My wife stayed in Belle Fountain Nursing & Rehabilitation Center for rehab and they had a pretty good service. Their PT OT was very good. The staff, they are overworked and understaffed. It's hard to pay attention to the people who need it more than they can give. The facilities, it's easy to find where you're going.

Used to be a wonderful place. Administration and care has went down so bad.. We've had 3 administrators in the past year.. No ones happy here. No one talks or smiles anymore.. Residents are just staying in their rooms not joining in no activities or anything since the new administrator came. We had a great activities department got ripped apart for no reason.. we're short staffed all the time.. I actually live here.

Ridiculous place. Unable to execute basic standard of care.. pain management, wound care or infection control. After expressing concerns on multiple occasions with director of nursing and administrator, nothing changed. Ineffective upper management. No one ever answers call lights. You hear the residents calling for help and no one around. I could go on and on. Don’t know what they consider nourishment. Most of it looks unappetizing and wouldn’t feed my dog.

Belle Fountain is severally understaffed. Patient care is non existent. My husband was there three weeks. Not one shower. Blood stain on the sheets stayed two weeks…sheets never changed. My husband was in severe pain. Requested pain medication. Nurses said they couldn’t reach the doctor. The protocol is to call the next doctor on the list if the first on call doctor isn’t available. There was an investigation started but I don’t know the results. My husband was actively dying with this pain. We took him home on hospice and he died the next day. The nurses couldn’t have put forth effort to help him at Belle Fountain? No compassion. Two pluses 1) the hall floors were cleaned daily. But the bleach smell was nauseating and adversely affected my asthma; 2) the outside wound care nurses were knowledgeable and compassionate. My husband was in another facility earlier this year. It was a palace compared to Belle Fountain. Final words….don’t go to Belle Fountain!

My mom is still at Belle Fountain Nursing & Rehab. The physical therapy and occupational therapy department is outstanding. I've never seen anyone like this. All of the staff are very caring and very kind. The problem that we have is they don't always follow through. Like we paid $42 on Saturday to have my mom's haircut and styled, and then Saturday, my mom had a shower. We said, 'Please, don't get her hair wet.' 'Oh, no, we wouldn't do that.' They said, 'We have shower caps in there.' However, four hours after she had her hair done, they got her all wet and they said, 'We'll fix that.' That was Saturday and this is now Wednesday, and they still haven't done anything. They're very nice, but I think they're shorthanded. The time to get someone into your room to put someone on a bedpan, you could wait 20 to 30 minutes. They care and they would try but they're doing double duty. I love the food, but my mom hated it. I said, 'Mom this is good,' but she doesn't like it. Her roommate liked it, and my sister and I thought, 'Hey, this is pretty good food.' The other thing is my mom is on a low-salt diet, and yesterday, they brought her a hot dog and potato chips. Those are two things she shouldn't have, so we asked for a substitute, and they brought it, but she shouldn't have gotten that to start with. For activities, they have bingo, movies, iced tea social thing outside, and there's a Fourth of July party. The problem we encountered with my mother (and it wasn't their fault) is that she would go someplace, and then she would say, 'I need to use the restroom, or I need to use the bedpan.' So, she didn't get the full experience, but they were good. There's no central air conditioning, at least not in my mom's room, so they brought in a window air conditioner. It hooked up to the same cord as the TV, but then the TV kept going out because the cord would loosen. And the only thing my mom wants on TV is Channel 4 so she can watch Wheel of Fortune and Jeopardy! When they did the air conditioning, her television went out and they were supposed to reprogram it. But that has been six days now and it hasn't been reprogrammed.

There is very little care or treatment from staff at this facility. My sister was brought to Belle Fountain on 6/14 for rehab after a fall a week prior to that. She has the beginning stages of dementia and was put in a room far from the nursing station. While at Belle, she developed 2 bed sores that weren’t attended to until I took a picture of one and brought it to the Administration’s attention. She also had a UTI, was dehydrated and unattended to when needing to go to the bathroom. The lack of care is awful. They are clearly short-staffed and must be trying to cut costs. Thankfully, she was moved from this facility and taken to a hospital that is giving her the care that she requires. The lack of treatment and attention at Belle is disconcerting and inhumane. The only good thing I can say is that they pay attention to cleaning the facility. Just realize that just because it is clean does not mean they will give the care needed to your loved one.

DO NOT allow your loved one's to go to this facility. My father was here to rehabilitate from a broken pelvis. They provided medication and food and that is about it. They rarely answered the call light, so he was forced to hold it hoping for their assistance and usually ended up using a hand held urinal. He left there with a UTI. They did not bathe/shower him as scheduled/planned. The room was hot. None of the nurses or administrators helped with any complaints. A doctor told me the squeaky wheel gets the oil. My father is 85 years old and NOT a squeaky wheel so he was basically ignored by staff when family was not pleasant. They didnt provide water for him on a regular basis. Terrible place! He was there for 3 weeks and I had a pleasant experience with 2 CNAs. The social worker was not helpful and did not advocate for the patient. The building administrator stopped taking my calls and I received no additional support and nothing improved after sharing several concerns about his care.

Though it is a very nice place, DO NOT leave your loved ones here if you actually love them. The PT crew seems to be on their game, the rest of them other than the night shift are questionable. Wasn't aware I'd be sharing a room. Long waits for anything. They bring what THEY want you to eat. They play loud music all day, so getting any bonus rest is out of the question. Definitely not the best place to be when trying to recover and get care.

My Mother was at BelleFountain For 3 Weeks..She didn’t want to go there because she wanted to return home to her assisted living facility after a hospital stay..But once she was at BelleFountain she liked it and was very well taken care of.. I would recommend this facility to Family & Friends...Everyone is so nice & very compassionate!!

My cousin gets physical therapy at Belle Fountain Nursing & Rehabilitation Center, and it's minimal. They get her meals and they help her get to and from the bathroom. She shares a room with another lady. They have a calendar and I tried to bring her to one of the events but nobody knew where it was or how to find it. When we went around looking for it, they said, "Oh, evidently they decided not to do it today." So, it's hit or miss. There's a group that plays cards and she joins them. They showed a movie, which she enjoyed, and they did have a music thing that she enjoyed. The facility looks good. They have screwed her doctors' appointments and her transportation and a few other things. As soon as the Medicare ran out, they're kicking her out, even though they said she needed another week's therapy. They were supposed to be getting her to the point where she could live by herself comfortably again and she's not there yet. Their phone system is the first thing that they need to improve on. If you call the patient's room, it takes you to a voicemail, but there's no way to pick up that voicemail. People were leaving my cousin messages and she had no way to pick them up. I have left message upon message upon message for different staff members, and they tell me that they never received them. It's very frustrating when you can't get through. Sadly, it depends on the staff again. Some days, if she rings a bell because she needs to use the bathroom or get a bedpan, they come. This morning, she waited two-and-a-half hours for someone to come and bring her a bedpan. Yesterday, her roommate sat on the toilet for over an hour waiting for someone to help her. It depends on the day, because I've seen them be very efficient as well. The good ones are really, really good, but the ones that aren't are really bringing them down. There were a lot of nice people. I've met my share of really kind and efficient people there. Unfortunately, I've also experienced the other.

This place is shady. They moved my mother to a facility 40 miles away in the middle of the night with out letting anyone know. Never saw the same nurse or aide twice, had to beg for information.

My mom was at Belle Fountain Nursing & Rehabilitation Center before, and they did an excellent job working with her and getting her to a point where she was comfortable. We had to quickly get her moved to a nursing home environment again. They were extremely helpful and did everything they could to make sure we could move her to that same location where she had rehab. Their staff is wonderful. They're very helpful and informative. They make sure we know what's going on and how she's doing. They have a nice sitting area that is being used a little bit more now since some COVID measures have lightened up a little bit. Everything is functional. The rooms are maintained and cleaned every day. They have a nice lunch area. They also have an area where the residents can watch movies, play games, and do activities.

I was at the Belle Fountain Nursing & Rehabilitation Center just this year. Some of the aides they have there could be quite scary. They require them to work 3 days straight and a 12-hour shift. The first day is good, the second day is so-so, but they get so exhausted on the third day that it gets scary. They're almost rude. So, you can't blame them because they have to work those shifts, and that's because of the people that bought it. That's unfair and there's a big turnover. The food has gone down bad too, but their physical therapy was the best. They push you and push you if you don't think you can do it. When you leave there, you're strong.

Belle Fountain Nursing & Rehab was very nice. The staff was very good. The food was good. It was a nice facility. I don't know about activities because my friend couldn't get out of bed. She had fallen and broken her hip. I'm sure they had something because when you went in, it had a nice big family room or something. They had activities, but she wasn't capable to use them. The room was neat and clean. There was plenty of room, and it was nicely spaced.

Terrible place. My father was sent unresponsive to hospital. Six hours later when I come for a visit…they let me know and what’s worse they didn’t know where he was sent. Director Holly is useless if you want any answers. Stay away from this place!!

My brother is in Belle Fountain Nursing & Rehabilitation because he kept falling. He was there before also. He likes it there, the food is good, it's a nice place, and they do their job. The staff is wonderful, they're really good to him and everybody. You get to order what food you want, it's all been pretty good, and he likes it.

My husband was in this place for a month. In that time he got so sick he had to go to the hospital. I asked that they send him there at 4 pm. They deemed it non emergency and he didn't get to the hospital until midnight! He arrived there with kidney numbers at 6, complete confusion. a bowel obstruction, a severe UTI, and SEPSIS! The ER doc said he was a pretty sick guy. Is this what you want for your loved one? It's too bad that this medical care was so bad because the Physical Therapy was awesome!

Had a family member in here for 3 weeks... most people was friendly but... a couple nurse aides told my family member they had no bed side toilets... and fussed at them for having diarrhea and said they needed to call some one so they could get a bed pan or help getting to the toilet... but when you call someone it takes 20 to 45 minutes at times for someone to show up… These nurse aids also said that my family member stunk ... that made my family member shamed... you have to share rooms so you don’t get any privacy... they discharged my family member without doing another chest x-ray , it had been 10 days since the last one... you could here something was in there chest... and sent them home with a low grade fever... a little over 24 hours had to call EMS And back to the hospital... she also picked up a resistant bacteria in her urine… They said they probably picked it up from this nursing home from it not being properly sanitized... it was hot in there too... not happy about this place... when we left no one came to take us out... we had to do it ourselves... I know no Nursing Home is perfect… But I wish things were different… I’m very sad ...

My mother-in-law passed away recently at Belle Fountain. She had been in 5 facilities over the past 5 years. Belle Fountain was by far the best one she had been in. The place is immaculate, the cleaning staff is in constant motion. Belle Fountain does not have the normal "nursing home" smell. The food was good and snacks were constantly being dropped off. The nursing staff was wonderful, meds given on time, they actually cared about her comfort. We were there every day and 18 hours a day the last week, so we got to observe a lot. By far the best nursing home around. Thanks to everyone who help, (which was everyone), it was appreciated and will be remembered.

My sister was a resident in this facility for about 3 weeks. While I think the care was average. You will always have good employees and less than stellar employees. My concern was with the communication btwn Social Services, Discharge Planning and the Resident (My sister) I was the one who they called for everything. They stated it was their policy to that a family member MUST go with them to dr appointments and care conferences. MY sister was her own responsible person, why MUST I attent?? When it came to discharge planning....A JOKE!!!! My sister not only had to call but arrange her own transportation home. They called ME stated I needed to be there, because residents are very nervous about going home. When I questioned about her DME, I was told it was going to be ordered. Received a call from my sister who was told they would order It but it would be delivered to her home? Why wouldn't this be done prior to her discharge. Make sure the Wheelchair fits, works, etc. Can you just imagine 90 year having to call for her own ride, not getting a wheelchair (or walker) The nursing staff however....They were great! Underappreciated by management

These two people sound like they were not happy with the care their loved ones received. If that was the case, why leave them there. I cannot stand when people make complaints about things and do nothing else. If your love ones were treated so poorly why did you leave them there, because what they are saying isn't true. My love one received excellent care there and returned home in better shape than before she went there. These are just false accusation and this facility and they should be sued by who ever owns that facility.

I would avoid this rehab also, if you need any kind of medical care. My mother was in there with a broken arm, and needed assistants with any type of physical movement. Our family members were there everyday, if not, she would not have been taken care of. Most of the aids there are wonderful, kind and caring. The nurses on the other hand, do not have time to care for their patients. They hand out meds and that is it. And good luck getting your medication on time. It is really sad. This was my moms 3rd visit there in the last 4 years. The 1st two visits weren't as bad, but this last visit was horrible. If you are young and can take of yourself, it might be ok. But I feel sorry for the elderly that need medical attention, because you won't get it there. My mom progressively got worse during her stay there, and whenever you tried talking to the nurse, or asking the nurse for help, she would say she was busy. My mom will not be going back there, because unfortunately she passed away. If any of you are considering sending a family member there, please do your homework first, and make sure they are able to take care of their self, if not, this might not be the best place for them.

I would avoid this center at all cost. My mother was commissioned her after a broken shoulder. She has several medications, is diabetic, and has low blood pressure. The Belle Fountain Nursing and Rehabilitation center ignored instructions for her medication, missed several doses PER DAY, and claimed other meds were under control until she became ill - at which point they admitted their mistake. They are vastly understaffed and we worried for her care every day she was inside. Unfortunately, there was no way to remove her. She stayed 1.5 weeks. It felt like a lifetime.
