The experience at Charlotte Health and Rehabilitation Center that my family had was horrible for the lack of another word. My spouse was there for short-term therapy after being in the hospital for two and half weeks. Ther therapy department is the only positive thing that I can say about the facility. The nursing staff gives medication without checking a resident's blood pressurer. When he would ask for assistance getting to the bathroom he was told to wait because the nurse assistant was taking up the finished dinner trays. It took her 15 minutes to return and then it was to late to go to the bathroom. The Unit Charge Nurse was in his room when I stated that something was wrong with him all she did was to just look at him from the foot of the bed. She didn't go up to him to check him at all. He ended up going to the ER and was in the hospital for 21 days with results of serious health problems. When they would change a resident they would not all the time wipe and clean them sometimes they would just put a clean Depend on them and out the door they would go. After this experience there I would not recommend this facility to anyone at all. Look somewhere else to place your love one.

Do not recommend. Our father was there for rehab as a dementia patient with a foley catheter after a hospital stay. After a few days Medicare cut him off and we lost two appeals (other professionals in the field said they could have tried harder to help us win the appeals), so we were on self-pay waiting for a urologist appointment because he couldn't go back to his memory care with the catheter. We finally get to the urologist appointment, to find that he has no catheter. Turns out, he pulled the cath out himself, a week prior. After investigating, it seems that when he pulled it out, the nurses tried but couldn't get it back in, so they left it out without informing the Director of Nursing or our family. Dad turned out to be ok but it cost us thousands of dollars in unnecessary charges for care he didn't need and wasn't actually receiving, not to mention that he could have damaged himself (he's on blood thinners and catheters are not meant to be pulled out like this!) and no one was notified.

I toured Charlotte Health & Rehabilitation Center. I like the fact that when I first walked in, it looked clean. I like the rehab area. All we have gone on is a tour so far. We haven't seen inside any other room because all the rooms were full the three times that we went. The staff seemed friendly, and the people seemed to be interactive with each other. They had an outdoor space. They had games that they were playing in the dining room. The dining room is where the interaction of the residents was taking place. They seemed to be interactive, more so than other places that we had been to. They weren't serving meals at the time. They were pretty much playing games in the dining area. They had some board games and bingo. However, we didn't get a chance to see one of the rooms. It would have been good even if we could have seen a picture of the room. The person who took me around was very nice and very informative. If all the things she stated are all my husband is going to receive, it's going to be very helpful. What we saw in the facility was very clean. The common areas were very clean. They didn't mention any appliances or anything like that. They said that the bathrooms were almost like a Jack and Jill because the bathrooms are shared, so I would like to see that concept.

This is a horrible facility. It is infested with bed bugs and roaches. They do not administer medication on some of the days

I think this is a great place for your loved ones, it has a great therapy department and caring nursing staff. Its also a great place to live when that time comes for your loved ones. Place newly remodeled.

The dietary department was wonderful each time I visited my Aunt here. A big concern we had was making sure she ate, and The staff was always friendly and helpful which meant a lot to my family and I during such a difficult time.

I had a family member who did rehab at this center in Nov. I am a nurse and found Charlotte health to be an unspoken charm. I had personal contact with management team immediately and all were helpful. Staff great and therapy could not be any better. Unfortunately my loved one took an expected turn for the worse sooner than we thought. The DON spent time with us ensuring our transition was peaceful. Thank You to all.

This rehab center has an acquitic therapy program. The underwater treadmill rises to the floor level so residents of all dx can use. I’ve never see this before. Amazing!

PT is great, Nursing adequate, Trach care poor, Medical care horrible. My Husband had pneumonia for 5 days before he was given any treatment. Transferred to ICU died 2 weeks later.

I was at Charlotte Health and Rehab for almost 2 month and the care was excellent. You can't bet the therapist. the DOR checked on me weekly, he is simply amazing. Sometimes the call bell response time was longer than desired, but overall my needs were met. The therapist absolutely make this building great! I would recommend that they hire more CNAs. Even with some minor concerns I would recommend this place to anyone. They truly care.

My mother was at Charlotte Health Care for a few years until my family had had enough.. The last straw was when my mother somehow got a scrape on her baby toe that went untreated until my family insisted she be seen by a wound care specialis. She was sent out to be treated weekly and some progression was being made until there was an ice storm in January and was not able to be seen for over a week. To make a long story short, her foot became gangrene while under the care at Charlotte Health Care, which resulted in emergency hospitalization and LEG amputation.. My family had had other ongoing problems with the nursing staff (some major some minor) but did not think it would get this bad. We had even met with the Director and DON on occasions with issues. Obviously, nothing being done. My family immediately moved my mother to a different long term facility after her amputation, as we knew that if she went back to Charlotte Health Care, she would not live long. I would NOT recommend Charlotte Health Care to my worst enemy or anyone's pet. This was the absolute worst experience of my family's life and we regret ever placing our mother there.

Screen Name Concerned Offspring appears to have had their nightmare as well in the Long Term Care unit. Unfortunately, our experience resulted in the death of my father. I could write a novel the length of War and Peace stating the facts and anymosity I have towards Charlotte Health Care Center, but I'll keep it brief in hopes that it will be read and strongly considered if someone is looking at Long Term options in the Charlotte area. I do this in loving memory of my father. Upon admittance into Charlotte Heath Care, my sibilings expressed concern over the care he was receiving after only being there a few short days. My mother and I went and met with the administrator and one of her associates(who I am sure was just there to witness should any legal concerns arise). We also met with the Doctor to find out what kind of healthcare my father would be receiving. After the visit, my family continued to visit and an employee of the facility became close enough to my sister that she shared photos of my father on the ground naked and in his own excrement. The employee told us in person and followed up via text about how inhumane the facility was. It was on this visit that my sisters DEMANDED that an ambulance be called so we could take him back to CMC for evaluation because it was clear that he was neglected. Upon arrival and admittance at the hospital(which we initiated), it was learned that my father had suffered Kidney Failure as a result of dehydration. He passed away approximately a week later. Tough story for me to write less than a couple of months removed and the day after Father's Day. Please take care of your loved ones and don't consider Charlotte Health Care for long term healthcare.

We chose Charlotte Health and Rehab as she has been here before. It is clean, and they really treated her well. The therapists were great. Some of the nurses are great, but some of the overnight staff was average.

When my mother-in-law entered Charlotte Healthcare Center for rehab we loved the nurses and physically therapy staff. Their food has always been horrible but the place was treating her well and is convenient for all the family to visit. She has since been placed in long term care which is another wing. Because she can do for herself they do very little for her besides provide medication. She goes days in her same clothes including sleeping in them because no one pays attention that she has not bath or changed clothes in day. We have brought this to their attention but it continues. When we make requests for evaluations it takes weeks and months and many reminders before it ever takes place. The food continues to be horrible and cold and we can not get them to stop giving her food so soft it looks like someone has already eat it once. We bring her food from outside and she has no problem eating it but we can't seem to get them to listen. The family is starting to discuss whether the convenient location is worth putting up with the other problems. I use to recommend this place but certainly can not recommend the long term care portion.

I was recently cared for and cared about at Charlotte Health and Rehab Center following a brief hospital stay. The staff were very kind to me and very attentive to my needs during this difficult time. The building was very clean, no bad odors at all. Of course, it's not like being home - nothing replaces the comfort of your own bed. The meals were great, my order was taken just like I was in a restaurant and the food came out tasty and warm. There was a Nurse Practitioner there everyday - thank goodness I didn't need them everyday, but I did see her visiting other patients. The therapy gym is huge with so many different pieces of equipment - there is something for everyone at any level of participation. Although, I hope I don't have to go back, I would not hesitate if I needed therapy. Thank you for all you did to make my stay pleasant.

Charlotte Health Care Center was great, I was impressed, and I was pleased with the care that they put for my father. They have a swimming pool ,and the food is not hospital tasting even though he is on a sodium diet. The only problem that I had is the linens in the rooms as they don’t change the linen often enough, but I really like the staff.

I'm very disappointed in this facility. My father was on his third stay here because of convenience. Unfortunately during this stay he had suffered brain damage due to a massive stroke which left him unable to care for himself. We received a call that he fell out of bed and that he was checked and they didn't find anything wrong. He's like an infant so we couldn't understand how he ended up on floor between two bed. We had facility call medics, we get to the hospital he has a knot on back of his head and front. Very upsetting, he can't talk so who knows how much pain he was in. Do not I repeat do not think twice about this facility.

DO NOT allow anyone you love to enter the doors of this place. Nursing staff is rude, inattentive, loud, uncaring and, from all I witnessed over 20 days, capable of bringing prescriptions to the room and nothing more. One of the occ therapists and one rehab person were WORLD CLASS and I hope they are smart enough to know that they could get a job anywhere they want. I have a full page of violations and complaints that I intend to escalate with the facility, my insurance company and my congressperson. Inexcusable to treat people the way they do. Could not be more disappointed.

My 80 year old mother needed some short term rehab after being in the hospital for 3 days from bladder surgery. She became week from being in bed and needed help to regain strength in her lower body. I checked her in late Friday afternoon and the nurses seemed nice, however, the facility was old and dingy. One of the knobs on the dresser was broken so my mom could not open the drawer and she did not have a working TV in her room. When I asked about the TV I was told they would fix it. I came back at 9 am the next morning I found urine on the floor right outside the bathroom and inside the bathroom. My mom told me about it right away and said she told the nurses at 3am what had happened and no one cleaned it up. Due to her surgery she is not able to hold her urine for long and needs to go right away, however she was told she needed to call a nurse for help. The nurses took too long, so she went by herself and did not make it in time. When I asked about the urine on the floor, the night shift was blamed, however they did clean it up. The TV was never fixed and when I asked about that I was told she would have to wait until Monday for the maintenance man. The food was horrible and my mom would not eat any of it. The only thing that was good was the physical therapy and occupational therapy. They were professional and helped my mom a lot. In spite of the therapy, I wanted to take my mom to my home and look for alternative therapy since the other conditions were very depressing and not conducive to her getting better. I was told that a Doctor needed to sign off and that could not happen until Monday at the earliest. If I took her out without the Doctor's consent, it would mess up her insurance coverage. In the end, I signed her out to come and be with me during the day and only took her back for therapy and bedtime. Mots of the people at the facility appeared to have neurological injuries, which is not a good place for someone who is in their right mind to be. There are no telephones in the rooms, but they have 3 cordless phones that the residents can share. I did not like that either. One evening when we were dropping mom off in the dark, there was a resident in a wheel chair out by the road. This clearly shows a lack of supervision. Most of the staff were courteous when I spoke with them, however, with all the things I saw an experienced, it made me wonder what went on when no one was watching. I got my out on Monday morning and I will never take her back there again. In the future I will personal visit anyplace I am considering for rehab even if it is only short term and I would recommend that you do the same.

We really liked Charlotte Healthcare Center. It was very clean, updated, nice, and newer-looking facility. The staff is really friendly. The only issue I had with them was communication. It was difficult to get through to speak to someone when you had a question or concern. Even though my father-in-law had a bracelet on his legs because he has slight dementia, he would go out the door. And even with the door alarm, the staff just couldn't get out quick enough to catch him, and he'd be out on the parking lot. He would want to just leave and go out the door. But the staff is really nice and friendly. He was always very clean and well-kept there. It would be nice if the residents would have an opportunity to be taken outside. He really just wanted to be able to go outside when the weather is nice. He actually thought he was in jail. But I understand there's not enough staff to do that. Also, one of the things that we're not happy about and trying to get change is a lot of his stuff have gotten missing. So we actually had to purchase a lock ourselves for his own cabinet so he won't lose things.

I visited this facility with a client while she was searching for short term rehab assistance. The facility is not the most updated I've seen, but it is clean and pretty well maintained. Staff are friendly and kind to the residents, and I observed more than one staff member calling a resident by name during my visit. The rehab staff at this facility were very knowledgeable and really took the time to talk to me, and to my client, and explain what she could expect following her surgery, how rehab would work, and what their expectations would be before she was able to return home. The dining and common areas at this facility were clean and well kept, but certainly could have used some updating (as could the facility as a whole). My client asked a couple of different residents about the quality of the food served and they both commented that it was good. My client ultimately decided on a different facility because she preferred a place a bit closer to her daughter, and a little more updated. I do think I would still recommend this facility though. It could use some improvements in the building but the quality of care and friendliness of staff are excellent here in my opinion.
