I am a friend or relative of a current/past resident

My uncle is in Woodbury Health Care Center. He is in memory care. There is no access to the outside for the residents by themselves. It is an institutional environment. The staff seems inexperienced. He has one roommate. Four people share a bathroom. Nothing's been remodeled for 20 or 30 years. The building was built a long time ago. They have some activities, but he hasn't participated in them. They have an activities individual that does different things for them. They need more experienced staff, which is hard right now. The facilities need to be remodeled. He gets offered a shower once a week. Whether they're using the bathroom or not, just to share it with three other people seems unrealistic nowadays, especially with people that are in that type of a situation and have issues with their memory. He was in transitional care there, and the transitional care was much better.

They seemed to be short staffed while a member of my family was there. The care plan was often not followed and when brought up, we received assurance that it would be followed, it was just lip service. There was a medication error that was very egregious, staff needs to read charts and patient care plans. Family basically had to tell staff what our loved one could do, not do, thank goodness we were there. During a phone conversation which was on speaker, I heard a resident yelling for help and it just continued on and on. It was very disturbing to me. My family member said the yelling has affected his/her sleep and is often ignored. Fortunately, our family member was able to leave. I would never recommend this facility. Many of the staff cut corners and don’t seem to care. Not much warmth or compassion going on in that place. The administrator is condescending.

My mom was in Woodbury Health Center. It was for rehab. They provided meals, lodging, and medication. The staff couldn't have been nicer. She loved it there. It was very clean, very organized, the staff was very friendly, and the food was good. They helped her with bathing, dispensing meds, clothing, and they were very attentive. The facility was up to date, they had central visiting areas, big commons, and a big cafeteria for eating. The location was easy to get to.

My friend was at Woodbury Senior Living for skilled nursing. The staff was very attentive. I actually sat with him while they had dinner one night, and the food seemed reasonable. He's very, very pleased with the physical therapy he received. It's a little older facility, but it's well maintained.

I visited this facility

My 55 yr old husband stayed here to get rehab following a broken leg. We would never stay here again, or allow a loved one to stay here. Thankfully, my husband was very articulate and could speak for himself. I am concerned for those who are totally dependent on staff to identify basic needs like pain med administration, toileting, and bathing. He was in terrible pain, and would have to call someone to give him a pain pill. It took between 15 min and 1 hour for them to answer, so he learned to call ahead of time. He was there for 4 days before someone assisted with a bath, that person was me, because we were told they are too busy. Therapy was okay, but so randomly scheduled. He could not count on getting therapy at a specific time of day. Also OT time was wasted on him, as one of the exercises was to use the yellow pages and a telephone to call a paint store to buy a gallon of paint. My husband had no cognitive issues what-so-ever as he was there due to breaking his leg. That therapy time was wasted on him, and could of better served another resident. My husband was bored and started tracking how often they got his ordered food right. One has to fill out a menu with food choices for the day. I do not know why they bother because it was consistently wrong. I am glad that we could experience this place enough to know that it is NOT a good place for us or any of our loved ones.

My mom was at Woodbury Health Care Center for a short time. We thought it was a good, nice place. However, my mom ended up going into the hospital. My mom used to tell us that when she would turn on her call light, she would have to wait an hour for somebody to come and get her. She ended up going to the hospital center with a bladder infection because she had to hold her urine. They didn't respond to people in a timely manner. The place looked nice. The activities were good. They had bingo and church. My mom liked the therapy, but she did not like the food. Some of the staff members were very nice. However, one time when we were there, there were two who were sitting out in the hallways for an hour having their own conversation while the call lights were going off. They were more interested in their own social interaction than working with the people.

Staff are awesome when it comes to activities and parties. The staff always make residents feel included when they don't have family members to celebrate with. It is very warming and heartfelt to see. They threw a fabulous Christmas party. Could be more organization when it comes to the kitchen. Barry, Sherri and Mary are absolutely wonderful, they are always there to talk to me about my husband and his care. They are so patient.

I am/was a resident of this facility

Physical therapy and occupational therapy are wonderful. If you would like to participate in activities, they will bring you to and from. Most transitional care residents don't like to participate because they are just here for therapy and don't have time. We do our own groups, like cards and puzzles together. My stay has been so far enjoyable. When I ask for help, I get it. We are understaffed in a lot of areas, but it's hard to know when you're goimg to need extra help. You'd hate to come to work and be sent home. I love how they tried to get to know me and my hardships, challenges and experiences. They have many different worship services available, and you can participate in any of them. My husband is in memory care and they work very hard to make sure we are matched up at activities. We've been to worship 3 times a week, bands and performances, and art activities. They do everything they can to make sure we have quality time together. He recognizes me again and knows im his wife. He is happy and that's my main concern.

I visited this facility

We visited a family friend who is dying. When we came in our friend could not have reached her call button because it was on the floor. She was not able to reach any liquid refreshment because the tray table was off in the corner. She was agitated and uncomfortable. The odor of feces and urine was strong. We actually noticed it on our clothes after we left the building. Parking was limited and most spaces were being used by the staff. I would not recommend this facility to anyone.

From day one, this place struggled to get it right. Both my parents were there for transitional care and we chose this place because they could be together. Transitional care paid through Medicare doesn't have the beautiful, fancy rooms and services, these are for private pay. Don't be fooled. No TV's in every room! Sent my Mom home without meds and without calling in prescriptions. Still trying to do the legwork on that. They claim they send back to the pharmacy which is unbelievable. Tried calling nursing supervisor and now my calls aren't returned (surprise). Don't call during meal time and don't press the call button if you need help in your room then either. Understaffed and not professional 95% of the time.

The aides are clearly understaffed it repeatedly took more than 15 minutes to answer the call button, it is always about 90 degrees in the hallways and freezing in the rooms hasn't been remodeled for 37 years so yeah, it's old and i would not recommend putting my worst enemy there.

My mom received hospice care at Woodbury Health Care Center during the final weeks of her life. The staff was extremely caring, supporting her with kindness and compassion as well as giving our family the space we needed to accompany her in the ways we wanted to. The Woodbury staff worked well with the hospice team members coming in to care for my mom. The nursing assistants were dedicated to her comfort, the nurses responsive to her changing needs, the chaplain was very present to Mom and to us, and other staff came by to communicate their support as she lived her final days. She received several visits from the integrative therapies staff, including one special visit where the music therapist played a harp and my mom received a foot massage and Healing Touch. Our family was very grateful for everyone's care, and feel my mom had a very peaceful passing.

The most important thing we would like you to know is how well all the staff here treated my dad, and us family members too.We can not say enough good things about how we were treated. Staff went out of their way to make the stay here very pleasant. The staff was very Knowledgeable and worked hard to help my dad to get better. All the staff members were extremely helpful and friendly. Unfortunately we did not catch everyone's name who helped during the stay but the fine staff members you have working here Emira housekeeping kept the room extremely clean and orderly. Brian aide , does an extremely good job. He was always asking if we needed anything and was friendly. Cori from occupational therapy immediately put my dad and us family at ease with her wonderful personality and sense of humor that helped make the therapy be very productive. She suggested using a weight on my dad's arm to help control his familiar tremors while eating. It has worked very well. Juanita Occupational therapy also did a wonderful job. Charine a aide helped make the stay pleasant and was always helping with any needs we had. Patty an aide is a fantastic person and on thanksgiving day out family dinner plans didn't work out so Patty went out of her way to set up a lovely table in the dinning room that we were able to use to eat the food we had pre-arranged from Tinucci's. A day that looked like it wasn't going to be a good day turned out to be a wonderful family holiday celebration thanks to Patty!!!!! Dr. Parmar did an excellent job , She kept us informed on my dad's progress and helped us on 12-6-16 by ordering a test to make sure my dad was hydrated enough before going home. Rick social services did and excellent job in guiding us through system and setting up home health care. Amanda nurse was one of the first staff members we encountered when we can here. She went out of her way to make us comfortable here and provided for my Dad's medical needs. She was always checking to see if there was anything we needed. I know there are more staff members who went out of their way to help us but unfortunately I didn't catch their names. I'm sorry they wont be specifically get the well deserved recognition and thanks they deserve. I apologize for any misspelling of names or titles of the staff members mentioned above. Food . The food was excellent and my dad enjoyed it very much. It was so good he ate things that normally he wouldn't eat at home. The parking at the facility was good. Even when there were many vehicles here there were always a place to park. Bottom line is we want to thank everyone here very much for the wonderful professional care you provided for us. We would definitely tell others about how well people are treated her! Physical and occupational therapy did an excellent job. My dad's strength, balance and stamina improved greatly. We can not think of any negative thing about my dad's stay here. Within a very short time her, we knew my dad was at a safe professional friendly facility that would de everything in their power to help my dad get better, be comfortable and feel like he's at home.

Hi My mother is in the nursing home here and so sad to say this Nursing Home needs desperate help very poor on every thing the aides need to step it up if you can find one i am family of 10 and we all go see my mom everyday because of the care here and when we do see her we have to change her bedding because no one does. The food is Horrible here and this place is so so dirty. What can i say it is so depressing. She is on 2nd floor and the kitchen is so dirty all the time the cupboards are so sticky i don't think the ever clean it. They all seem so lonely these Seniors deserve better care and weather they are short of help some one needs to pay attention. Please Make this floor a better place to visit and give theses senior more of your time. I always feel bad to leave because of the bad care here.

Mom in transitional care. She was in bathroom yelling for help. The emergency call rope was broke in her bathroom and therefore nobody came to help her. Luckily I was coming down the hall and heard her yelling for help. Assisted living did not work well for her either. She did not get enough help when she really needed it.

My mother was here for 15 months before she died. She had moved from senior living to assisted living and then to the nursing home. No complaints until she came to the nursing home part. First complaint is roaming male residents that would come into her room. My mother was non verbal by this time and could have done nothing to protect herself had it been necessary. Staff didn't seem to know how to answer my complaint. Second, as my mother lay dying, she was left in her double room and the entire family had to put up with a loud and obnoxious roommate while we were trying to say our final good bye. Finally, as my mother lay unresponsive and dying, someone took her gold cross necklace. This necklace was made from her husbands rings after he died. My mother wore it 24/7 for 17 years and NEVER took it off. But as soon as she became unresponsive it disappeared. Some of the staff were caring people others were not.

Woodbury Health Care Center is good. It is kind of a la carte depending on what services you need. The staff seems to be genuine people. They like their job. My dad is in this facility. The room is small; it's not too bad. The dining room is close to his room, so it is good. Their dining room kind of reminds you like an old-fashioned cafeteria. It is kind of a quaint little diner. They have a pastor that comes in Wednesday and Saturday, so we don't have to drive my dad all over town going to different churches, and that's the only social interaction that he does so far. He has only been there for two months.

My mother is a resident. We sit on her bed waiting at least 20 min. Before a nurse or aid comes after pushing her emergency button. They go threw my mothers personal things and throw away papers that they think are trash. One time I left my mothers last will and testament in her three drawer cabinet. It got thrown away along with that Sundays news paper and it was monday. They all back each other up when finger pointing happens. They wouldn't give my mother any money when she asked for a twenty.because her balance said 6 dollars .after she has been a rresident for a year . which is 80,000 dollars they have been paid for rent.

My husband was there to recover from surgery; care was okay but he was put into a room with a patient who listened nonstop to very loud tv, which proved to be intolerable. Staff either could not or would not put a stop to this, so only alternative was to pay extra for private room. They forced me to pay for a month, knowing he was there for only a wk or 10 days. I gave them a check for $1050; his bill came to $385. 2 1/2 months later, I have not received a refund. I consider this nothing less than thievery.

Stay away. The transitional care floor is OK, but once my mom was moved to the 2nd floor it was a constant battle with the aides and management. 45 minutes for her call light to be answered. Unacceptable.

Both my parents were in transitional care at Woodbury Health Care Center. The care there was good. The hospice was great. The staff was wonderful, they were all very caring. Each staff person has a lot of people to take care of, so they're not always right on time with meds, but it's taken care of. They need to hire more staff. They have all levels of care there, and they have activities going on everyday.

Very disappointing and upsetting experience with this facility. Not enough staff, can't get meds in a reasonable amount of time, patients requiring help are out of luck. Hope I never have to go here! Exasperating is all I can say.

When I visit my dear friend she is lying in bedwatching TV. I always ask a nurse helper to get her up in a chair so we can go visit in a sunny room. I will be honest and say staff assistance is usually a problem they seem to be never available. Food seems ok but seconds are not offered. My friend seems depressed so I visit weekly to offer a chance to visit and catch up.

My grandmoother was a resident of this facility for seven months. She was well cared for by a caring and thoughtful staff. My grandmother had worked as an LPN at a long term care facility, so she had high standards for her care. The only negative was a lack of security. She was harrassed by a male resident who was confused and thought she was in his bed. He was angry and violent, she was not the first resident he had harrassed this way. I think that there could have been better control over residents with a history of acting out at other residents.
