I am a friend or relative of a current/past resident

The people at Fair Haven Of Forest City are great with mom. They do a good job keeping up with her, treating her, and helping her with everything. The food is good. The facility is really clean. They have a lot of social stuff that goes on where they have them get involved.

I am a friend or relative of resident

I have a relative in this facility. The overall experience has been good. My relative is suppose to be on a strict diet. Food Service is constantly given my relative food and liquids they are not allowed to consume per their Doctor. Dr. Whitworth has ordered a special diet for my relative. My family is having to take food to my relative. My relative was required to use a diaper by this facility. The third shift CNA would not change the diaper when my relative had soiled the diaper. My relative was informed from the third shift CNA the diapers were designed to be used more than once before changing. The facility needs more help. The staff of CNA's is over worked most do a great job. The nurses feel like they are above doing patient care like changing a diapers. The average answer to the call button is 30-55 minutes and that is with a relative in the room. Overall a clean facility, staff including nurses need to be reviewed on proper care with dealing patient care and cross contamination. They are complacent when it comes to proper technique. My relative is overall happy living at this facility.

I am a friend or relative of a resident

The Occupational Therapy staff was focused and professional, but the patient care, handled largely by Certified Nurse's Aides, was inconsistent. Some days the patient's dentures were cleaned, some days they weren't. Sometimes it took less than 5 minutes for an answer to the call bell, sometimes it took 20 minutes and a walk to the nurse's station. My relative, a woman in her 80's who had a stroke, developed an open sore on her buttock, which was only noticed by the staff 3 days before the patient was discharged, and was described to us as "a skin tear". The clothing my relative wore on Christmas day was "lost", and when we complained about it, the staff was defensive and told us we should have put names in the clothing and brought a hamper for the room, none of which was told to us at admission. The "private" room shared a toilet with the next room. The toilet tank cover was cracked in half and the floor in the toilet area was stained. There are two large potholes at the entrance of the driveway, and there isn't enough parking space for staff and visitors, so people park on the grass. Although my relative made some improvement with daily therapy, the experience was disappointing. A nursing home's business is taking care of people who can't "do" for themselves. This home's management would benefit from a long, hard look at it's procedures for training, communicating, and supervising to find ways to improve procedures and care for its patients.
