I am a friend or relative of a current/past resident

When my mother was admitted to Life Care, her room was dirty. There were crumbs and food wrappers in the dresser drawers, the floor hadn't been mopped and it looked like there was urine on the AC unit. On multiple occasions she has waited upwards of 30 minutes for someone to assist her in the restroom. She has had to shout from her room for assistance even though her call light was on. She has been there for 10 days. Just on Monday, of this week (5 days ago) they created her treatment plan and we met as a team and they said they would revisit the treatment plan in a few weeks and discuss a discharge plan. Today they called to tell me that my mom has reached her stay limit and will be discharged after the weekend. I know for a fact she has not reached her stay limit. When I called BS the social worker told me that she has met her therapy goals... in 5 days. No, she hasn't. She does not feel safe and they failed to add going up and down stairs on her treatment plan even though we brought it up several times. They have not even attempted to have her go up or down stairs. Her home has 4 steps to get up just to get inside. At the initial meeting I asked if I should get a ramp and they told me no and to wait until we meet to discuss discharge. Well, we aren't meeting to discuss discharge they are just going to kick her out whether she is ready or not. The food is terrible, the activates are always the same, the facility is dirty, some staff are okay others are terrible. I would never in my life recommend this place to anyone.

What I liked best about Life Care Center of Port Orchard was my mother-in-law's room, it was very big. She got lucky and got a room that was by herself. It had a big bathroom and it was next to the nurse's station. The food doesn't taste very good. When you walk in, no matter what time of day, it always smells like poop and pee down the hall. The rooms are dirty. The physical therapist was very good, though.

I have a friend whom I have visited quite frequently in the three years she has lived here. I have always been impressed with how the CNA's and nurses work together in the tougher times when staffing issues happen. No complaining...just doing! I am also impressed with how much the activities staff entertained the residents even during the worst of the lock-down. Now, with everything opening up, they are bringing in new activities every day! I would certainly bring my family member here to stay...My friend is very happy here!

My husband was at Life Care Center of Port Orchard for a couple of weeks for rehab. We weren't happy with it. He never got a warm meal. It was not bad, but it was just that every meal was cold. Even the scrambled eggs were cold, and they weren't good at bringing him water either. The staff was OK, and the facility was nice and clean. He was supposed to have a lot of physical therapy, but he never really got that much.

This facility is terrible. While my dad was there, they were understaffed, rude, secretive of his care, manipulative, and uncaring. They severely neglected my father. They over medicated him, left him in feces for hours at a time, wouldn’t let him use a toilet, he had feces in his bedsore, on his hands, and on his clothes on multiple occasions, they wouldn’t brush his teeth, they wouldn’t give him proper assistive devices, they gave him a broken wheelchair when he got there (they had to roll up the cushion because they didn’t have one that fit the chair). When I complained about the bad care and neglect the director of nursing told me that maybe my expectation for care was too high. The staff yelled at us the first night that we were there because we were feeding my father (His doctor had told us to feed him that day). They wrongfully took away my father’s therapy. He was always covered by insurance and they refused to give it to him. Then when he finally did get his therapy back they did it improperly and tried to make me sign a Medicare document saying that I would be liable for 20% of the therapy costs (my father wasn’t on Medicare). They falsified his progress to make it look like he was doing worse than he actually was (I found out later that they get paid more money if they do this). Don’t go to this facility for therapy. They will not give you good therapy. I complained to the executive director many times and he would pretend that he cared and then would continue to do bad things to my father. That person is very manipulative and should not hold the position that he does. They over medicated my father to the point where he was very ill and throwing up everyday. They would lie about how often he would throw up. When I begged for him to see a doctor, to change his medications, they wouldn’t let him for many days. On the day that they finally let him see a doctor he was extremely weak and had lost lots of weight. Instead of letting him rest that day they did a bunch of therapy tests on him and hid that from me (he was in no condition to be tested that day). When they finally let him see a doctor he was taken off the medications that were making him sick, didn’t throw up anymore, and could finally eat. The staff wouldn’t communicate with me about my father’s care. I would have to frequently request his medical records so that I could try to find out what was going on with my father’s care. They almost killed my father in this place. I still have nightmares about it. This facility does not care about you or your loved one. All they care about is profit. The fact that this place is in the Kitsap County Community makes me sick. People come to a Skilled Nursing Facility because they are vulnerable and need help. Life Care Center of Port Orchard takes advantage of that vulnerability. I can’t even list all of the terrible things that they did to my father because it would be over 100 pages long (I wrote it out). If you are an elderly person who doesn’t have much wrong with you then you will probably be ok in there. If you are vulnerable, have sores, brain issues, catheter, need a lot of care, etc...then you will not get good care here. Also, if you do go there, make sure that your loved one sees a doctor outside of the facility. The MD and ARNP (from Swenson Healthcare) are corrupt as well. It is my experience that they help Life Care Center of Port Orchard with covering up their bad care. Make sure that your loved one sees a legitimate doctor outside of the facility. If you are experiencing bad care at a Skilled Nursing Facility then please report it to DSHS Residential Care Services: 1-800-562-6078

I visited this facility

My mom's been at life care 2 months. In the beginning the staff was on top of her care. But as the visiting slows down so does the attention of care.my mom has dementia and can't do much of anything to help herself.i would keep track of things. ( No tooth brush for 2 weeks) I would do it. Wouldn't wash her face Ect. I did.. meds not on time. My mom can't talk. Walk ect on her own. At lunch one day I watched my mom drink 6 glasses of water.and juice. ( I gave them to her) I got mad and asked the nurse. If she can't talk how do you know when she's thirst? He looked at me like . Ha da. Your right. Us the family. Kids may Knott be able to care for are parents. But never ever stop checking on them. Make sure they are getting looked out after. If Knott there will be a repeat of 1995. A 8 he shift went by and no one noticed a man lay dead in his room undetected for 8 hrs.

This facility is clean, doesn't smell, and is a pretty nice set-up. But the nurses and their assistants just don't have time to provide appropriate care. My mom has been there a week and every single day she rings the buzzer to get help to go to the bathroom and (literally) no one comes to help her for hours. I saw it multiple times a day. Then when she does use the bedside commode, the basin isn't emptied and it sits right next to her bed full of urine for a half day. The nurses are all super nice and cheerful. They're just too busy to give proper care. Meds are given late, food is never what she ordered.

Alot of neglecting and nurses that dont give medications. When my mother was on the west side of the place, she enjoied it. however once they moved her, the CNAs there do not give her the call light button, they shut the door completely, one nurse in particular refuses to give my mother her PRN medications because she has already received them. That funny! I was there with her once ALL DAY and not once did this nurse give my mother her PRN medications. Im a CNA myself and i know what these people SHOULD be doing. Find another place to put your loved one in.

My aunt, who is three months away from being 100 years old, has lived in Life Care Center for 2½ years. It's been a great experience for her, but they don’t specialize in memory care, and there's no lock-down. Both my aunt and my mom were there. The rooms are big, the facilities are nice, and there are wide aisles, which I think is important.

I visited this facility

I was just recommend last week by the hospital to check this place out. The place was very inviting, warm and smelled good. I noticed that all the staff looked happy to be there and caring for the patients. I was welcomed right of the bat and was given a tour and was able to pick a room for my grandpa that very moment. He has now been there for 4 days and he would rather be home but says this is pretty close. The entire staff is very warm and welcoming to him. I am so happy that I found a place that really cares.

This was the cleanest building that I had been in. It also had the best nursing team, they were very professional and took excellent care of me. I was recommended by a friend of mind who is a nurse and I would recommend not going anywhere but here.

I was a pt here and the staff was great my sister was here too last year. The therapy team is amazing, They serve very good food, nursing is caring and prompt. I would go no other place but here.

I visited this facility

This place was amazing my dad was there for about 6 mo after an accident. The staff was so caring and kind. My dad was so nervous in going to such a place but they made the transition so easy. The food was pretty good especially since my dad was on a low sodium diet they did the best they could to give him something tasty to eat. I would most certainly choose this place again if ever put this position again

This place is absolutely awful. My grandma is in there well was she is in the hospital now due to a bladder infection. First the lost her. They had her in a room she wasn't even marked in so they didn't feed her. Then they gave her medications and did not write it or anything in her chart over night. The nurse that came on duty knew nothing of how her night was or what they gave her. She was not acting like her self at all. She has a strict diet due to her medical issues and what ever is on her meal slip is what they are suppose to give her. They never do. They give her whatever they feel like sloping on her tray. How hard is to match what is on the meal slip? This is peoples lives they are messing with. The nurses or someone should double check the slip and the meal to see if they match. I was there for a month and all this happened. If it was up to me and not another family member I would find another facility before they kill her.

My wife has been at Life Care for a week now. It's not quite as good as the other nursing homes that she's been to. There were a few times that they forgot to get her for dinner. That wasn't good. But they're getting better. The place is clean. They have a TV in the room but my wife can't plug it in because the cords are too short to plug in.

I visited this facility

This was an amazing building. The care that was given to my mother was so good. I can't say enough good things. We toured several other buildings and Life Care of Port Orchard was by far the cleanest and had the friendliest staff. The care and compassion demonstrated was incredible. Thank you Life Care.

I visited this facility

I found my experience while visiting The Life Care Center in Port Orchard to be extremely frustrating. Every time I visited, they were short staffed, shower's were not being given as scheduled and the food choices did not seem healthy. There were unanswered call lights constantly on. Some hallways more than others smelled of urine

I visited this facility

I have had the chance to spend the last month at this facility caring for someone who needs lots of assistance and rehabilitation, the CNA's and nurses at this place are so caring, the building is clean, there is no odor, all the staff seem caring, and committed to providing a safe, happy place to both live and receive rehabilitation. I have worked at many of these types of places and this one is far and away the best I have seen.
