My wife is in Horizon Post Acute and Rehabilitation Center. You don't get inside and there's no visitations. They're difficult to reach on the phone, though they'll eventually call you back. The place has a very positive reputation. It's in a very park-like setting that's placid and serene. I can't say anything bad about them. They're busy and can't just pick up the phone and call any time. I have talked to one of the therapists and the staff nurse; they're very professional. I brought a cake during her birthday and the staff was by the door, so they told me to put the stuff down and I could leave; those are the rules. They also accept COVID-19 patients. The location is right across from housing and the neighborhood is single family homes that all look neat and clean. It wasn't a gated community, but it was better than average community of working people.

Physical therapy is almost non existent. Out of a 11 day stay, my loved one only got a total of hour and ten minutes of minimal therapy.

Don't send your loved ones here. The therapy staff have no experience with using proper therapy techniques with helping patient needs to get better, the equipment is underrated for patient needs. They had a patient who had been here over a year fall in a Hoyer because they never had the correct Hoyer for a large person. If you need a pain med at 5pm you won't get it until the nurse gets there when she decides to arrive at 11 pm, then say no one told her about it. The cna's will only help when they are good and ready to help you. The food is ok, but the menu never changes.

Never send a loved one to this place. It is horrible. Meals were always cold! I had 2 meals that were warm out of 12 and always was unappetizing. When you use the call button be prepared to wait 20-60 minutes before they respond. At night you would hear other patients yelling for help. You could hear staff fighting among themselves, very unprofessional. Nurses would tell patients to shut up and not talk to them that way or I don’t do that I’m a nurse. They NEVER return your calls and they screwed up my medications when I left. Stay away from this place!

If you love someone who needs help, DON'T SEND YOUR LOVED ONE HERE. The only positive thing I can say is about MOST of the Physical and Occupational therapists. There is 1 blonde woman who treats me like I'm a 2 yr old so I treat her the same way. The male therapists are awesome. You nor your family members will be updated or be told the truth. I met with the director Scott who has not been back to speak to me as promised. Everyone passes the buck around here. No one accepts responsibility for anything. They like to blame someone else. The rooms are small with broken down furniture and you will only get about 20 TV channels. I couldn't tell you about any activities because due to an outbreak of COVID-19 here, it's closed. The food here sucks as well. I have a lot of food allergies and of course I ask about questionable foods. I was assured yesterday that the Taco meat had 0 Taco seasoning. Thank the Good Lord above that I never leave home without Benedryl and I was able to keep myself alive. I've yet to get an I'm sorry from anyone here. This place was rated 5 stars by our insurance company but I plan on emailing them, and everyone else that I can about how unsafe, and pathetic this place is. This is so sad that they are getting away with treating people. I plan on emailing, calling and handwriting letters to every department/place/state agency as well as the Better Business Bureau all about how I was treated. If you love your loved one, please don't send them here.

Please do not send anyone to this facility. Awful! me and my family still traumatized about our loved one there. Had to call 911 to get them out. Our love one still will not speak of what happened there. Just tells us like being in prison. The nurse was disgusting and no one would make a move without her. Our loved one was was sick and lethargic in 2days he was there. Mind you had our eyes on him the whole time. Mercy on this nurse's soul. She should not be doing what she does. Physical therapists an aid saw him declining and were very helpful. Have 1star review because wouldn't have been able to write a review. If hospital tells you your lives one needs to go here or any other facility please know you do have options to send loved one back to hospital or a different facility. We didn't have the knowledge we have now.

My grandmother was just discharged from here. Don’t get me wrong she had a lot of helpful people there. There were many people that were very nice. I was not pleased with how she was discharged. She had raw private parts, was wincing in pain and NOBODY told us. It would take 2 seconds to say to us, “hey she has this. You should buy some medicine.’ NOBODY said anything! Where is the courtesy? Furthermore, how long was she like this that this wasn’t communicated to us! Secondly, we bought a sliding board that was supposed to have been sent home with her. It was not! NOBODY made efforts to get it to us before leaving for the day fully knowing we need it help her move. I was way too angry to call and be pleasant so I had my uncle, my grandmas son do it and we had to go pick it up. This is unacceptable that it had to come to us getting it because of their failure! Throughout her stay, We never got updates on her regularly. They only gave us one opportunity to video chat. They had the least bad covid numbers yet somehow she got covid. I’m not fully saying this specifically was on them since we can’t know for sure. It takes so little to be nice especially right now. I get it people are busy, overworked right now but this is no excuse for the lack of communication and failure to provide the needed equipment to care for my elderly grandmother!

Good Samaritan Society is a descent nursing home. I would recommend it to a specific group of people. When I was there in 2011, I noticed that the Good Shepard was very clean and odorless. At the staff was very pleasant and tentative to the residents. I didn't see too much social interaction for it to be a nursing home. This facility is good for individuals who need rehab and they are going to stay short term. The facility is an older facility so do keep that in mind. Out of five stars, I would rank the grounds a three the common areas a three the residents' rooms a two the staff a three and the quality of care a three. Overall, I'd give Good Shepard Society a rating of three stars because the facility is older and needs some renovations, also the staff was not very friendly when I needed supplies for the patient I was caring for.

My mother has been in Glendale Care Center since 2007 with dementia and I have never seen anyone treat her badly. They have gone out of their way to do everything for her to make her comfortable. I don't know what that person that wrote the other review saw, but it must have been in their dreams. This is one of the cleanest places I have ever been and believe I have even worked in many of them. You are going to have a time when there is a smell, after meals when the staff changes all the diapers. There is no way that you can change that many diapers at one time and not get a smell. It goes away. They have music, they take them to baseball games, stores etc. and their food there is excellant. My daughter and I would pay $2.00 for a meal ticket and the food is terrific. I would recommend this nursing home to anyone. My mother is now passing and they are there for her. God Bless them.
