room other than the hospital room. It's $25,000 a month, but she's not paying anything right now because of Medicare and her insurance.

My grandmother was transferred to Plaza Healthcare after her second surgery within two months from falls. She went for pain management and therapy. She was with Plaza for a month and a half. In the time that she stayed at Plaza they were able to get her back up and walking on her own with a walker and helped her manage her pain. The therapy team at Plaza is 5 stars. They encouraged her daily to do her very best so that she could get back home with my grandfather. The Nursing team and the CNA team on the first floor where she stayed was amazing. They treated her with respect and attended to her needs as they arrived. They treated her like family. She could not stop talking about how helpful and loving everyone was. She made some wonderful friends as well with the residents that stay there long term. She had a special connection with her Social Worker as well who was always available when my grandma needed some extra love and attention. Once she felt comfortable she started participating in their daily activities. The activities team at Plaza was wonderful. They made sure she had her daily newspaper to read and encouraged her to join in on fun activities that are planned throughout the day. She even planted tomatoes in their new garden. As for the food she did not have too big of an appetite when she got there but by the time she left she was eating well again. She said her favorite thing to eat at Plaza was their lemon bars. Overall the care, love and attention my grandmother received at Plaza was wonderful. I hope she does not have to go through another surgery or fall. But if she ever needed to go someplace to recoup I would not hesitate to request her to go to Plaza. Thank you to the staff of Plaza for all you did to help my grandma get better.

We transferred my mother who was on a ventilator here from another facility due to that facility renovating. Long story short..my mother's ventilator was hooked up incorrectly and she went into respiratory distress. Turns out this wrong hook up on her life sustaining machine blew both her lungs and permanently damaged a valve in her heart. Plaza healthcare took responsibility but that was about it. My mother was here for 7 days. She spent 5 days in the ICU unit at Scottsdale Memorial and died on July 4th 2014. Thank you Plaza Healthcare for killing my mother. We thought you were going to take good care of her, instead you pushed her over the edge. Incompetency and complacent employees and management here. Please think twice about putting your loved one here. The 3rd anniversary of my mother's death is approaching and I write this review in her honor and on behalf of my entire family

My mother was transferred from Acuity Specialty Care to Plaza Healthcare on 09/01/2015 because Acuity went bankrupt. The first week, there was no doctor to do a check up nor review her medical records. My mother immediately suffered bed sores and developed pneumonia and they never treated her properly and on 09/22/2015, my mother passed away. She was swollen head to toes. She had puss coming out of her ears. I am looking for an attorney to sue this facility. They are horrible. Don't take your loved ones to this place. These people are there only to make money.

My lifelong friend of 42 years was transferred to Plaza recently. I had been by his side for more than 30 days and I was very familiar with typical protocol relative to his care. My father is in the advanced stages of COPD so I have a fair amount of experience with respiratory issues. Ten minutes after he was admitted to Plaza he experienced a choking episode. He was panicking. I walked from his room to the nursing station as there was no Nurse button in the room yet. I conveyed the situation and sense of urgency and was told that the Respiratory Therapist was on the phone and they would be there when they were done with the phone conversation. I was amazed and very concerned by their response lack of empathy. I returned to the room and he was extremely distressed. I waited for an agonizing 10 minutes. I went to the Nursing station and was told that she was still on the phone. I let them know that it was serious and he was in distress. She said that "someone will be in". I returned to the room and waited while my friend was drowning in his own secretions. His condition, the look in his eyes, the sound of a loved one drowning and the inability to help was tearing me apart. Another 10 to 12 minutes passed and I returned to the nursing station and demanded that someone respond immediately. By the time they responded 25 minutes had passed. His condition required 4 liters of oxygen and he was transported with oxygen and cannula. Once disconnected from transport oxygen it took Plaza staff 45 minutes to provide oxygen and then it was only at my request. This was the first 45 minutes of what turned out to be a 4 day nightmare. The staff members were seemingly devoid of compassion and respect for their patients. There were a few exceptions. For the most part they were rough when nasal suctioning and it was evident as he bled frequently. They were rough when moving him in the bed. He said they act differently when I leave. At one point they dropped his oxygen mask on the floor and they were about to put it back on when I said "are you going to use the one that was just on the floor?" and only then did she get a new one. These were a few of what turned out to be the minor issues. His condition worsened while in their care. Over the course of the 3rd and 4th day he could not breathe and complained many times. When we brought it to their attention they said, "he's fine" after looking at the pulse oximeter. He was breathing much harder to get the same amount of oxygen saturation. They continued to suction and his condition continued to worsen. The afternoon of the 3rd day I asked the staff if they thought it was time to return him to the hospital and they said," the Doctor is aware of his condition." He was panicking and getting worse on the 4th day. When I arrived, he said that they disconnected his nurse button because he was pushing it frequently. In all fairness, I'm confident that he let them know just how frustrated he was with that nursing button. Another one of his friends phoned the nurses station on the afternoon of the 4th day. After hearing about his condition he asked the same question, "don't you think he should be transferred back to the hospital" and he received the same answer, "the Doctor is aware of it". Later that evening, he was in extreme respiratory distress and one of the nurses stated, "screw this, that's enough, call 911" . The ambulance transported him to the hospital "code 1", lights and sirens. The physician that treated told him upon arrival stated, "your lungs failed". The experience terrified him. It changed him as a human being. Mention the word Plaza around him and the expression on his face and his body language speaks volumes. He was transferred to a place where the staff is compassionate. A place where the staff pride themselves on the number of lives that they give back to their patients through aggressive rehabilitation, North Mountain.

I am from out-of-state and recently visited my sister at Plaza Healthcare on Granite Rd in Scottsdale. We were in and out at different times of the day over the course of 4 days, so we were able to observe care during different shifts. While we found individual staff members hard-working, the overall organization and management of the facility is deplorable. They simply do not have adequate staff to take care of the number of patients there. The day prior to our visit, my sister had not had a sponge bath in days, so she made a special request to be bathed. Asking again while we were there, she was told, "We'll have to see where you are on the schedule." That need was ignored the entire time we were there. In addition, call buttons go unanswered for unreasonable lengths of time. One evening when we arrived, we were informed that she was using the bed pan, so we waited in the visitor waiting area, checking back every 15 min. to see if she was ready for company. We waited for over an hour before she pushed her call button, which was actually answered in a timely manner. The whole while, though, another patient's call button alarm bell went unanswered, even with staff present at the nurse's station. Physical therapists are easily put off. If they arrive at a moment when my sister is experiencing nausea, they cancel the session for the entire day. This has occurred so often that she is in far worse shape now than when she left the hospital, where, with assistance, she was able to use the bedside commode. Plaza staff is content to put her on a bed pan, then walk away for up to an hour at a time. This, combined with the lax physical therapy routine, has left my sister weakened to the point that she can barely sit up on the side of her bed without experiencing dizziness. Walking and sitting in a chair, abilities she was able to perform on a limited basis at the hospital, are completely lost. The day of our last visit, the portable commode had been removed from her bedside, indicating they had decided not to even try to have her use it. When she was first admitted to Plaza, they failed to arrange proper transportation for her to make a scheduled appointment with her oncologist. Transportation eventually arrived an hour and a half after the appointment time. A formal complaint about the incident was addressed by administrators, and appropriate changes were made to improve the transportation service. Although, on the day of her next appointment, I observed a staff member complaining (with an annoyed eye roll) in the hallway as she left my sister's room, that my sister wanted to wear street clothing rather than a gaping hospital gown to the appointment. That request was eventually addressed and my sister was delivered to her appointment on time. She was accompanied by an extremely attentive and caring CNA escort, who would receive a full 5 stars, if I were to rate his performance. After the appointment we waited over an hour for transportation to retrieve her. Having been weeks since my sister had been in a seated position, the 4 hours she spent in a wheelchair that day drained her energy completely. The day we left Arizona, I received a text from my sister saying she was delivered to an outpatient appointment without a CNA escort! I contacted my niece, who told me the staffing administrator didn’t know about the appointment till the day she returned from a 3-day absence. They apparently asked my sister if she wanted to go alone. She had waited a couple of weeks to have a faulty chemo port replaced, so I assume she agreed to go without an escort. I don't know details, as having spent another block of hours away from the bed that she's become accustomed to living in, she lacked the energy to even lift her arms to answer the telephone when I called. I was also alarmed to learn that her call button was ignored twice yesterday, leaving her to soil herself both times. That is the last straw, prompting me to compose this review. If your insurance, allows other options than Plaza Healthcare in Scottsdale, take a serious look at them!

My mom was at Plaza Healthcare for three days only.If I could rate this zero stars I would, but there was one CNA who actually seemed to care about the people, so I am giving one star for him. When I went to visit Plaza, it was in the day time. I was impressed by the Director's information and it looked like a good place, but, it is not a good place. The evening hours are different, it's almost like the sun goes down and so does the staff. One CNA in particular needs to be terminated from her position! My mom told me she went into her room and said, "I just came in here to see if you were dead..." THEY DO NOT LET YOU STAY THE NIGHT FOR A REASON. The next morning my mom told me what she said and my stomach dropped! There was also another incident (in the THREE days)! She arrived Friday and Sat & Sun they do not do paper work, so I got her out asap! The staff is not sufficient nor courteous to the patients/residents. Look around and see there are MANY unhappy faces here, LOOK in the rooms and ask the family that is there what they think, we didn't get any good answers from them. We took our mom out and placed her in a better place. YOU HAVE RIGHTS and call your insurance company to give you a list of places they will pay for, and GO check them out. This place was a nightmare. I have to say when the director(s) hear from me, they tried to accommodate, but you can tell this was temporary, as they would go back to OLD HABITS. I hardly EVER write reviews, but I feel you need to know how uncaring a lot of the staff is here. There are some people who are okay, but just we just didn't feel safe leaving our mom in their care! Also, you have to make arrangements to have someone with your loved one all the time of possible, wherever, you go. Many people in this industry are not as caring as they should be, that is a fact! I hope this helps you out.

First day there for my wife. I was led to believe that there were TV's for each bed. Not so. And if you're in the bed nearest the door, the person in the inside bed has control...meaning if they draw their curtain, it cuts your bed off from being able to view the set. Rooms are small and cramped. Her room must be right at the "staging area" for everything that takes place at Station 2. There is so much noise and constant traffic past her room, that you can't get any rest. And they insist on keeping the door open. The first day my wife becomes ambulatory, we're out of here.

My mother-in-law is at Plaza Health Care. We chose this facility because my sister-in-law works there, and it would be easier for us. The food is actually good. They do have activities, like bingo, and they are pretty decent.

My husband has been at Plaza for 7 years and resides in a subacute area of the home. He is severely disabled and is on a ventilator. The care he receives is second to none. I am there every day for several hours and I can give testimony to the fact that Plaza has extended itself in order to give the care needed for my husband. It is one of a very few nursing homes that has a Respiratory Therapy unit who monitor his breathing. There is a wound nurse who attends to him when something unusual appears on his skin, there is an infectious disease doctor available if infection becomes a concern and there are many RN's who care for him on a daily basis. I have looked at the deficiency reports online and my only comment is they are not related to reality. If I was scoring Plaza I would give it 5 stars.

My aunt has to stay here because of a long term disability. She is one of the younger people there but that has been alright. I found Plaza to be very good. Plaza is expensive but an excellent value for those on Medicare/disability/or other services. To begin with, each patient has thier own room creating a more home like atmposphere. They have a very full activity schedule that keeps the patients busy. For bedbound paitents or patients who can't get out for every thing such as my Aunt, the activities ladies bring activities to them. In addition, staff and nurses really take time to visit with patients and get to know them. One retired staff member bonded with my Aunt and still keeps in touch with her through the phone and occasional visit, and this is not uncommon among other patients as well. The nurses have always been very attentive and friendly, and many of them have worked there for years. Everyone is always very accommodating when my family visits and we have been very happy with them.

Plaza is a large medical facility which emcompasses a convalescent home and an assisted living as well. It is a memory care assisted for dementia residents called Mimosa. The assisted living is in the front of the nursing home but is set-up toyally separate. It houses 50 residents and currently there is 30. They have large private and semi-private rooms. All handicapped equipped. The staff are all certified caregivers and certified med techs with previous healthcare background. There are 4 on days,3 pm and 3 on nights. They have an RN during days on staff who oversees this unit and comes in frequently and as needed. There is a nurse practitioner who visits weekly and some doctors who come in. They have a psychiatric group they work with. They have all meals cooked in the nursing home kitchen so they have an available dietician. They serve three meals and several snacks. The food has a variety diet but choices at meal times limited to that meal entree. The food is typical of a nursing home or hospital style. It is nutritious however. The residents eat in their own dining room. They do, however, have a choice to go to the 50 diner just behind the unit in the building. This is usually when family visits. Outside they have a beautiful courtyard with walking paths, shaded areas to sit, lush flowers and greenery. There areas for outside activities when it is cooler here. The staff take them for walks in the morning. They have an activities director and wide group of things to do. They have bingo, karaoke, exercises, current events, birthday parties and they put a day on where family is encouraged to come and they do a social or have outside entertainment. At times they may go out on the bus or with famiky but rare due to the residents mental condition. They have a beauty salon. The staff appears to be caring and residents taken care of. For someone that has a debilitating mental condition and cannot be at home or in a more independent setting it is a good facility.
