I am a friend or relative of a current/past resident

I gave Heartland of Hillsboro, formerly Legacy of Hillsboro one star because zero is not an option. My mom has been a resident here for a year and a half. The rooms are too tiny for two people ( 12'x16'). My mom is bedridden and when we visit her roommate goes into use the bathroom, with the assistant of a nurses aid, and the aid does not care to give my mom's roommate the dignity of closing the bathroom door, pulling down the roommates pants and taking off her depends, exposing the roommates naked genitals for me, my mom and brother to see, which should be against the law if it isn't. They are constantly moving my mom to a new rooms, damaging her refrigerator and television, which belongs to my mom, when they move her. Mom is bedridden and today when I visited, she had been in this new room for four days, I found her doctor ordered air mattress to be unplugged and deflated. Had it been like that for four full days and no one cared to plug it in? There are no chairs for the residents to sit in, either bed or borrowed wheel chairs that are not suited to the residents size . Call lights are not answered for sometimes an hour or more, and because my mom needs her needs met sooner, she calls out for help. To fix her calling out, the nursing home keeps my mom doped up on Ativan, a very dangerous amnesiatic drug around the clock, every single day. My mom hasn't been dressed in months, and is kept in a hospital gown, rarely gets a shower, and although she was ambulatory and not at all incontinent when she entered this " nursing home" a year and a half ago for " physical therapy" she was bedridden and given a permanent catheter so she would be easier to deal with. She recently was hospitalized for a drug resistant urinary tract infection that she got at the nursing home, it was so bad the ER physician asked me how long it has been since her catheter had been changed, because it was "the worst he had ever seen". Ice water, which my mom never has when I go to see her is dipped out of a big cooler, gross. Please do not put your loved one her. The care is so bad it borderlines being criminal. They have ruined my mom's life and health. This place should be closed immediately.

I feel Heartland is the best place we could have found for our mother, who suffers from dementia. ALL OF THE STAFF genuinely care for her well-being and are always in touch with us for even the slightest change of anything. Sure they are understaffed, but so many people are just too darn lazy to work, so you can't hold the facility liable for that fact. All the nurses and aides alike treat our mother like she was their own. She is always clean, has water at the ready to drink, and the place most importantly is CLEAN!!!!!!!

I am a friend or relative of a current/past resident

My mom has been a resident here for over 9 weeks now. So far I like what I see. I visit pretty much every other day, sometimes every day, sometimes skipping 2 days in a row. She is well taken care of and they are very kind to her. She has told me often that she doesn't want to leave here; she looks very happy most of the time. Everyone knows her name, and I feel she gets personal care and the nurses take time with her. The kitchen staff are wonderful, as are the ladies who clean the rooms. Most everyone is friendly and seem caring. The facility does not have a nasty odor when you walk into the front door like most rest homes do. If the quality changes I will certainly post a review reflecting that. I'm sure no place is perfect and there are issues everywhere, but saying that I will also say this- you can't please every one, and some people are just nasty and would complain if they found a hundred dollar bill because it was wrinkled. Also not sure that an occasional visit qualifies a person to make a sound judgement call.

My mother is at Heartland of Hillsboro right now getting physical rehab. It’s a very nice facility, and they are taking good care of her.

I am a friend or relative of resident

Horrible place. They are under-staffed, they do not care for the patients nor do they take their safety or overall health into account. I have seen them allow patients with respiratory staph sit in the hallway and cough continuously, seen them unplug and NOT plug oxygen machines back in, and serve meals cold and not feed the patients that require help feeding.

I visited this facility

Nice staff. All 4 of them. Not enough staff at all. Needs at least 4 more patient aides each shift!! And a janitor/maintenance employee outside. The trash can outside the entrance door at the patient/guest patio has been over full for 5 days now. Food drippings all around it and covered with ants which has trails going underneath the patio table & chairs. The odor isn't pleasant when a picnic is planned out there either. The back door by the therapy room is an eyesore with dozens of cigarette butts & some snack wrappers laying outside the door on the ground. Been there quite some time.
