Self Health Care & Rehab Center is nice and in a separate building than the nursing home. The staff is good. My mother-in-law has a private room, and the place has amenities.

The admin/social work side of this place is awful. Typically a facility like this will help you work through the Medicaid process. And typically they even apply for you but at the very least they help you

The facility itself is visually nice. The care my grandmother received from the nursing staff was good - for the most part. I really liked the nurses I met. However, she did not receive a bath the 18 days she was there which we did not know about until later in the stay. My grandmother can be very difficult about bathing (typical of elderly) because she is terrified about falling but I can't believe she did not have one bath the entire stay. The admin/social work side of this place is awful. Typically a facility like this will help you work through the Medicaid process. And typically they even apply for you but at the very least they help you. They tried to make us believe that if we did not hire an attorney - and one they coerced us to use - that my grandmother would be discharged until Medicaid was approved or we would have to pay out of pocket. They told us we could apply for Medicaid ourselves but that would be with no help whatsoever from them and since there was no way it would be approved in time, she would be discharged or we would have to pay out of pocket. All of this seemed very odd and very sketchy to me. I always thought a social worker's job was to help with this very thing. I did not consult their attorney recommendation. I contacted another one on my own. During my consultation session, he advised me that yes, all we were being told from Self was very odd. He said that the only time people get an attorney involved is when the client has assets as the attorney can help protect some of those assets. My grandmother has nothing, absolutely nothing, which we made very clear to Self. The attorney said it would be senseless to hire him or any attorney and an attorney should not matter in the admission to a nursing home process. This is what should have happened according to the attorney and according to many others who have been through this process and according to other nursing homes we contacted... The nursing home should apply for Medicaid for the client. The client or client's family must gather the documents required by Medicaid, for example bank statements. The nursing home accepts the client with Medicaid pending. Medicaid gets accepted. Medicaid pays the nursing home. We were able to find a nursing home close by willing to help us navigate the Medicaid process. They are applying for us. Self has discharged my grandmother three days early based upon acceptance of this other facility. This may or may not be normal... I don't know. But Self should transport my grandmother to the new facility which they are also unwilling to do. They say we have to transport her. The facility we are moving her to was surprised by that with us. The admin/social work side of this place is absolutely useless. Useless. This may or may not be the fault of the social worker. I do not know. It may just be bad policy of the facility itself the social worker must abide by but they are absolutely useless..

Self Health Care and Rehab Center is wonderful. So far we have been really happy with it. It’s centrally located between my three siblings, and it’s one of the reasons that we got it. The biggest thing was that we were able to get her into a private room. The room is nice and large, and we have her set up with the nurse, but then we have been able to put our furniture in there. She has a living room area also. The staff is absolutely great. We are very happy with the nurse that takes care of her on a daily basis, and then the caregiver that works with mother with her personal care is very good.

This is an excellent facility. Staff are very accommidating . My ONLY complaint is the food. My mom is on a no salt/no sugar diet. Dietary had a VERY hard time sticking to this. It got to the point where she refused to eat anything because they could not tell her if had salt in it (processed or can). I feel like if you cook it...you should know what's in it. Hopefully they will be able to fix the dietary problems. I would definitely recommend this facility.

Excellent run facility; friendly staff; adm helping with Medicaid applications and working with family members; well run and came highly recommended from an assisted liviing facility nearby which was not at that time licensed to care for advanced dementia patients.

I was very pleased with the love and attention not to mention the care that she received while residing at this facility. Her nurses were very caring and loyal. They made her smile when she was a bit down and they comforted her when she was inconsolable. The activities director at this facility was absolutely the best in all of the land; she made sure that my grandmother was not left out of anything; from Bingo in the cafeteria, to the spa day when they had the residents hair fixed and their fingernails and even the toenails painted if thats what they wanted. My grandmother loves Elvis, so they even had a look alike come in and have small concerts for them. We couldn't have asked for better care or assistance in her recovery. I would definately use them again if ever the time should come. The only complaint I had was the smell. I know that can't be helped because every single one of the nursing homes that I visited smelled the same way; medication, alcohol and clorox just like the hospitals.

From what I've seen the facility is very nice. The workers are always very humble to the family members as well as the patients inside. I've never seen any sort of problem with the nursing home itself or anything else inside the nursing home. The owner of the nursing home, Mr. Self, stops by frequently to check through and make sure things are flowing, as they should. Much like the employees that he has, he himself is a very nice person and very friendly to the residents and their family members. We even held a birthday party for my 96 year old grandmother who is in the nursing home and the employees had just as much fun being there as we did. They were always there if we needed anything at all or any assistance with drinks, ice, or anything you can imagine that could be needed during that time. It's a great facility.
