My aunt is in Schervier Rehabilitation and Nursing Center. It's very nice and clean, and the people are nice. There are some issues, but all in all, I recommend it. The social worker at the hospital recommended them. They're caring, kind, and attentive. Whenever there's a problem or whether they want to administer any medication, they call me. They called to ask me if it was OK to give the flu shot. I don't meet with the staff as often as I like, but they have a care team that includes a social worker, physical therapists, occupational therapists, dietitians, and the doctor. I only met them once because my aunt changed floors and we did a Zoom conference and they were all there giving me updates on her. They're very professional. I was there once for dinner, and it looked pretty good. I haven't observed any activity, but I called once and she was participating in music, so she couldn't come to the phone. The upkeep is excellent. They're doing some renovations on the first floor, but not the part that visitors go to. You can see it near the elevator. The facade and the layout are very nice. I like my aunt's room. She has a roommate now, but the roommate has her own section and she has her own section, so it's like she has her own private room.

• Rooms are adequate but none of the beds have railings. My mother is 88 years old and a fall risk, and they did not approve bed railings! • While the menu selection is decent, the food is not edible because it is always cold (not lukewarm) downright cold. You could understand if it became cold en route from downstairs to upstairs, but the food is freezing!! • Staffing ratio is terrible. One Nurse for an entire section with over 50-60 patients. The aides are either busy with breakfast, lunch, or dinner or taking their 45 minute break, so they do not tend to the patients regularly to change them, bathe them, walk them (if they can walk) to the bathroom, etc. • They do not allow the patients to get up from their beds even if they can walk a little because they do not want them to fall and get in trouble, which is understandable, but they will leave them in the bed all morning or for hours soiled. • My mother has been there for over 3 weeks and has seen a doctor 1x. • They arranged for my mother to go to an external doctor’s appointment by Ambulette, but they never arranged for the Ambulette and ended up calling an Uber which my mother had to struggle to get into because of the wheelchair. Lo and behold, only to get to the appointment to find out it had been cancelled. • If your loved one has a health plan, they will call you to take them out of the health plan and switch them to a different Medicaid|Medicare plan under the pretense their normal health plan will deny services.

Wonderful care. The Schervier team kept me up to date on my grandma’s care. If you are put into a tough situation with a loved one, make sure you choose a 5 star facility like schervier. I am so grateful to have found a facility that treats their patients like family.

Awful..If you care about your family member do not send them here. It takes forever to get anyone to call you or email you back. The aides are very disrespectful to the residents. The security guards are the best thing about the place as they are really good and wonderful people. The management is awful. They do little to nothing to assist the family with questions and concerns. I only experienced this place during the pandemic. But I just needed to let others know how awful this place is

My mother is a resident here and seems to be reasonably well cared for. However, with the Covid-19 situation we are not allowed to visit and have to rely on telephone to talk with our mother and have to rely of the staff to answer the phone and hand it to our mother. It has been horrible. I understand the restrictions on visitors because of the Covid-19, but that makes phone calls all more important. I call and the phone rings and rings and rings and no one answers. Then, sometimes, like today, someone picks up the phone and then hangs up. I tried three times today and someone picked up the phone and hung it up all three times. We can't visit our mother, why can't they make it easier to talk with her?

Do not send your loved ones to this horrible place. They did not handle the covid-19 crisis properly. They had workers going in and out of the premises with no gloves or mask which allowed the virus to spread inside. DISGUSTING PLACE

My mom entered on 12/27 & left on 1/16/2020. Let's start with the positives: Clean, room set up pleasant, aides are warm and friendly and PT/OT staff efficient and knowledge. Rehab is a misnomer here as total rehab is 5 hours per week!! So it functions as a nursing home but bills as rehab. LPN staffing and often one does not know what the other has set up. They put everyone who may need toileting assist in diapers as they are too short staffed to get to the bathroom on time!! Why is that?? Short staffed!! Is this rehabilitation? 1 hour of therapy. 3 meals and never never leave the wheel chair. Rehabilitation? Between meals Pts wheeled into the "tv" room with dim lighting and an aide( babysitter) sits in a chair at the rear of the room. Rehabilitation??? No, nursing home. Finally the admin staff has a defensive stance when addressing your concerns, they attempt to help(ie no diaper and NO tv room as my mom hated both was our agreement care plan going forward) So this plan worked for two days and then the diaper was back on and she was parked in the tv room. I visited every other day and she had 3 other guests covering my off days. Just NOT rehabilitation.

Because this is no longer a 3,4 or 5-star facility. They are understaffed on the floors with long term patients. I have spoken to several other family members of patients who are living in this facility..they are in agreement with me as well. Schervier Rehab & Nursing Center is UNDERSTAFFED. They have 3 aids covering 40 patients. The aids are overworked & can not give proper care to the patients.. I'm not criticizing the hard work concerning the nurse's aides. But, I am criticizing the owners & administration for not hiring & given the proper care to their long term patients. This is only a brief comment on what else I and my family have experienced at this facility. I would like to to add that my 99 yr old mother has been in this facility a little over 2 months & the conditions are just getting worse. If your planning on using this facility make sure ur constantly on top.... watching & advocating for your loved one.

I recently visited a client. To be honest I had never heard of Schervier Nursing Center. To my amazement I found it to be clean, loving and caring as confirmed by my client. The staff was respectful and courteous. I learned not only was it extremely clean, but the food served to the residents were delicious as well as fulling. The other residents appeared to be content and happy as well. I find Schervier to be a hidden gem. I would whole-heatedly recommended this Center.

What a mess.. There is NO COMMUNICATUION with any of the staff. Each staff member works for themselves and is out sourced. There are no personal relationships formed and it feels like everyone is there for a pay check.. When you ask a question.. They push you to go ask a different dept.. they won't ask.. you or the patient must ask.. STAY AWAY

This place is terrible for two distinct reasons: 1) Below average staffing which leaves residents (customers) waiting and frustrated. Schervier touts their Medicare.gov overall rating with a high number of stars but on the same Medicare.gov website for STAFFING they are BELOW average. This, patients are not treated as well as they should be. 2) On two specific occasions when visiting, aides were nasty to my relative. Overworked? Don't know but still nasty. I also heard aides speak with each other in a nice, kind tone. I wonder why they don' use that same kindness and tone with patients! I also have a friend whose mom was there and she told me she also had a similar experience. She also said admin was incompetent and rude. I was also spoken to rudely by admin business office and admissions. Unbelievable.

Despite staff being friendly, care and competence is sub par. My brother was admitted here after a two week hospital stay. Admittedly his health was still very poor and because he was considered a fall risk in the hospital he had not been out of bed for over 2 weeks. They considered him uncooperative because he wouldn’t eat and wouldn’t participate in rehab. A social worker never contacted me to discuss his goals and I know this to be protocol in rehab facilities. In fact he had pneumonia, and tested positive for the flu when they noticed he was barely breathing and called 911. Unfortunately they failed to secure his wallet, his phone and his house keys...even after a week later and numerous phone calls they failed to locate these very important items. He never recovered and died last week...I would have thought they would have called by now. I would never recommend this facility!

I was the relative of a resident who was at this facility for almost three years . While here, my loved one did everything for herself except administer her own medication. She was even asked by staff members to assist with attending to other patients. The facility did not welcome the idea of making the rooms more comfortable for my love one. In fact, they often complained about having too much in the room. Some of the staff members, at times,were disrespectful toward my love one and had to be reminded that my love one was a patient, not a staff member. They even called the police on my 78 year old love one stating my love one was a threat to the home, because my love one argued about wanting to go home. This resulted in my love one being taken to North Central Bronx Psychiatric Hospital, and the hospital found my love one to be fine. My love one went home,and fell ill immediately after leaving. While my love one was hospitalized, my family tried to get my love one back into Schervier, since my love one was used to and had friends there, and the Social Worker, Ms. Wadie, told the hospital Doctor not to accept my love one, because my love one was not suitable to be there.

M loved one is in Schervier Nursing Care Center. We chose it because they are very Catholic, friendly, helpful, and respectful to the patients as well as the families. During the week, they have games, social Sundays that he enjoys going to, and services every day. There's a monthly calendar with different activities.

Schervier is clean and nice, and I felt it is safe. The people are nice, and the grounds were pretty. The staff is lovely, caring, and very good. She plays bingo and reads.

I found a Catholic place called Schervier Nursing Care Center. They take very good care of my mom. The staff is great. They're very kind and generous with their time. My mom is happy there. They are very liberal with visiting hours. Also, any time I have any questions about anything, they answer me right away. I think the staff is very well-suited to the job that they do. My mom's room is lovely. They would invite you to bring items to make them feel more at home. They have a lot of activities. Weekly they have someone come in, and they'll have singing. There is a lady there who always does arts and crafts or bingo and things like that. They also have sing-a-longs. My mom is in the right place.

She moved in Schervier Nursing Care Center about two weeks ago, and so far it is OK. The process was just simply registering her in and filling out the paper work. They had a room available, so it was pretty easy. Her room is kind of spacious, and it's about medium sized. She can put her couch and her little things she wants in there. They have bingo and arts and crafts. The place is secure. They have two security officers that I have seen, and you have to sign in. The dining area setup is like a typical family style eating. I liked that she was happy and she likes the way her room was. There was a nice atmosphere.

My doctor recommended it and I went there. It was a beautiful and clean place. The staff and the people were very nice. But it was just difficult for me to put my mother in a facility.

When I entered the nursing home, I was greeted by an eager and polite desk receptionist and saw many residents of the nursing home in the cafeteria eating and watching TV. They did not seem very content. I then signed in at the desk and was told the room number of the resident I was looking for. My neighbor was in a room with another resident in another bed separated by a curtain. Our privacy was respected. My neighbor had a TV and an adjustable bed along with a book and a lamp. The room was very sanitary and the food seemed above average compared to most hospital foods. The staff was very courteous. There were no signs of mistreatment and my neighbor was actually content with how everything was. She was glad to see me so I could relieve her of her boredom. The only criticism I have is my questioning the interactivity between the staff and the residents on a social level. I feel that most interactions are strictly service and there is not connection on a personal level.

My first experience with Schervier was in the 90s at which time my great-grandmother was living there. She was there for about five years. The grounds and facilities always looked very well kept. During visits, she often seemed happy and high-spirited, and was always glad to see us. She spoke well of the recreational and religious services they provided, I never once heard her complain about anything, save for a single nurse that she thought was a bit too rough. They took very good care of her after she fractured her ankle, however there was an instance where she nearly died due to dehydration. We were told it was related to bronchitis she had been suffering from, but a few of us did suspect some neglect involved. When she passed away, they were very helpful with all of the arrangements, from contacting family members to even hosting the wake and funeral.In the mid-2000s, my grandmother was put into Schervier after breaking both of her hips. She was in a different part of the facility, due to the fact that she suffered from schizophrenia and was on a government income. This part of the facility was quite dirty compared to the places I remembered from my great-grandmother's stay. The nurses seemed very cold, both to her and us, as if they had no desire to be there. One day, she complained of chest pain and palpitations, but they waited for about 45 minutes to see if she improved without running any tests. When she finally did have a heart attack about an hour later, they called an ambulance to bring her to a hospital about an hour and a half away, rather than a local one. She was pronounced dead on arrival. When we received a call from Schervier about three hours later, we were quickly told by a nurse who did not even give us her name that our grandmother had "expired" (the exact word used) and that someone would have to come down with ID to make arrangements. Before any questions could be asked, she hung up. We had to call back to even find out how it happened.
