A Grace Sub Acute & Skilled Care, located in San Jose, CA, offers skilled nursing care and short-term rehabilitation. The community provides a range of amenities and services aimed at supporting the well-being of its residents.Amenities at A Grace Sub Acute & Skilled Care include organized activities and programs, entertainment options, social events, and spiritual activities. Residents have access to WiFi/internet, salon services, and group exercise sessions, all designed to keep them engaged and active.In terms of services, the community has nurses on staff to provide medical care. Physical therapy, occupational therapy, and rehabilitation services are available. The community also accommodates special diets and dietary needs, with therapists on staff to assist with various therapeutic requirements. Located 40 miles east of San Francisco, atop 267 acres of scenic property, the Masonic Home at Union City offers panoramic views and a moderate climate. Its gated grounds are home to about 300 residents who live in comfortable studio or one bedroom apartments. The campus offers independent living, assisted living, skilled nursing, memory care, and on-site post-surgical rehabilitation. Residents enjoy multiple health and wellness centers, walking trails, activity areas, comfortable indoor and outdoor areas for small or large groups, a lodge room, and a worship center.
