I'm a resident since January and I'm so happy here. I'm disabled due to severe rheumatoid arthritis and a stroke. Living alone got to be impossible. This place is very clean and stays that way. The food is great and portions are plenty. Here they have the most amazing staff. Janitors,House keeping, Maintenance,CNAs, RNs, Kitchen staff, Case Managers, Supervisors...it's all about the Patient's and Residents. They care and it shows. I've been to several in Phoenix and Life care of Glendale blow's the others right out of the water. The difference is Day and Night. I'm very grateful to be here.

Poor maintenance of facility! Dirty restrooms. Understaffed and not worth even one star. If you value your loved one, look further for care.

My grandmother says the ladies who work here are very good at their job, and also very compassionate. She is really grateful to be in their care. She was scared about her first day of physical therapy, but once she stood up she was surprised how well it went. My grandma's mental health has improved being here. So I am posting this as a thank you to all the hardworking people who care for each of the guests. For those concerned about family staying here, these are the things to know: You can visit anytime 24/7, press the doorbell button, and sign in. If you call the main number you might have difficulty getting transferred to a room, but the staff was really awesome and went to her room and called me back right away to be sure she could be on the phone. Each bed has a hanging closet, desk, TV and 6 drawers for belongings. Guests can have snacks and plants, and fluffy blankets, pictures etc. It's an older building so the rooms don't seem modern, but it's very comfortable for people who need this kind of help. I would rather have better care than some fancy vaneer anyway. So it feels like a much better place than I anticipated based on the other reviews. They have a rehab room, and other common areas that are more modern looking...if you care about that kind of thing.

The people who are supposed to take care of patients here were very neglectful. It was heartbreaking seeing and hearing how a family member of mine was treated. One woman working there was extremely rude, her name is Sylvia. She would get frustrated and yell at my family member, and as we were leaving this facility, she was seen sniggering (as if she was glad the patient was leaving, very disrespectful). The people who are supposed to give care to the patients seemed as though they really did not want to be there. For example, my family member was left in their diaper for many hours even after telling someone they needed to be changed. Overall, no one seemed to be doing their job, to clarify, very well if at all.This experience was terrible, if you truly love your family members, then do not send them here.

I was very disappointed from the time I entered the transport. Who send az transport knowing the patient just had surgery and the gurney couldn’t even raise the head and applies the belt over the injured part. Since I could not set up, I felt ever numb with aggravated my surgery. Once I got to the facility I could not believe that I had to share a bathroom with someone. That went way years ago. After washing up in the morning they didn’t have hampers to put the towels and there were two bags of toiletries that were used I the bathroom. By being in the medical field, I could not believe this is still going on and insurance companies are paying for this. I am use to hospital food, but this food was horrible. I was here for PT and I could not get an answer when I was going to PT. They finally came at 1:30 pm and just walked to PT room for 10 mins and walked back. There wasn’t any PT for the weekend, so why am I here. Financial people came in threatening me to pay my deductible not checking or knowing that my hospital bills took care of my deductible. Because of all the above, I decided to leave. I wouldn’t recommend this facility to anyone with or without insurance.

I want to say I was very glad I chose this facility! I was suffering with a fractured knee and unable to put any weight on the leg for weeks. I has a 2 week stay with the LifeCare Facility. I found the staff very kind, helpful, encouraging! I visited another facility prior to this and saw a huge difference in the manner of care! Please consider the LifeCare Facility before making your choice so you would not regret it!!!! I give them a 5-star rating!!! Very grateful for this facility!!!!! Jane T.

I would NEVER voluntarily go here or take a family member here again. This was the only room available that accepted our insurance. I speak only for the the third floor where my husband was rehabbing. The third floor is filthy and the bathrooms are disgustingly dirty. I gave my husband bed baths, rather than expose him to the shower there. I hated that he even had to use the toilet in his room. I would only use the first floor restroom. The entire third floor smells horrible. The third floor nurses are NOT good. Medication is not given on schedule. My husband was given medication at the wrong time and caused excessive vomiting. We did not experience friendliness or compassion by any of the personnel except one night nurse. The techs are very unfriendly. Patients sit in a wheel chair in the hallway on the third floor for hours. Some cry and many complain that they are cold. It was cold on the third floor. I wore my jacket while inside. We heard calls for help from patients all throughout the day. Calls are not answered. The physical therapy is a joke. The food is inedible. There was one aide in the dining room that was very friendly. The speech therapist was very good and very friendly. My husband felt like he was a prisoner. This place needs to be investigated. My husband was in no way ready to go home, but we came home anyway and obtained in home care. Do not take a family member here without a thorough investigation of the place. I am not a snob. My husband had just spent almost 6 months in 4 hospitals. We were eager to be in a skilled nursing/rehab. This just isn't anywhere we would ever recommend.

I just spent approximately 8 months there recovering from a number of health issues the staff was amazing unfortunately they appear to be made to work extremely long hours with limited staff which is unfortunate. I would have no problem referring a family member or friend to this facility

This place is not even worth one star! They have dropped to a 2 star facility with Medicare and soon after state comes in I am sure they will be a 1 Star or even closed down. My mother was here and they took horrible care of her! She was there 2 weeks and never saw anyone from Therapy! They only took my mother because she was a Medicare plan and Medicare pays more money, they knew they could not and would not care for my mother. They have a questionable relationship with Banner Thunderbird and my mother was sent before I even knew it! I was not able to visit this place or she would NOT have gone. They had 2 separate cases of BED BUGS while my mother was there, a outbreak on the 1st floor and 3rd floor! GROSS! I was told by the nursing staff that it was OK to have patients with CDiff & MRSA... SHARE ROOMS! I came in to visit my mother and timed how long it took for someone to answer her call light....it TOOK 39 mins! Even though my mother was not ready I still took her home and contacted social services of Arizona. If you really care about your loved ones DO NOT bring them here!

My mother was a patient here for hip surgery. The staff were amazing and we loved the therapy department. No they're not perfect but who is? We definitely will return!

My father was in this place, early 2016, passed away 3 weeks later. I know the reason, but wish not to say, but he was on the upswing after being released from a very good hospital, in great spirits. He was rushed to the hospital, after not recieving his meds, which caused his passing.

My elderly friend 90 years old is here. In the few hours of my visit I was asked to feed an AP patient and I have no experience. I'm just a visitor. When button is pressed for bathroom help they took 10 minutes to arrive and then left her alone in the bathroom and when she was done took more time for them to come back. When I leave the room to find someone, they are no where to be found. The rooms are not full so I am not sure where they were. If you have a loved one that needs assistance to go to restroom, beware. It is an accident waiting to happen to your loved one. If your loved one is AP, beware a visitor may be asked to feed them.

Life Care Center of North Glendale was great. I really liked it, but my dad did not like the apartment. He likes a gazebo or patio home. This was a fabulous facility. Everybody looked joyous.

My Dad was in the Life Care Center for rehab. They neglected to put an alarm on him knowing that he was a fall risk. It ended up that he did fall out of bed. They found him on the floor. They don't know how long he was laying there for! They transported him to the hospital. He then went to hospice where he passed away seven days later. I would look around for another facility.

Its maken me choose a star but i give this place no stars.. My mother was in this facility and she didn't get the care she needed.. In fact she had to ungo anther surgery... Plz don't send your love ones here.... Took them an hr just to give my mom roommate pain meds..

Tomorrow as I leave my 37 day stay has been phenomenal..the nursing staff is very knowledgeable and professional.. my medicines were given to me on time,all my questions were answered and I was made to feel very comfortable and safe.. CNA'S that i had here were so caring so helpful. anytime I turned my light on for assistance they were always here right away. physical therapy and occupational therapy were very professional very helpful and after having a stroke I do feel safe being able to go home and care for myself and as for my speech therapist Angela Wow she is amazing lady...kind, caring, educated in her field.. everyday she pushed me to strive and believe in myself and Esther my case manager, so proficien!! She made sure my discharge went smooth..I had everything I needed to go home such as wheelchair, walker, and any other medical supplies that I needed she took care of everything.. made all my appointments ... I didn't have a worry in the world very proficient.. And the administrator Andrea she would come to my room 1 - 2 times a week just to check on me see how I was doing, lovely lady..if you need a place for yourself or a family member to become stronger after an accident or an illness I recommend Life Care Center of North Glendale, It is awesome !!! I'm not where I need to be..but I thank God I'm not where I used to be. Melody Johnson

The nursing staff was very caring. The rooms were OK. They were small with two beds and two student desks. Life Care Center of North Glendale was pretty good; they just couldn't meet my mother's needs. She needed a memory ward, and they didn't have that. The only entertainment they had was when people came from outside to sing to them and they had a lot of people come. She was there almost two months. The food wasn't the best. The food would have to be a 2, and as far as activities, that would be a 1.

Life Care Center of North Glendale is fine and seems well organized. The staff has been very helpful. The rooms are very clean and well manage. They have different activities, like bingo, movies and trips, but my brother was not able to go on those because he is kind of wheelchair bound. He doesn't really participate in any of the activities. I have eaten a meal there and I think it is good. My brother is a picky eater and he doesn't really like the food, but I don't think it is against them. They have dining rooms on each floor and there are four floors. The first floor dining room is a little nicer and have a little more room. I like that it is close to where I live, only five minutes. It is close to a hospital, in case anything ever comes up. It is well managed, clean and smells good. I never go in there and smelled urine or anything objectionable. It seems like they know what they are doing. They are doing it for a long time and they do it well. They have different levels of care. They have people there just for a week or two, some people who are there permanently, and they have everything in between. However, they can improve on their meals.

This place is not a caring facility. I was a patient after a spinal fusion and lamenectomy for one day as my daughter a Nurse Practitioner took me out. It took 45 minutes after you push button for someone to come in to ask what you need, then another before they take you to restroom. It took a total of 5 hours before I could get pain meds as they had to order from a drug store! No bed rails as you have to ask to help when they should know what you need before the hospital discharges you to them. I would never recommend this place to anyone as I would not want them to go through just one day there.

Life Care Center wasn't very clean, and I was very dissatisfied with the physical rehab that my mom was receiving there. At this place where she needed skilled nursing, you push your button if you soil yourself and you lay there until they got there to take care of you. Maybe they can improve their staff to patient ratio, so that if she went to the bathroom, there would be somebody there to help. The woman who was in the bed in the same room as my mother had the same situation happen to her. She needed help and had been pushing the call button, but nobody came. And then, she started crying, so I left the room to go get a staff member. I had to walk around the facility to find somebody and to tell them that the old lady has a really bad problem, she's been crying and asking for help and somebody please go. Instead of that individual going, that individual got on the radio and called to see if there was somebody else available to go. I did not want my mom being treated that way, nor would I allow that other old lady to have to go through that. I went and sought out a staff member to get her help myself because I wouldn't want someone to do that for my mom. She was not able to get in and out of bed because she had broken her pelvis, and she did not receive the level of care or the level of physical rehab that she required. It was not at all what I would want for someone else's mom, let alone my own.

Does not use hip percauctions on hip patient the aids here are not qualified to be helping the elderly after hip surgery . After 2 serious surgeries this place needs to be reported !! DO NOT GO HERE PLEASE

I’ve been living in Lifecare for almost a year now. It’s easy to get around here. We have entertainment, people come in and sing and we have Bingo and outings. They have a bus to take us to scenic places, casinos and stores like Walmart. The food is not the best though but the staff is wonderful.

My grandmother has been at this facility for a few years now. We have had several issues, relating to privacy, care, and theft. It is hard to get private time with her and her roommates, opening the curtains and peeping in when we are with her. The nurses do not take care of her like they should. She goes several days without bathing, to the point of smelling terrible. She will be in the same gown and bedding for days, even though sheets and her gown are messed with food. While we have moved her a few times to help with the privacy issue, the nurses still do not seem to care no matter where she is. We have also had numerous issues with people stealing from her. We are not sure if it is staff or patients, but the most recent was her telephone (which they had replaced with an older, crappy one) and that we had to provide in order for her to get in-room calls, as well as money she received for Christmas from her sister. It is sad that the people there are not treating her as she should be treated and not taking care of her as they would their own mother. It is also ridiculous that we have to worry about her items being stolen everytime we leave.

This facility, which houses about 200 residents, is a nice, secure, and clean place"”with no odors. It's located right across from Thunderbird Hospital and in Glendale, which is fairly residential, with some businesses scattered about. There is skilled nursing care here, but no particular memory care. It is family-oriented and friendly and welcoming to visitors"”equipped with vending machines and offering a spacious landscaped grass area with misters out front and a waterfall area where residents and visitors can gather. All the rooms are shared, but they are a good size, clean and tiled. There are also some common areas on each floor: four sections to a floor, considered pods"”with sitting areas in each pod. There is one dining room on each floor, and the food served there is good"”well balanced with protein and vegetables. Tellingly, residents do not complain about the meals. There is a beauty shop and a restaurant onsite that is nice and clean, offering a few choices off a small menu. But the quality of the food is good. The activities include bingo and other games, as well as sing-alongs; few outings into the community are offered. There are occupational and physical therapy staff members who are good about taking residents promptly to and from appointments. There is a nurse and one CNA to each section, who has 10 residents each. One recurring complaint is that the staff is a little lax in medication management; some residents say they don't get their pain meds on time. And one patient who had a bladder infection was not adequately toileted or monitored. The residents, most of whom are elderly, basically require a fair amount of assistance, but don't always get it in a timely way. The cost is similar to other facilities in the area.
