My mom was there for two and a half weeks for rehab, bed linens were not changed ONCE. Physician ordered an nutrition supplement twice a day, lucky if she received it a handful of times the entire time she was there. Food was almost always cold and inedible. The excuse was oh, we're out of this or that. Did not matter who you talked to because the issue was never resolved. Shouldn't have to keep after someone to clean the bathroom either. Other than the therapy department, would not reccomend this facility.

TERRIBLE TERRIBLE TERRIBLE!!!!!!!! The doctors will not speak to family members. Any questions, answers or concerns can only go thru the already understaffed nurses. If you speak to different nurses you get different stories. The wait time for a nurse to respond to the call button is an average of 1/2 an hour. I stood at the nurses station while several rooms called for help and I watched as they were being ignored because the nurses were too busy having personal conversations. my mother was given medication for which no one explained the possible side affects. I had my mom transferred to another rehab a week after she got there. DO YOUR HOMEWORK PEOPLE! if it's someone you love...go somewhere else

No one, even the director has no voicemail, right there should be your first concern. Nurses give residents meds a hour before and up to a hour behind when the resident is to receive them, but "within medicare guidelines" so when you're unable to do PT because your meds are wearing off they don't consider that it might be because of that. Any person with a loved with Parkinsons knows that they can't function properly without the timely meds. You get on a schedule to get a shower in a room across the nurses station. None in room. They love to throw the word dementia around. "Well, your Mother has dementia" Yes she does and it is slight dementia don't assume you know more than I about her level of dementia. She lives with me and I am a licensed health professional. OT said the glasses are too heavy for my mother to use but still have her use them in the dining room. Then she breaks one. Luckily she didn't cut herself. 25% staff great Ate a meal with my mother, the food is disgusting. Whipped instant potatoes. Heard this place was great in the past but not in our experience.

My mother was at Dycora over a year ago and things were OK. This past March and April things were very unsatisfactory. Many care givers were rude and uncaring. The therapists had a negative attitude. They would come for her before she even had breakfast, and she would t ell them she wanted to eat before having therapy, and they would say she should come now "to get it over with ". Or they would come 5 minutes before she would be going to lunch, and said it would be a "fast therapy session " If my mom ever needed therapy rehabilitation in the future, she would never go back there again.

My mom is in rehab at Dycora Transitional Health. I see them as an average location. The care has been OK. They promise that they're going to do various things, but then people forget, and so we have to remind them. The food is awful. They need to improve it. My mom complains about it everyday. I also know she is having issues with her phone and with her TV; however, everything else seems to be OK. Everything else is what I expected.

If you love your family member do not send her/him to Heritage Square. New Managemetn is terrible, they only care about cash they collect and do not care about residents. AMY the director is very discriminatory to both staff and residents.. She smiles and talks a good game when the family is present, later treats everyone like dirt . Nurses have no regard to residents needs. If you call for the bathroom, you will wait for at last 1 your, then they will leave you in the toilet for another 45 minutes. if you wet your bed, they will change you by the end of their shift. If you ask for a sip of water in the middle of the night, they will tell you it is not your breakfast time. My mom is very quiet and not demanding person . She begged for a glass of water and the nurse refused to give it to her even though , there are no doctor orders to limit her liquid. The nurse was simply too lazy to do it. It completely broke my heart to hear this .They staff is heartless, think twice before making a placemet decision for your mom. , Your love one will only decline instead of getting better. Heritage Square should be closed by the State.

Both of my parents were residents at this facility in the fall of 2016. My mom, in the past, had received pretty good care but in fall of 2016 the care was awful, to say the least. There were too many incidents to relate but a couple that stand out was when my mom asked for assistance to go to the bathroom in the middle of the night...the aid left her sitting on the toilet for over 2 hours and my mom was unable to get up on her own. In another incident a "nurse" and I use that term loosely noticed a scab on my mom's neck, my mom had indicated that it was bothering her with her shirt collar, hoping just to get a bandage to go over it....instead of a bandage the nurse took a pair of scissors out of her pocket and cut the scab off and told my mom not to be such a baby. The wound became infected and took quite some time to heal. My dad was also at this facility in October of 2016. After being diagnosed with terminal cancer my dad was admitted to Golden Living to try to get his strength back prior to going home. While at the facility he was leaving every day for radiation treatments. Those treatments really upset his stomach and he was supposed to be given anti nausea medication. Well for 24 hours he not only didn't receive his medication but was vomiting for 24 hours and never given any liquids. When my sister went to see him the next morning she couldn't believe how sick he was, she called our sister-in-law who is a nurse and asked her to come in and assess him. Needless to say my sister-in-law who is a nurse made the decision that he had to be transported to a hospital immediately for fluids....after an ambulance was called the head nurse of the facility came in to ask what the problem was, my sister-in-law informed her that he was SEVERELY dehydrated and she was having him transported to a hospital for IV fluids....at that time the head nurse told my sister-in-law that he could just get that at the Golden Facility which begged the question why he hadn't been given fluids in the last 24 hours! He was transported and without an hour of getting IV fluids felt 110% better!! There was one very nice, attentive aide at this facility (Trudy) but overall the care my parents were received there (on the days that Trudy wasn't working) was deplorable. My parents were discharged from this facility in early November 2016 and my mom passed away 11/21 and my dad 11/30.

If you are considering having a loved one go to Golden Living-Heritage Square please think twice. This facility is lacking in quality trained staff (high turnover), poor upper management, social worker and Nursing Managers, lacks regard for patients basic needs being met, lots of blaming of why things do not get done, little to no rehabilitation actually happening and in general this facility has too many problems to mention. Please know that your loved one deserves better than what this facility is willing and able to provide. DO NOT GO TO GOLDEN LIVINGCENTER!!!

After falling and breaking a bone in her leg, my mom stayed at Golden Living Centers. The staff here were wonderful. They provide a free meal for the family and the meal was served at their "bistro". We had a choice of two entrees and were served like we were in a restaurant. My mom was kept very busy with activites like bingo and movies. All the other patients were friendly, happy and outgoing and my mom made many friends, some cried when my mom left. The staff took the time to treat the patients as people, listend to their wants and needs, doing what they could to fulfill the needs. My mom talks about this being the only place she would go back to for rehab. When leaving the nurses that were discharging her welcomed her to come and visit whenever she wanted. Her stay at Golden Living Centers was a wonderful experience.
