My husband is in a skilled nursing facility called Center at Arrowhead, LLC. It's a good place. It's clean, and he is well taken care of. The people do their job well. He's been there almost three weeks now. They cannot do therapy if he has pain in his leg. He's the only one in his room. It only has one bed. The whole building and the surrounding area are clean. The inside and the outside are clean. The people working there are clean. The bathroom is clean. The staff is good and friendly, and they care for my husband. The meals are also good. Everything is OK for me.

To the Director and the Staff at Center of Arrowhead. Our experience at this location was outstanding. I am so glad that out of 15 locations provided I choice the right one. From the Director to the employees was great. Is there room for improvement absolutely, but for the most part it was very a very good experience. The Staff was excellent and if they were not they know who they are. The Food was good. Hosp, Rehab food is better for you then home. Not a lot of season which we don't need. Now, the most important part about these facilities is to stay apart of the facility with your love one. This is not a drop off and not be present. We all are in this together to get our love ones back the health and strength they need. GOD says to be present with the body. So, who ever body is in need, others have to be there to. I was there daily 5-7 hours a day. If you don't have that time and you have other family members that can help out then take shifts. It was me only so I did what I needed to do. Very clean and Professional facility.

If you are reading these reviews, it is likely because you are trying to find a safe, caring environment for your loved one. This is the reason that I am writing this review. The main point of this review is to encourage you NOT to choose this facility for your loved ones. There are many, many issues with this facility, but I will provide just a couple for you to get an accurate picture of the patient experience. My mother arrived there on a Friday night following a hospital stay. On Saturday morning, she informed me that she had been pressing her help button for 30 minutes with no response. I called the front desk to ask someone to go in to help her. The receptionist took my mother's name then placed me on hold. I waited on hold for 12 minutes before being disconnected. When I called back, the receptionist said "Oh, yeah, what was your mom's name again?" By this time, she had been left without assistance for over 45 minutes. Again, I was placed on hold then transferred to a voicemail with a message that said the mailbox was full. It took me about an hour to get anyone to answer, and by then, I was already driving to the 45 minute drive to the facility to assist my mother. When I expressed my serious concerns with my mothers care and informed the facility that I was in contact with Medicare to log a complaint (which was true), my mother's care TEMPORARILY improved. By the following day, we were having the same issues. Once again, I called the facility to request care for my mother. On my first attempt, I was transferred to the on-duty manager "Jason", but got his voicemail and his voicemail box was full so I could not leave a message. When I called back again, I was transferred to the case manager so I left a message. She also did not answer and never returned my call. Many times, nobody answers the main phone number. Given the many issues, our family made a decision to remove her from the facility for another option where she would be safe and properly cared for. My mother arrived at the new facility this evening and I already notice a MAJOR difference.

Charge nurses don’t pay attention. Total lack of communication between caregivers. Too many double shifts worked. Extremely poor care. The only decent thing was the food and even the dietary dept is lackadaisical. Medication errors. Do not send any one that you care about here.

My sister has been at The Center at Arrowhead for 3 weeks now. It's been a very positive experience. It's very clean and very new, and the staff has been very cooperative. There's physical therapy available for her. She was in a private room -- a small apartment type -- without kitchen facilities, but it was a nice size, and accommodating for her needs.
