I am a friend or relative of a current/past resident

My Father is currently a resident, just until the end of the month. He has only been a resident for a month and has lost weight, fallen out of his wheelchair, and has a knot on his head, his room remains dirty. I provided him with a comforter and some how it went missing and has never been found. While many of the nurses and CNAs are nice the level of care is garbage.

My sister was a resident, this was the worst care ever, place was not clean, when she called for help she they would come when they wanted to, one night it took them three hours to get to her. She was not able get out of bed to help her self. Because of the care she did not get, her knee surgery, it got infected she had to have another surgery, they waited 24 hours to put her wound fac back on her after coming back from her dr's appointment. she had tell them she would call 911 before they put the wound fac back on. Now she is fighting the insurance company. She is having to have antibiotics for the next six weeks to try to handle the infection, IV antibiotics. The insurance company is stating they will not pay for care.

Great rehab, new administration that is involved and will take care of issues if there are any Nice large private room while we were in rehab Also we were able to keep our family there while Medicaid was getting approved.

Above-average physical therapy at Trail Lake doesn't compensate for the large gaps in nursing and dietary. Although a few staff go out of their way to help, the administrator is such a problem that my advice to families needing skilled care is "keep looking." Not worth the hassle.

Rooms are dirty! If a doctor has just come, there will be used bloody bandages on floor or blood droppings. Floor is only cleaned in small walk area. When you try to talk with staff, they hardly look up at you. They are always standing around desk talking with each other. Never smile , always act like its a pain for them to have to do one more thing, When there is someone who is bedridden can't do anything else , you would think they would have a little compassion for them, after all why are they working there if they just don't care and don't want to be there, my family spends everyday all most all day and into the night there. because they known from passed experiences she will not eat or get her diaper changed.Its so sad to me that when people get old and need care that this is how they are treated. There is one lady that comes in and smiles to her all the time and calls her by name , and has patience with her. I wish you had many more for her! we have spoke to main office employees for help in this matter. we are always told that they will look into it. but to this day noting has changed . its been much longer then we would have thought she would have lived. I just whish her quality of life could be better

Family member was a resident. Loaded her up on 3 different benzos to control her. Large gash on arm that went to be unexplained.

This is a joke under new management and going down staff sucks From Don to ADON,they act like they care only wben State is in the buliding,barley have Laundry detregent to wash there clothes,dont Bathe the patients like they should.I wouldnt Dare put my love one in this place thats why alot of the good staff left.

I am giving this facility two stars because I have a family member who has been there for a year and they still do not seem to inderstand that she cannot care for herself. She has continuous oxygen ordered by her doctor and is usually hooked up to an oxygen tank that is bone dry. She requires routine breathing treatments that are never monitored, She forgets what shes doing and turns it off so the medication sits in the nebulizer until someone dumps it out or one of us finds it and gives it to her. I have found her plastic cup FULL of pills sitting on her besaide table and this is where I draw the line. There is no excuse for failing to deliver her medication. I have discussed the issue with the facility numerous times and no improvement, only the same excuse that they are short staffed. Not good. The two stars go to the housekeeping staff. The building is kept clean.

Be careful of personal belongings. Apparently, the staff is not at fault for any damages or throwing away items. It is the responsibility of the elderly that is in their care to watch out for their personal items even thought they may not be in the room. My mother's dentures was accidently thrown away by a CNA but it was not the fault of the staff member, it was my mothers fault for putting them in a covered cup because she could not reach her denture container. The Social worker denied all responsibility. So don't bring your dentures to this facility, could get thrown away when you are in the bathroom.

My dad was at Trail Lake. They did everything. They took him to dialysis. It appeared to be nice, and it was OK for a facility. The rooms were large, and the facility appeared to be clean. They served meals, and for the most part, it was a decent little place to be.

My mother is still at Trail Lake Nursing & Rehabilitation. My problem with it is that it's very expensive. It used to be great, but it’s kind of gone down a bit. I don't know if it has gone down a bit or it's because I'm more aware of it. She moved in there first because I liked it. It was kind of newish, and I liked the décor, but she has been there 2 years now. Last month, they fired their assistant activity director. Now, there's only one activity director, which I have complained about. The one they have works very hard, but it's not as good as two.
