Please heed these reviews. My mom was in Garden Gate for rehab after a broken hip. The facility is filthy, food is unrecognizable. She fell twice with in 48 hours, trying to get to the bathroom. She kept complaining of arm pain after the second fall. 2 days after I moved her out of there to another rehab, found out she had broken her arm in the second fall. I truly believe she would have died in there if I hadn't gotten her out.

Absolutely filthy. The facility needs a thorough cleaning. This place should be closed down. I do not drink coffee so I ordered hot tea. My water for tea was stone cold. I had to get out of there.

Run as fast as you can away from this place. The worst experience of my life I checked myself out ama which means against medical advice. My entire butt is all bed sores bc they wouldn't answer call button. I have a rescue inhaler which they kept and when I screamed please help me I need my inhaler they just walked by. I was ready to call an ambulance to get me out of there. This facility needs to be thoroughly investigated and I'm very tempted to call the news to relay my experience to them. They fed me egg salad sandwiches for breakfast lunch and dinner. Food wasn't even edible.

My mother is in there right now. It is awful and just smells. When you go to the desk where she is for a question the staff is on their phones and looks at you like you have 2 heads. No one knows nothing. I went up there at lunch time my mother didn’t have a tray I asked they said she refused it. Well maybe she did but the visitor next to her said they didn’t even come in here. So I asked for a tray it came!!! No cover everything was ice cold like it was someone else’s leftovers. Awful. It’s not their mother it’s mine. The head of administration and people advocates should be ashamed of themselves. I am glad she is leaving at least she will eat.

Please DO NOT put your loved one in this facility. My husband was there for 10 days. Not once did they bathe him. I did it. I wouldn’t feed a rat with the food they served. The staff congregated at the nurses station ( I counted 6) but yet made my husband sit on a bed pan for 1 hr & 30 min. When I got someone to finally clean him, I was told there is a staff shortage due to Covid. The place was filthy, laundry scattered on floor in hall & to top it off my husbands room had ANTS. Disgusting, filthy, unreliable staff. Our loved ones deserve better..

the owner or president should be ashamed of this facility...my father was admitted there in january 2022 the room smelled like pee it was not cleaned very dirty. they did not take take of my dad they did not take him to any activities or shower him or clean him they just let him lie in bed when i was there it took 45 minutes for a nurses to come help me take him to the bathroom, when they come he peed himself and i got the rolling eyes , they gave my dad food that he could not chew told them to make soft foods never did everytime you need a nurse they were on their personal cell phones texting and looking at face book, or no one was at nurses station.. next the pill cart was never locked up i could of stole medicine from it next the kitchen was gross i was waiting to see the rats come one of the aid wore a shirt that had sexual sayings on it (i thought you would have a dress) code) they communication between nurse and family members was poor they never knew my dad had fracture ribs until i told them they did not give him any of his daily medicines until we told them .. i could keep going on and on, but i know nothing will get done IM REPORTING YOU TO THE HEALTH DEPARTMENT I DO NOT SEE HOW YOU GET A 5 STAR RATING FOR THIS PLEASE DO NOT SEND YOUR LOVE ONES HERE FOR ANYTHING

My father was a patient here for 12 days and it was horrible. Don't ever arrive on Friday because there are no services until Monday and no one checks on you. They left him in his wheel chair for 6 straight hours without checking on him. They wouldn't wash his hair for him. When he complained he was cold the aides threw the blanket at him. They didn't change his clothes or bed on Sunday or take him to the bathroom. He went alone to the bathroom and then got yelled at by the Aides. The PT Staff, Speech Pathologists and Nurses were good to him but the Aides were awful. When he left the facility he was scheduled for a haircut in the morning. They took him in the afternoon and didn't wash his hair before his haircut. His hair hadn't been washed in 5 days. Here is the best part, they have lost all of his clothes and sent him home in only his underwear. Who does that? The transport service couldn't believe it and made sure they gave him a lot of blankets and had the heat on in the car. When the nurse heard this from my brother she almost died. Then they lost all of his clothes. They said someone came to pick them up on Wednesday. Neither one of my brothers did. They were waiting for the call to say the clothes were ready to be picked up. They don't have a system in plan that you sign out what you are picking up? This is outrageous. My father better receive a check for those lost clothes and to send a man home in his underwear. Please if you don't have to don't send your loved ones to this facility. I am so disappointed in the management of this facility and especially the night shift. This was a horrible experience for my father and I hope no one else has to go through this.

The staff here portrays themselves as hardworking, caring and empathetic but they are far from that. Aside from a few good ones, most of them are sloppy, uncaring individuals. Call lights are on for longer then a half hour, and if a resident has to pee, they end up peeing their pants bc they waited so long. Food requested by my loved one is never what she asked for and they refuse to swap it out by saying "we are out of sunny side eggs" because they don't want to cook it over. I never receive a call from a supervisor updating me on any of my loved ones needs, care, heath, anything. I always call and leave messages with no response. EVER. It is totally unfair to someone on the outside doing the best they can. How can you do anything when you never get a phone call back? The staff needs to really think about why they are there and if they can't genuinely put patients and families first then they should go elsewhere. I myself work somewhere where families come first ALWAYS so I know the bar needs to definitely be raised here by a lot. I would never recommend.

This place is horrible! The staff was totally uncaring, ;very sloppy appearances which was reflected in the care they gave (or didn't).Call light was ignored for very long times-45 minutes was the norm. Food was horrible,Maintenance was very poor. I went there for in-patient physical therapy. The therapists were uncaring and amateurish.The one who thinks she's in charge of the therapy dept has a very bad attitude.All she did was keep her nose in her cell phone during my useless PT sessions.Worst 4 weeks of my life.DON'T GO THERE!

Without other options our loved one needed care and this was our only option. First, admitted to the "rehab" section. As much as the staff tried to work with our loved one, most of the time it was fruitile, as forcing someone to do something that they absolutely cannot do, is cruel. The room was filthy and we have swifter pads to prove it. Wet items were found left on the floor. Room are cramped for two people, barely giving each person 2 feet on either side of their beds. Some staff I will say was pretty good, to work with what they had, but the worst was the overnight staff. I spoke with anonymity to some day nurses who agreed that the overnight staff was useless and coming on their shifts, they pretty much had to cover the overnight problems plus their day work. I spoke with housekeeping on their cleaning schedule and was assured the rooms and halls were cleaned daily. Yeah...I went three days in a row and saw "remains" from an incontinent guest on the hallway floor with footprints in it. UGH! It's hard on loved one to leave their family member in a situation like this. But sometimes their care is beyond their capabilities and depend on professionals to care for them. When my loved one passed and the undertaker came to take care of things, he heard someone yelling for help and went to the nurses station to address this. He was ignored. Afterwards he told me he was appalled at this place. I always say...treat people as YOU want to be treated, and someday some of these workers may be here and then they will see what they did to their patients. And I am sure they won't like it either

horrible Facility my family members call light was on atleast 15minutes on the heart rehab floor so I stood outside the room and witnessed atleast 6 call lights on . staffing seems to be a big issue here a nurse was running from room to room I told her my family member needed to use the bathroom her response was I have 2 people on The toilet right 15 minutes go by she comes in I asked why It took 30 minutes her response was she’s working the whole hall by herself. This is unfair to the residents and my family member who was forced to go in their pants doesn't seem like the facility cares about the residents or their employees I gave The facility 1 star Because the nurse seemed to try to toilet the residents

Terrible place but unfortunately there is nothing you can do for your loved one. My family has tried for over a year. It’s usually dirty, the patients are neglected and dog food is better than the food served. The rehab section is different from long term so don’t confuse the comments of some. Long term for them is just waiting for patients to die. I cannot believe that this is considered a top rated nursing home. I feel sadness for all seniors living here. Wish our politicians would wake up and help our elderly ☹️

Susan should not work with anyone. My mother in law is 87 years old most of her children but not all are out of town and we have all heard how condicending and uncompasionate she is. Another note the food is horrible. I have viewed pictures of the "chicken and rice" wich in all actuality turned out to be chopped chicken. She personally did not need chopped up food and thought she was going to recieve fried chicken. She had a doppler preformed and the "staff" left her sitting there in her underware for 4 hours, she was right across from the nursing station! Final note they "lost" quite a few of her clothing items. All in all we expect your facility and others like it to respect and care for our live ones. You should rethink how you care for your patients. It will end up being a law suite.

Garden Gate Health Care Facility looks good. The staff provides everything she needs. They have all kinds of activities like Bingo, games, TV, newspapers, singing, and music coming in once a week. My mom's room is a little old-fashioned but it's adequate. The place looks professional. It's extremely high cost. You have to be at the Marriott Hotel to get your money's worth there.

My Mom was in this facility 12/5/2018 thru 12/28/2018. It was very dirty. My mom, who was bedridden for her stay was not cleaned often and many times her call light was ignored for long periods of time. I visited her daily and every day I would clean her room and found used gloves and towels on the floor or on her sink. Her garbage was often over flowing. I discussed this matter several times with the staff and although they agreed with me that it was unacceptable, nothing was ever done to change it.

On March 13, 2008 my fater's wife filled out a specific menu request for my father's meals. To date he has not received what was ordered. Thursday I talked with the charge person and she said it she entered it in the computer and he was getting what mom ordered. Last night my brother was there and still the food ordered was not delivered. He does refuse a lot of food but the family has taken control of that. We feel that even if he just picks at what is there it is better than nothing. We are supplying Insure for nutrition and he drinks that. Also could someone monitor his water intake. He is not drinking enough and his hand is swelling.

Rude nurses. Don’t answer patients who need help. The nurses are fast to respond with not allowing them to make any choices for themselves. I seen a women in her 90 ask to just lay down and rest and she was told no it is almost time for lunch and replied your here for rehab I guess it is ok to take there rights decisions choices away from them and tell them what they are to do. She only asked for a rest after being up half the day. They crowd the tv room with patients I guess this is a way not to allow them to turn lights on and ask for help. I truly am so hurt to see patients get treated like this. Sad

My uncle was at Garden GAte and had excellent care. It was truly a blessing for my aunt and uncle. The place was neat and clean. Food was delicious.

This is a terrible facility. The aides are rude and the nurses incompetent. The rooms are small, hallways cluttered, and it's either too hot or too cold. The aides don't want to do work. My mother was there and she would be changed once a shift unless I told an aide. Even then she had to wait a significant amount of time. They were constantly understaffed which is probably why she wasn't changed often and explains the aides attitudes. I heard from various staff, it was also a bad place to work for. The food is terrible. They claim to be a 5 star facility but, I don't see it. Overall 0/10 stars. I wouldn't recommend them to anyone.

I was at Garden Gate for rehabilitation after back surgery. The staff was great and the facility was clean and welcoming. I definitely recommend Garden Gate for therapy.

It's hard to believe this facility has received some good ratings. I guess that's just a sign of how bad many nursing homes are. My father was at Garden Gate a couple of years ago. His room was tiny, dark, and depressing and looked out onto the wall of another building. In December, someone kept opening the window in his room and leaving it open (possibly to smoke?). Of course, the staff denied it. We found his pills unswallowed in his mouth. He was brought to a freezing cold dining room dressed in just a light gown, and I would find him sitting in a wheelchair, two feet from the table, unable to even reach his food. The staff explained the cold dining room by saying that room was added on later and "it's always like this". The wound care he received appeared inadequate and the doctor too rushed. One of the nurses was fairly good. Hallways are cramped and cluttered and too many silly holiday decorations didn't help. I can only hope the administrators of this place have the opportunity to be patients/residents here someday.

My Mom is in Garden Gate for broken femur, she has to have the hoyer lift to get out of bed and to use bedpan, sometimes she sits for an hour waiting. Aides do not want to work, food is terrible, and it is not very clean, with tiny rooms. Would not advise anyone to be their family member here. It's bad when patients have their light on for help and 2 aides walk by and say they are not responding to any of the lights. I could go on and on, but I guess you get my drift. I can't wait till survey time.

I visited Garden Gate for my mother. I didn't care for the cleanliness of it. It seemed cluttered. It didn't seem like a home-living. It was more of an institutional living, and that turned me off right away. The staff did show me around. One thing different there than where my mom is now is that they had like little individual dining rooms with maybe 15 people in it. Overall, what they could improve on is to tidy up their hallways. That was the biggest turn-off for me. I was very unimpressed. Their main hallways seemed to be cluttered.

My mother was placed there for rehab after a fall. She had been there before without any issues. This time it seemed that the staff was not very attentive. She ended up falling at night near her bed and bruised up her leg. Then the next day was taken to the the hospital when throwing up and incoherent. Turned out she had been overmedicated with her epilepsy pills and the doctor at the hospital told us she was "toxic" which was causing her blood pressure to sky rocket. Needless to say she will not be going back there!

Garden Gate Health Care Facility was the first place that would take my mom for rehab, and she's been there ever since. It is an older facility; a medium scale for cleanliness and the look of it. The service is satisfactory and activities are great. However, the rooms are too small. They can make it bigger and cleaner. The food is terrible too. They could have a better menu and food choices, like more fresh food. In addition, the air quality is not good; it's either too hot or too cold.
