It would have been nice to give the five star review that the staff and lovely facility deserve, however the aftercare/ follow up service deserves zero stars. My 85 year old mother was released from the facility about ten days ago needing and entitled to PT and a visiting aide, to be set up by Brooklyn Center. I can’t count how many messages I’ve left for the social worker with not one call in response. My last message basically ended with a plea to get at least a response and direction as to who I need to talk to if not them. I’m beyond frustrated and angry.

They don't take care of the residents as they should. Residents are often left in the day room staring & looking out in space because of lack of person to person contact with them. They don't clean & bath them properly, they don't check on patients when their in there rooms anything can happen. staff are just there for paycheck & that's it.

My father has been in Brooklyn Center for Rehabilitation and Healthcare since January. They're providing pretty good care. His initial problem was he fell and hit his head, so he had a brain trauma. I don't think that they were equipped to deal with that, but as far as the physical therapy for his hip, they were great with that. The nurses were great. Everybody is pretty nice. The place is clean, but you can't get inside to really see and say much because of COVID. They just started opening up for in-person visits, so it was hard to really get an inside view of it. As far as talking to people on the phone, it was fine. He was unhappy with having the food puréed, but he just got upgraded to regular food. He told my aunt on his video call that he had some chicken that was really good. I believe the food is very well thought out per patient's needs. He ended up in a private room, and he likes it. It seems like a really nice and clean facility. I haven't been able to see inside myself, other than the foyer where the receptionist area is. The front seems fine. When we do video calls, the room seems fine.
